Compression of the subclavian vessels and brachial plexus at the superior aperture of the chest, most commonly against the first rib. Other terms for this syndrome include scalenus anticus syndrome, costoclavicular syndrome, hyperabduction syndrome, cervical rib syndrome, and first thoracic rib syndrome.

Alejandro J. Katz, MD, OMD, LAc, QME Thoracic Outlet Syndrome Frequently orthopedists, neurologists or chiropractors refer patients with a diagnosis of thoracic outlet syndrome. This article will help you know what it is and how to treat it. The symptoms of thoracic outlet syndrome (TOS) are pain, numbness and tingling of the upper extremity. The compression of the brachial plexus, subclavian artery and vein between the clavicle, first rib and the scalene muscles seems to be the cause. The nerves mostly affected are the radial; ulnar (90% of the cases); median; and musculoskeletal nerves. The patient's symptoms are pain/tingling/burning of the forearm/hand/fingers (third, fourth and fifth fingers). The pain medications usually prescribed include ibuprofen (600-800 mgs 2/3 times a day), naproxen (500 mgs two times/day), vioxx (12.5/25/50 mgs) or celebrex (100/200 mgs). A diagnostic test that could be used for TOS is a nerve conduction study (NCS), which will show whether an abnormality is present (the test's results are usually inconsistent).

My Orthopedist and Neurologist think I have Thoracic Outlet Syndrome. I know this is very difficult to diagnose and is often misdiagnosed with surgery being performed unnecessarily. I do have the cervical rib and lose the pulse when the arm is raised. I've had an EMG but I am now being told that it was done incorrectly so another will be done tomorrow. Is there any other way of diagnosing this? I have a lot of shoulder and neck pain as well as tingling in the hand and fingers, especially when the arm is raised at all. The neurologist says that if I did need surgery there are only four places in the US that he would send his patient. He only mentioned two, which were LSU and The Cleveland Clinic. If I'm told I have TOS I want it proven to me that I really do have it! Is there a test that will definitely show it?

Thoracic outlet syndrome is caused by rib anomalies or musculofibrous bands between the clavicle, cervical spine, and first rib. Alternatively, this may be an acquired condition due to malunited clavicle fracture or body habitus (thin or muscular). An anomalous anterior scalene muscle is the most common cause. Any condition which narrows the costoclavicular space may cause the symptom complex. These patients experience ischemic symptoms in the arms with abduction, associated with loss of the radial pulse and possibly a subclavian bruit. Complications include poststenotic aneurysm, subclavian artery thrombosis, mural thrombus with distal embolization, or venous thrombosis. Surgery can be curative.     Thoracic outlet syndromea myofascial variant: Part 3. Structural and postural considerations
    BM Sucher and DM Heath Thoracic outlet syndrome involves more than just local neurovascular compression. Myofascial release treatments and stretching exercises may be only partially or temporarily successful unless all related components of somatic dysfunction, including craniosacral mechanisms, are addressed. Structural and postural abnormalities in the frontal plane, as with a short leg, and in the sagittal plane, such as lumbopelvic imbalances, as well as neural involvement all contribute to thoracic outlet syndrome symptoms. Once segmental restrictions are treated and symptoms diminish, postural correction and strengthening exercises may be initiated. Osteopathic diagnosis and treatment of the local, regional, and remote structural problems is necessary for optimal treatment of thoracic outlet syndrome and the maintenance of a symptom-free status.

    Thoracic outlet syndromea myofascial variant: Part 1. Pathology and diagnosis
    BM Sucher Four cases of thoracic outlet syndrome are described, with observations to support a primary myofascial etiology involving the scalene and smaller pectoral muscles. It is believed that thermography can be extremely helpful as an aid in diagnosis of thoracic outlet syndrome and, when combined with Doppler blood flow studies and photoplethysmography, can help localize the site of the pathosis. Thermography is a sensitive, non-invasive test that most clearly demonstrates pathology in the hand views. Global changes throughout the entire hand suggest vascular or reflex autonomic involvement, while a more dermatomal distribution suggests specific neurologic involvement. Vigorous stretching and a unique form of myofascial release manipulation has been successful in rapidly treating patients with thoracic outlet syndrome. Follow-up thermography (perhaps hand and forearm views only) can be used to monitor response to treatment and to objectively document improvement.

    Current Treatment Options in Cardiovascular Medicine Published Abstract The clinical presentation of thoracic outlet syndrome depends on which anatomic structure is compressed in the area of the thoracic outlet ( eg , the axillary-subclavian artery, vein [Paget-Schroetter syndrome, or effort thrombosis], brachial plexus, or the sympathetic nerves). The clinical syndrome may be isolated to one or a mixture of these compressed anatomic structures. Although there are multiple compressive forces, the first rib is the common denominator, and extirpation of this structure is the 'gold' standard for therapy. The various syndromes are discussed in their clinical order of frequency. For severe nerve compression, whether upper or lower brachial plexus, the best surgical procedure is the transaxillary first rib resection, anterior scalenectomy, and neurovascular decompression.
