What causes physical disability?
Spina Bifida
Spina Bifida is a developmental defect which occurs within the first six weeks of pregnancy, caused possibly by a combination of genetic and environmental factors. Some of the spinal bones which normally protect and cover the delicate nerves of the spinal cord do not close. Consequently the spinal nerve tissue protrudes through this gap and forms an open swelling on the back. This could occur anywhere from the back of the head to the lowest part of the spine. Even though surgery at birth closes the gap, the damage to the spinal nerve tissues can never be repaired. The degree of paralysis will depend on the extent of damage to the spinal cord and the position of the lesion. Usually there is some paralysis and loss of sensation in the legs and lower trunk. This could result in:
  • a need to use a mobility aid such as callipers, crutches, a walking frame, walking sticks or a wheelchair various degrees of bowel and bladder incontinence. There are several methods available to manage this problem.

    History and Mission The National Center for Medical Rehabilitation Research (NCMRR) was established within the National Institutes of Health (NIH) by legislation (P.L. 101-613) passed in 1990. The Center is a component of the National Institute of Child Health and Human Development (NICHD). The mission of NCMRR is to foster development of scientific knowledge needed to enhance the health, productivity, independence, and quality of life of persons with disabilities. This is accomplished by supporting research on enhancing the functioning of people with disabilities in daily life. A primary goal of the Center is to bring the health related problems of people with disabilities to the attention of America's best scientists in order to capitalize upon the myriad advances occurring in the biological, behavioral, and engineering sciences.

    Introduction
    Spina Bifida is an umbrella term that covers three conditions of varying severity: Spina Bifida Occulta, Spina Bifida Cystica Menigocele and Spina Bifida Cystica Myelomenigocele. These are described in more detail below. Spina Bifida is a neural tube defect or spinal dysraphism a failure of normal closure of the neural tube during the development of the embryo. The other neural tube defect is anencephaly, but as this is fatal before or very shortly after birth, it has little relevance for occupational therapy.
    Types of Spina Bifida
    • Spina Bifida Occulta , where there is only a minor deficiency of the posterior part of the vertebrae and no damage to the meninges or spinal cord. It presents no symptoms or difficulties in the vast majority of cases, though in some cases the spinal cord may become tethered against the vertebrae, with possible impairment of mobility or bladder control. This is often treated with corrective surgery. Often the only sign of spina bifida occulta is hair or a dimple at the site of the lesion. Many people live their lives unaware that they have the condition. Spina Bifida Cystica has two forms, both of which involve a blister-like sac on the back, where the meninges, or meninges and spinal cord, have ballooned out through the gap in the vertebrae:

    Spina Bifida
    Spina Bifida or "split spine" is a genetic birth defect that occurs when the fetus is only 26-28 days old. The mother may not even know that she is pregnant yet. It is the second most common birth defect in man and occurs in one in every 750 babies born in Canada. The neural tube, or what will eventually develop into brain, spinal cord and vertebral column, fails to close completely, leaving an opening through which the spinal cord and/or the lining of the spinal cord may protrude. The spinal cord is the pathway for messages to be passed back and forth between the brain and different parts of the body. If the spinal cord is affected, there can be full or partial disruption of these messages. Therefore, paralysis and/or loss of sensation can occur to the areas affected, usually, but not always, below the level of lesion of the back. There can be "sparing" of some nerves, causing a confusing and mixed picture, ie., where one leg functions differently than the other. Infants often (80-90%) go on to develop hydrocephalus and may have other problems as they develop and grow.

    and site news. The Mission of the Spina Bifida Association is to promote the prevention of spina bifida and to enhance the lives of all affected. Spina Bifida is the most common of the neural tube defects and is one of the most devastating of all birth defects. The National Institutes of Health state that Spina Bifida will occur in 1 of every 1000 births in the U.S. It occurs more frequently than Muscular Dystrophy and Multiple Sclerosis combined and cripples more children than Polio at its peak. It results from the failure of the spine to close properly during the first month of pregnancy. In severe cases, the spinal cord protrudes through the back and may be covered by skin or a thin membrane. Surgery to close a newborn's back is generally performed within 24 hours after birth to minimize the risk of infection and to preserve existing function in the spinal cord. Children born with Spina Bifida experience varying degrees of paralysis, loss of sensation in the lower limbs, difficulties with bowel and bladder, hydrocephalus, learning disabilities and latex allergies. The challenges brought about by Spina Bifida continue throughout life. To live independent and fulfilling lives, people with this birth defect may need extensive medical, psychological, and educational therapy.
