Stevens-Johnson Syndrome Definition: A variant of bullous erythema multiforme. It ranges from mild skin and mucous membrane lesions to a severe, sometimes fatal systemic disorder. Ocular symptoms include ulcerative conjunctivitis, keratitis, iritis, uveitis, and sometimes blindness. The cause of the disease is unknown.

Stevens-Johnson syndrome
DESCRIPTION: Until recently Stevens-Johnson syndrome (SJS) was considered to be the same as erythema multiforme major, a severe form of erythema multiforme in which more than one mucosal surface was involved. Now it is thought that erythema multiforme spectrum is a single entity. The milder form, also known as erythema multiforme-Hebra, either has no mucous membrane involvement, or may involve one mucous membrane. The more severe form is erythema multiforme major, involving more than one mucus membrane. In both these variants, it is a self limited hypersensitivity reaction, usually to a preceding viral infection, and has an excellent prognosis. SJS is a generalized hypersensitivity reaction, usually to a drug, in which the skin and mucus membrane lesions are early manifestation. It may progress to its more severe form, toxic epidermal necrolysis which has a high morbidity and up to 40% mortality.

Abstract
Severe skin adverse drug reactions can result in death. Toxic epidermal necrolysis (TEN) has the highest mortality (30-35%); Stevens-Johnson syndrome and transitional forms correspond to the same syndrome, but with less extensive skin detachment and a lower mortality (5-15%). Hypersensitivity syndrome, sometimes called Drug Reaction with Eosinophilia and Systemic Symptoms (DRESS), has a mortality rate evaluated at about 10%.
Drug reactions are self-limited diseases and therefore, generally treatment is symptomatic. Prompt diagnosis, identification of, and early withdrawal of all suspect drugs are the most important preliminaries. The management of the patients must be undertaken in specialized intensive care units, with the same main types of therapy as for burns: warming of the enviromnent, correction of electrolyte disturbances, administration of a high caloric enteral intake, and prevention of sepsis. Efficacy of drugs used in some case reports is difficult to evaluate: intravenous immunoglobulins, cyclosporin, cyclophosphamide, pentoxyfilline, and thalidomide have all been tried. Corticosteroid use is debated and is probably deleterious in late forms of TEN. For DRESS, corticoids are used in cases of life-threatening systemic impairment. Specific nursing care and adequate topical management reduce associated morbidity and allow a more rapid re-epithelialization of skin lesions.

The overall incidence of lamotrigine-induced serious rash is approximately 0.3 percent in adult patients with epilepsy receiving adjunctive therapy, and this risk seems to be largely confined to the first months of treatment [ ]. Exact rates of Stevens-Johnson syndrome with lamotrigine are not known, but are reported as 0.02 percent in adults from the German Rash Registry [ ]. This case history along with the lack of other potential confounding factors were highly suggestive of lamotrigine-induced Stevens-Johnson syndrome; use of the Naranjo scale indicated a probable relationship of causality [ When lamotrigine was started the patient was also receiving valproate for several years; no adverse effects, including any minor skin rash, were recorded while on valproate monotherapy. With the seemingly unstoppable acute phase often lasting several weeks, SJS and TEN have always been a challenge to treat. Despite round-the-clock application of lubricant ointments, antibiotic and steroid drops, and daily lysis of adhesions using a glass rod, the majority of patients still end up with a lifetime of ocular problems. These sequelae range from mild dry eye, ongoing inflammation, scarred eyelids, entropion with trichiasis, photophobia, and decreased vision, to chronic corneal ulcers, corneal vascularization, total limbal deficiency, complete loss of tears, significant vision loss or blindness, constant severe pain, or complete loss of one or both eyes.

The investigators “are unaware of any previous familial clustering” of Stevens-Johnson syndrome associated with nevirapine treatment.
The non-nucleoside reverse transcriptase inhibitor (NNRTI) nevirapine is a standard component in first-line HAART regimens used in resource-limited settings. It is likely to remain widely used once lower-cost, fixed dose, generic antiretroviral therapy becomes more widely available, and it is anticipated that 100,000s of individuals will initiate antiretroviral therapy with nevirapine.
Rash and hepatological toxicities are well documented side-effects of nevirapine therapy, and to minimise the risk of hepatotoxitity it is recommended that women do not initiate treatment with nevirapine if their CD4 cell count is above 250 cells/mm and men if their CD4 cell count is above 400 cells/mm
Although rash in the early weeks of nevirapine therapy does not necessarily lead to the discontinuation of treatment, Stevens-Johnson syndrome is a rare, but potentially life-threatening necrosis of the skin and can occur in patients taking nevirapine across CD4 cell counts.

Allergic reactions to drugs can manifest in many different ways. One in particular is Stevens-Johnson syndrome. Stevens-Johnson syndrome is a potentially deadly skin disease resulting from a drug-induced allergic reaction. Stevens-Johnson syndrome and toxic epidermal necrolysis are two forms of the same skin disease, and usually result from a drug-related reaction. The two forms of this skin disease can be extremely painful as well as deadly. The FDA recalled the COX-2 inhibitor Bextra , which is already linked to these disorders, on April 7, 2005. In addition to Bextra, additional drugs have been linked to Stevens-Johnson syndrome. These drugs include some other non-steroid anti-inflammatory drugs (NSAIDS), Allopurinol, Phenytoin, Carbamazepine, barbiturates, anticonvulsants, and sulfa antibiotics. While the condition can sometimes, but not often, be attributed to a bacterial infection, additional cases have no known cause for the onset of Stevens-Johnson syndrome or toxic epidermal necrolysis. Experts still say that the most common cause is through drug-related allergic reaction. Stevens-Johnson syndrome occurs most commonly in older people, primarily because the drugs associated with the disease tend to be used in older people. Contracting Stevens-Johnson syndrome and toxic epidermal necrolysis is also a risk in people that have AIDS. Potentially higher risk groups are urged to remain observant for any signs of these skin diseases and become knowledgeable of the symptoms that could indicate the presence or onset of Stevens-Johnson syndrome or toxic epidermal necrolysis.

This was because the non-steroid anti-inflammatory drugs traditionally prescribed for these problems affected both the cox-1 enzyme and the cox-2 enzyme produced by the body. Because the cox-1 enzyme is responsible for protecting the lining of the stomach, many patients suffered chronic gastric problems. However, cox-2 inhibitors like Bextra were able to isolate and inhibit just the cox-2 enzyme, thus reducing the risk of gastric problems dramatically.

Stevens-Johnson syndrome
My husband's been taken into hospital after developing a strange rash that spread to his mouth and caused masses of blisters in his mouth, eyes and genitals. The doctors think he's had a severe reaction to some antibiotics he was taking for a chronic chest problem (the antibiotics were called co-trimoxazole, I think). Is this an allergy and how quickly will it settle? Maisie

It sounds as though your husband has Stevens-Johnson syndrome, which is a severe form of a rash known as erythema multiforme. It's caused by a cell-mediated hypersensitivity reaction to distinct immunological stimuli, which in simple terms is a form of allergy that often takes days or weeks to develop. Although the illness can be severe, especially at first, it usually settles over a period of about a month. Sometimes the rash comes back again at a later date. Stevens-Johnson syndrome can be triggered by various drugs, especially some types of antibiotics, such as those containing penicillin or, as is the case with co-trimoxazole, sulphonamides.
