A sore throat often makes it painful to swallow. Considerations Return to top Sore throats are common. Most of the time the soreness is worse in the morning and improves as the day progresses. Like colds, the vast majority of sore throats are caused by viral infections. This means most sore throats will NOT respond to antibiotics. Many people have a mild sore throat at the beginning of every cold. When the nose or sinuses become infected, drainage can run down the back of the throat and irritate it, especially at night. Or, the throat itself can be infected. Some viruses can cause specific types of sore throat. For example, Coxsackievirus sometimes causes blisters in the throat, especially in the late summer and early fall. Mono and the flu can also cause specific viral throat infections.

    Strep throat is an infection caused by the Streptococcus pyogenes (strep) Group A bacteria. The infection is more common in children than in adults. What are the symptoms of strep throat?
    Children with strep throat often have a fever and complain of sore throat, headache and stomachache. They may also have swollen, tender glands in the neck, or sores around the nose. Scarlet fever, a rare form of strep infection, is characterized by a sore throat, more general symptoms, such as fatigue, and a red rash on the body that feels like sandpaper. Some children can get very serious complications, such as rheumatic fever, and acute glomerulonephritis, if the infection is not treated completely with antibiotics. How is strep throat transmitted?
    The strep bacteria is found in an infected person's saliva. The infection spreads through the air when the infected person talks, coughs or sneezes. The spread of infection can be stopped by treating the infected person with an antibiotic. It is difficult to diagnose strep throat just by looking at the throat. The physician has to take a culture (or swab) of the throat to see if strep bacteria are present. If strep throat is diagnosed, the physician will prescribe an antibiotic, usually penicillin. This medication comes in the form of a pill, a liquid or an injection. If treatment is begun soon after the infection has started, the child will feel better very soon. This treatment may also prevent the serious complications that can result from strep throat.

    The group A streptococcus bacterium (Streptococcus pyogenes) is responsible for most cases of streptococcal illness. Other types (B, C, D, and G) may also cause infection. Group B streptococci cause most streptococcal infections in newborns and maternal post-labor/delivery infections. Some of the major syndromes associated with group A strep infection are:
    • streptococcal pharyngitis or "strep throat"
    • scarlet fever, most often preceded by a sore throat
    • skin infections (impetigo, cellulitis/erysipelas)
    • focal infections, limited to a particular body site, e.g., pneumonia, septic arthritis

        The signs and symptoms of strep throat are red, sore throat with white patches on tonsils, swollen lymph nodes in neck, fever, and headache. Nausea, vomiting, and abdominal pain more common in children. The illness is caused by the bacterium Streptococcus pyogenes and is spread by direct, close contact with patients via respiratory droplets (coughing or sneezing). Casual contact rarely results in transmission. Rarely, contaminated food, especially milk and milk products, can result in outbreaks. Untreated patients are most infectious for 2-3 weeks after onset of infection. Incubation period, the period after exposure and before symptoms show up, is 2-4 days. Patient is no longer infectious within 24 hours of commencing treatment.

Casano, M.D., (601) 932-5244 Jackson, MS What Causes a Sore Throat? Sore throat is a symptom of many medical disorders. Infections cause the majority of sore throats and are contagious. Infections are caused either by viruses such as the flu, the common cold, mononucleosis, or by bacteria such as strep, mycoplasma, or hemophilus. While bacteria respond to antibiotic treatment, viruses do not. Viruses: Most viral sore throats accompany flu or colds along with a stuffy, runny nose, sneezing, and generalized aches and pains. These viruses are highly contagious and spread quickly, especially in winter. The body builds antibodies that destroy the virus, a process that takes about a week. Sore throats accompany other viral infections such as measles, chicken pox, whooping cough, and croup. Canker sores and fever blisters in the throat also can be very painful. One viral infection takes much longer than a week to be cured: infectious mononucleosis, or "mono." This virus lodges in the lymph system, causing massive enlargement of the tonsils, with white patches on their surface and swollen glands in the neck, armpits, and groin. It creates a severely sore throat and, sometimes, serious breathing difficulties. It can affect the liver, leading to jaundice— yellow skin and eyes. It also causes extreme fatigue that can last six weeks or more.
