Spina bifida may cause various degrees of permanent damage to the spinal column and nervous system. Typically, the degree of paralysis, or a child's loss of movement, is related to the location of their defect. In general, the higher the cyst on the back, the more severe the paralysis. About 80 percent of spina bifidas affect the lower back. In addition to damage to the spinal cord, children born with spina bifida may also experience bowel and bladder problems, as well as learning disabilities, although 70 percent of those affected have normal intelligence.

Spina bifida is the most common central nervous system birth defect encountered by the pediatric neurosurgeon. The range of severity in spina bifida is very wide, varying from a simple bony defect of a spinal vertebra with a normal spinal cord and other nervous system components, to an entire complex of myelomeningocele with hydrocephalus (collection of excessive fluid in the cerebral ventricles), Chiari malformation (multiple malformations of the structures at the base of the brain) and abnormalities of the cortex or gray matter of the brain. The myelomeningocele itself is a skin or membrane covered mass, over the midline of the backusually between the level of the midchest to buttockscontaining incompletely and abnormally formed spinal cord tissue, nerves exiting from the spinal cord , and cerebrospinal fluid. Spina bifida occurs in about 3.2 per 10,000 births in the USA. It is much more common in whites than in blacks or Asians. There is a familial tendency to the occurrence of this disorder, although multiple environmental factors and exposure to many toxins/medications/infections have been suggested as contributing to the development of spina bifida. It is now established that folic acid, given before and during the early stages of pregnancy can substantially reduce, by perhaps over 70%, the occurrence of the disorder in high risk and first pregnancies. Prenatal diagnosis is usually possible via the measurement of fetal protein leaking from the open neural tube into the mother's blood stream or amniotic fluid surrounding the fetus. Ultrasound techniques are now so highly developed that some experienced sonographers report nearly 100% detection rates prenatally.

Spina bifida is een aandoening die in teamverband behandeld wordt, waarbij iedere specialist periodes heeft waarin hij meer of minder bij de behandeling betrokken is. Een spina bifida team is aanwezig in de meeste ziekenhuizen die een afdeling neurochirurgie hebben en het bestaat doorgaans uit: een kinderarts, neuroloog, neurochirurg, orthopedisch chirurg, uroloog, revalidatiearts en een maatschappelijk werk(s)ter. Het team onderhoudt daarnaast contacten met andere specialisten.
De open rug
Bij spina bifida (SB) is er, in de meest uitgebreide vorm, sprake van een niet sluiten van de huid, van een of meer wervelbogen en van het ruggenmergsvlies. Hierdoor ligt het ruggenmerg aan de oppervlakte. Het ruggenmerg zelf ligt ter plaatse ook "opengeklapt". Er zijn ook vormen waarbij de het ruggenmergsvlies de buitenste begrenzing vormt. Het ruggenmerg kan overlangs in twee helften gespleten zijn. Bij intacte huid maar open wervelbogen spreekt men van spina bifida occulta ("verborgen"). Als gevolg van de afwijking bestaat er een uitval van de neurologische functies onder het niveau van de open rug. Dit betekent in meerdere of mindere mate een verlamming van de benen en meestal ook uitval van de controle over de sluitspieren. Hoe lager het defect, hoe gunstiger het uitvalt. Enkele centimeters kunnen het verschil tussen lopen en een rolstoel betekenen.
