Description of Sprains and Strains
Although the terms sprain and strain are used loosely and have no precise clinical definition, a sprain generally refers to an injury to a ligament - one of the tough, fibrous cords within a joint that connects the bones together, and a strain refers to muscle injury. Sprains and strains often occur together in or near a joint, since joints absorb the stress of movement and are vulnerable to be being twisted or wrenched. The ankle and the knee are the most common sites of such injuries. In mild cases, the force of the injury tears a few fibers of the ligament or muscle, causing mild to moderate pain and swelling, which usually subside within a few days. A more violent injury may completely tear a ligament or muscle and involve bone damage such as a dislocation or fracture. With proper care, most sprains and strains heal completely without complication.

It's possible, though it's uncommon for a very young child to be injured this way because their bones and joint tend to be relatively flexible. She may twist an ankle or knee if she falls while climbing on something or she may stretch or tear ligaments in her wrist or elbow if she tries to break a fall with an outstretched arm, forcing those joints to absorb her full weight. Adapted from message distributed through SAFETY@UVMVM.BITNET, 11 -APR-1995 TORONTO, Canada - You are more likely to suffer a sprain or strain injury during the first four hours of a work shift or the first two days of the work week. That's one of the findings of a research study recently released by Ontario's Workplace Health and Safety Agency (WHSA). The study, "Patterns and risk factors for sprains and strains in Ontario, 1990" was co-authored by University of Toronto associate professor Bernard Choi and WHSA staff Marianne Levitsky, Roxanne Lloyd and Ilene Stones. Dr. Choi said the analysis based on the WHSA's database seems to indicate that returning from a rest period - such as a weekend or a good night's sleep - leaves workers susceptible to sprains and strains. He suggested warm-up exercises- like the stretching that joggers do before runs - might help alleviate the problems. The data also suggest that the first few months of the year are the riskiest for sprains and strains. The study says this may be due to cold weather. Another major finding from the study was that nurses, nursing aides and truck drivers were more susceptible to sprains and strains than other workers. "This is understandable when it comes to nurses," Dr. Choi said. "Nurses have to make many involuntary motions, such as when lifting patients, and involuntary motions increase the risk of sprains and strains." But Choi said the reasoning was less clear when it comes to truck drivers.

    To help sprains and strains heal faster, follow these instructions:
  • Elevate the injured area above your heart whenever possible.
  • Use ice packs for the first 48 hours, but never apply the ice directly to the skin. Place a wash cloth over the injured area and apply the ice 20 minutes on, 20 minutes off.
  • After the first 48 hours, apply heat to control pain.
  • If you have an Ace bandage, you may need to rewrap it if it becomes too tight or too loose. Unwrap the bandage if the area below it becomes cold, blue, swollen or numb.
  • Unwrap the bandage at bedtime.
  • If your clinician has prescribed an anti-inflammatory medicine or pain medicine (example: Motrin, or Tylenol 3), use it as directed.
  • If you have a foot or ankle injury, try not to put weight on it for two days. Returnable crutches or canes may be checked out from the Health Center as needed.
  • For a foot injury, elevate the foot for the first two nights while sleeping to decrease swelling. If the foot is taped, do not remove the tape at night as you would a bandage. Remove the tape only if the area below the tape becomes blue, white, swollen, cold or numb. To take a shower, cover your foot with a small towel and a plastic bag secured with tape.
