Endometrial adenocarcinoma and the Stein-Leventhal syndrome
K Jafari, G Javaheri, and G Ruiz Six cases of adenocarcinoma of the endometrium associated with the Stein-Leventhal syndrome are presented. The average age of the patients was 27.8 years. All patients were treated surgically; 2 had preoperative intracavitary irradiation, and 1 had postoperative intravaginal cesium-137 application. At follow-up, ranging from 1 to 15 years, all patients are alive and free of disease. Almost 90% of endometrial adenocarcinoma in association with the Stein-Leventhal syndrome is well differentiated, and appropriate treatment is associated with a good prognosis. Conservative therapy carries the risk of progression of the cancer to more advanced stages and has very little to offer for future fertility. It is suggested that these patients be treated as are any other patients with endometrial cancer.

Polycystic Ovary Syndrome (Stein-Leventhal Syndrome)
Polycystic ovary syndrome (PCOS), which is believed to be an inherited disorder, affects up to 20 percent of reproductive-aged women. It is the number one cause of infertility, and evidence suggests that it can increase the risk of diabetes mellitus, cardiovascular disease and cancer later in life. Occurrence usually takes place at the onset of puberty, but it can appear in women who are in the middle of their reproductive years as well.
Symptoms Some characteristic features of PCOS include Polycystic ovaries (as detected by pelvic ultrasonography Infertility, particularly anovulatory infertility Hyperandrogenism, the ovary being the main source of excess androgens Hirsutism Acne Alopecia Menstrual disturbances Insulin resistance Obesity, especially high waist/hip ratio and body mass index ( BMI
Treatment
PCOS should not be confused with benign idiopathic hirsutism, in which hormone levels are normal and ovaries are not polycystic. Likewise, PCO-like individuals who have nonactive ovarian cysts, which do not influence hormone levels, should not be categorized as PCOS patients. These individuals have normal hormone profiles and are not hirsute.

Polycystic ovary syndrome (PCOS)
Polycystic ovary syndrome (PCOS) is one of the most common endocrinopathies in women of reproductive age. According to the initial description by Stein and Leventhal in 1935, the diagnosis of PCOS was based on the clinical symptoms (oligo/amneorrhea, infertility, hirsutism, and obesity) in the presence of histologically verified polycystic ovaries. Presently, ovarian morphology can easily be identified by ultrasound, which has revealed that PCOS is not only linked to the "classic Stein-Leventhal syndrome", but women with polycystic ovaries exhibit a wide spectrum of clinical presentations. PCOS account for 75% of women with anovulatory infertility, 30% to 49% of secondary amenorrhea, and 85% to 90% of women with oligomenorrhea. The majority of patients with PCOS are hirsute. Obesity is also a frequent finding among women with PCOS. 30 to 60% of PCOS patients are overweighted.

    General Discussion Polycystic ovary syndrome (PCOS) affects women and is a complex of symptoms that are not necessarily all present in all cases. Some, but not all, affected women have multiple cysts on the ovaries (polycystic ovaries). Other characteristics include the absence of menstruation (amenorrhea) or irregular menstruation, failure of the ovary to release eggs (anovulation), elevated levels of the male hormones known as androgens (hyperandrogenism), excessive amounts of body hair (hirsutism), a high rate of miscarriage, and infertility. Three criteria often used for a diagnosis are menstrual irregularity, hyperandrogenism, and exclusion of other disease. There is some evidence that PCOS is an inherited condition.

    Findings and Diagnosis
    Stein-Leventhal (polycystic ovary) Syndrome The images show multiple follicles in each ovary, probably in excess of 8 on each side. The ovaries were top normal in size.
    Discussion
    The Stein-Leventhal (polycystic ovary) Syndrome usually presents as an obese, hirsute female with ammenorhea. The ovaries are usually 2-3 times larger than normal but up to 30% will have normal sized ovaries. Multiple follicles between 0.5 - 1.0 cm in diameter are seen in the ovaries. These follicles fail to ovulate and so accumulate in number.

The excess luteinizing hormone increases the production of male hormones (androgens). If the disorder is not treated, some of the male hormones may be converted to estrogen.     COMPOSITION:
    HRF is a sterile lyophilized powder for reconstitution and parenteral administration. Powder for reconstitution: (per vial) Synthetic luteinizing releasing hormone 0,1 mg or 0,5 mg. Lactose 100,0 mg. Sterile Diluent: (per ampoule) Benzyl alcohol 2.0% as preservative. Water for injection q.s. ad. 2 mL. HRF is a synthetic decapeptide identical in structure to the natural porcine luteinizing hormone-releasing hormone isolated and identified by Schally and his co-workers at Tulane University. PHARMACOLOGICAL CLASSIFICATION: Category A, 21.10 Trophic hormones. PHARMACOLOGICAL ACTION: This pharmacological activity is identical to the physiologic function of the endogenous LH/FSH-RH, which controls the release of antero-pituitary gonadotrophic hormone. (Under normal conditions, circulating oestrogen and progesterone levels influence, to a degree, the pituitary response by means of a feedback mechanism). However, the releasing hormone itself is completely devoid of LH and FSH activity. It will not cause a rise in baseline levels of LH or FSH in the presence of a non-responsive pituitary, nor does it have any other demonstrable pharmacologic activity. Therefore, the secretion of these substances is dependent upon the presence of a normally active pituitary.
