Syphilis down, gonorrhea up in U.S., study finds
WASHINGTON (Reuters) Syphilis rates are the lowest they have been since officials started keeping track in the United States, the government said Tuesday, but gonorrhea rates have started a troubling rise. These figures suggest that targeted information campaigns work, and that keeping people in the dark about sexually transmitted diseases gives those illnesses a chance to take hold, the Centers for Disease Control and Prevention (CDC) said in its report, released at a meeting in Milwaukee.

A Request for Redress of the Wrongs of Tuskegee
In January 1996, the Tuskegee Syphilis Study Legacy Committee met at Tuskegee University to explore how the government and the nation could publicly address the Tuskegee Syphilis Study and its impact. The final report of the Committee from May 20, 1996 is abstracted and presented in its entirety below:
Abstract of the Syphilis Study Legacy Committee
Final Report of May 20, 1996
From 1932 to 1972, 399 poor black sharecroppers in Macon County, Alabama were denied treatment for syphilis and deceived by physicians of the Unites States Public Health Service. As part of the Tuskegee Syphilis Study, designed to document the natural history of the disease, these men were told that they were being treated for "bad blood." In fact, government officials went to extreme lengths to insure that they received no therapy from any source. As reported by the New York Times on 26 July 1972, the Tuskegee Syphilis Study was revealed as "the longest nontherapeutic experiment on human beings in medical history." The Study continues to cast a long shadow over the relationship between African Americans and the bio-medical professions; it is argued that the Study is a significant factor in the low participation of African Americans in clinical trials, organ donation efforts, and routine preventive care. In view of this unacknowleged wrong and the the damage it has caused, the Tuskegee Syphilis Study Legacy Committee pursues two inseparable goals:

    What is syphilis?
Syphilis (pronounced siff-ill-iss) is a sexually transmitted disease (STD) caused by a bacteria (germ). It progresses in stages and without treatment, can lead to death.
    How is syphilis spread?
Syphilis can be spread during vaginal, anal, or oral sex through contact with an open sore or contact with a skin rash. The bacteria can enter the body through the penis, anus, vagina, mouth, or through broken skin. An infected pregnant woman can also pass the disease to her unborn child. Syphilis is not spread by contact with toilet seats, doorknobs, swimming pools, hot tubs, bathtubs, shared clothing, or eating utensils.
What are the symptoms of syphilis?
Each stage of syphilis has different symptoms. Syphilis can be spread during the first two stages of the disease. Primary Stage
A single, painless sore, called a chancre (shan-ker), appears in the first, or primary stage, about 10 days to 90 days after infection. The sore can appear on the

Italian Skeletons Reveal Old World Diseases By Rossella Lorenzi, Discovery News Columbus: Syphilis Spreader? April 12, 2004 Involving various sites throughout Italy, the study examined 688 skeletons dating from the Bronze Age to the Black Plague epidemic of 1485-1486. The remains were investigated for the presence of bony alterations characteristic of rheumatoid arthritis, gout, spondyloarthropathy and syphilis-causing organisms, called treponemes.

  • Indeed, syphilis is known to scar and deform bones. Legend holds that Columbus and his crew contracted syphilis in the New World and caused an outbreak in Europe. "Prior to Columbus, Italy was like a virgin. Rheumatoid arthritis and syphilis were not present, further supporting the contention that they are New World-derived diseases," Bruce Rothschild, the paleopathologist at the Arthritis Center of Northeast Ohio who carried the study, told Discovery News. The research matches Rothschild's previous studies, which found no evidence of syphilis in the Old World before Columbus. However, 687 skeletons from eight different populations in the New World, ranging in age from 400 to 6,000 years old, displayed signs of syphilis in bones that were at least 800 years old.

    Syphilis (Lues) - Treponema pallidum
Epidemiology: Between 1986-1990, the incidence of primary and secondary syphilis was reported to be at its highest level in forty years. Fortunately, in 1994 cases reported dropped to 450. Syphilis primarily occurs in the lower socioeconomic heterosexual population. Noteworthy is the dramatic increase in the number of reported syphilis cases among black women. From 1984-1989 the cases among black men rose 129% and among black women 192%. This is quite different from the cases reported in the late 1970's and early 1980's which primarily occurred in homosexual white men. Cases among this group has diminished dramatically, presmably because of the efforts of organizations such as Gay Men's Health Crisis which have educated people in the practice and importance of safe sex.
Treponema pallidum is a spirochete 5-15 micrometers in length. The organism is most easily seen under the microscope using dark-field examination or silver stain.

Screening for Syphilis RECOMMENDATION Routine serologic screening for syphilis is recommended for all pregnant women and for persons at increased risk of infection (see Clinical Intervention). See Chapter 62 for recommendations on counseling to prevent sexually transmitted diseases. Burden of Suffering
Syphilis is caused by infection with the bacterium Treponema pallidum which can be transmitted congenitally or by sexual contact. In 1994, 20,627 cases of primary and secondary syphilis were reported in the United States.1 The incidence of syphilis has decreased by 50% since 1990, but it is still high and now approximates 1970 rates.1 A growing proportion of cases is being reported among commercial sex workers and persons who use illicit drugs, especially those using crack cocaine and those who exchange sex for drugs.6,7 There are pronounced geographic differences in the incidence of syphilis in different communities. In recent data, nearly all counties with a high incidence of reported syphilis cases (more than 10/100,000 persons) were in large metropolitan areas or in southern states; nearly two thirds of all counties in the U.S. reported no cases of primary or secondary syphilis in the most recent year.1 The incidence of reported infections among Hispanics and blacks is 5-60 times higher than that in non-Hispanic whites.1 Individual communities may experience substantial fluctuations in incidence rates independent of national trends.

Syphilis verläuft in Stadien.
