Abnormal Curvature of the Spine
Poor posture, a congenital abnormality, injury, infection, and other diseases, such as cerebral palsy, can cause abnormal curvature of the spine. The most common kinds of abnormal curvature are lordosis, flat back, kyphosis and scoliosis. All these disorders can be treated with therapy, exercise, and, in some cases, surgery. Normal Spine Lordosis Flat back Kyphosis Scoliosis Lordosis , the inward curvature of the spine, is normal to a slight degree in the neck and lower back, but it can become exaggerated by poor posture or by kyphosis. Excessive lordosis in the lower back often occurs in someone who is overweight and has weak abdominal muscles. Once excessive lordosis has developed, it can become a permanent condition and may contribute to the development of osteoarthritis of the spine. Flat back , the absence of normal spinal curves, is the result of abnormal development in infancy or injury.

Care for the spine, whether it’s to address degenerative spine disease or a spinal cord injury, can be challenging. Jamie Baisden, MD, discusses what is involved in treating patients with spine disorders.
Q: Degenerative spine disease is a common condition. How do you approach patient treatment of this disorder?
Most degenerative spine conditions should be treated non-operatively. Such management entails treatment of pain with anti-inflammatory medications, muscle relaxants, occasional narcotics, and an aggressive physical therapy and conditioning program. Those patients that fail to respond to non-operative treatment may be candidates for a minimally invasive surgical procedure or a more reconstructive procedure depending on the disorder. After surgery, an aggressive rehabilitation program is usually incorporated in the treatment and, hopefully, prevention of further spine injuries.
Q: Spinal cord injuries obviously have a different treatment approach. Can you describe that?

      Spinal Disorders Almost everyone at one time has experienced significant back or neck pain, such that normal activity is difficult or impossible. Back pain is the second most common reason for physician visits, and back surgery is the third most common surgery performed in the United States. Spine disease includes: degenerative arthritis, spinal stenosis, herniated or degenerative disc disease, instability due to arthritis, trauma, scoliosis, or spina bifida, osteoporosis, fractures, and spinal cord tumors. Neurosurgeons are instrumental in the surgical treatment of spinal disorders, and in fact, over two-thirds of all surgeries performed by neurosurgeons are for spine disease. Treatment of spinal disorders requires a multidisciplinary approach involving your family physician, physical therapists, chiropractors, massage therapists, accupuncturists, pain management specialists, and surgeons. At the University of Virginia, we collaborate with all these health care providers to provide comprehensive care for patients with back and neck pain. With this approach, surgery is often avoided. However, if pain persists despite extensive non-surgical treatment, or if surgery is the most appropriate first option (such as in spine tumors or some fractures), we have nationally recognized spine surgeons well-versed in all current techniques and aided by a top-flight nursing staff to successfully treat all spinal disorders.

The Spine Center at Orthopadic Hospital was established in 1988. It has since evolved into a comprehensive spine center for evaluation and treatment of complicated spine problems.

At any given time, roughly 1 million Americans are on temporary or permanent disability because of back problems. Almost everyone will, at some time, experience back pain that is severe enough to make them miss work or to give up a pastime. The cost of back pain in this country is astronomical, estimated to be $50 billion annually in health care costs and lost productivity. Fundamental spine research is crucial to advancing physicians’ understanding of spine disease. One area of research at The Cleveland Clinic focuses on the spine’s response to stresses and strains. Long-distance truck and bus drivers, and heavy equipment operators, for instance, are exposed to strong vibrations that cause early disc degeneration.Manufacturers are using prior work by our spine researchers to develop special ergonomically designed, vibration-dampening seats. Until artificial disc technology (another effort of our spine researchers) is perfected, spinal fusions will continue to be necessary, and better fusion techniques are being investigated. Intervertebral cages that support the vertebral bodies can now be implanted through new minimally invasive techniques. Researchers are also exploring the use of stem cells and biomaterials to eliminate the need for painful bone grafts in spine surgery. Innovative investigations of the microscopic changes that occur in bone during placement and removal of surgical implants are underway, as well as studies of bone's response to microfracture and osteoporosis, and the effects of commonly used osteoporosis medications on spinal fusion.

    To understand the pain caused by the spine disorders, spondylolysis and spondylolisthesis, it is best to first get an idea of how the normal spinal column is set up. Picture the spine as a stack of wooden blocks. In between each block is a slice of Styrofoam. Now consider that each of the wooden blocks has a short peg sticking out the back, and each wooden peg connects to the one below it and wahlah! You have the spine.
