Spina Bifida
The diagnosis can of ten be made on the basis of a high resolution ultrasound or elevation of maternal alpha fetal protein levels. Other times it is not recognized until birth. It is widely accepted that most children born with spina bifida should be aggressively treated both medically and surgically. Recommendations are for early closure of the back and spinal cord as well as management of hydrocephalus if indicated. The important thing to realize is that this operation is designed specifically to prevent infection or ongoin g trauma to the exposed spinal cord and not to return function which is not present at birth. This is not an insignificant distinction. Hydrocephalus is usually present in @80-90% of children with spina bifida and should b e treated when determined, usually before discharge from the hospital.
As many different parts of the body and nervous system are potentially involved, the child will need to see numerous physicians and therapists for follow up. In addition to physical therapy and early intervention, the child may need treatment for incontinence as well as bracing for the legs or spine. In many areas of the country this follow up is provided through multidisciplinary spina bifida clinics. These clinics are staff ed by the various physicians and therapists whose specialties pertain to the treatment of spina bifida.

Spina bifida is the most common of a group of birth defects called neural tube defects (NTDs). The neural tube is the embryonic structure that develops into the brain and spinal cord. Often called open spine, spina bifida affects the backbone and, sometimes, the spinal cord. It is among the most common severe birth defects in the United States, affecting 1,500 to 2,000 babies (one in every 2,000 live births) each year. Spina bifida and other NTDs occur more frequently among Hispanics and whites of European extraction and, less commonly, among Ashkenazi Jews, most Asian ethnic groups and African-Americans.
How does spina bifida affect a child?
In the embryo, there is a tiny ribbon of tissue that folds inward to form a tube. This structure, called the neural tube, forms by the 28th day after conception. When this process goes awry and the neural tube does not close completely, defects in the spinal cord and in the vertebrae (small bones of the spine) can result. There are three forms of spina bifida:

Spina bifida is the failure of the spine to close properly during the first month of pregnancy and results in varying degrees of paralysis, loss of sensation in the lower limbs, difficulty with bowel and bladder management, hydrocephalus, and learning disabilities. It takes a team of parents, doctors, therapists, teachers and others to help a person with spina bifida achieve maximum potential. But with proper attention, that person's potential is unlimited. The cause of spina bifida is unknown, and the scientific community is unable to explain whether the determinants of spina bifida are genetic or environmental. Recent scientific research, however, shows that folic acid, a common B vitamin, may prevent more than half of all spina bifida births. According to the United States Center for Disease Control, if all women of childbearing age in the United States who are capable of becoming pregnant would consume 0.4 mg. of folic acid per day, the incidence of spina bifida could be reduced by over 70%.

The Spina Bifida Association of Western Pennsylvania (SBAWP) is a charitable, non-profit organization committed to improving the quality of life for people with disabilities, especially those with spina bifida and their families, by providing a continuum of direct service, education and advocacy.
Spina bifida is one of the most devastating of all birth defects affecting nearly one out of every 1,000 newborns in the U.S. It results from the failure of the spine to close properly during the first month of pregnancy exposing the nerves to damage or infection. At birth, the most obvious signs of spina bifida are either a cyst or indentation on the baby's spine. Surgery will close any opening, however, damaged nerves cannot be repaired. Individuals with this disability often suffer from loss of sensation in the lower extremities; bowel and bladder incontinence; leg and foot deformities and learning disabilities.
Since 1969, the SBAWP has evolved from a parent support group to become a nationally recognized provider of direct services to individuals with spina bifida. The organization serves nearly 1000 children, teens and adults living in 28 counties within Western Pennsylvania and portions of New York, Ohio, and West Virginia.

General Facts What is spina bifida? The term spina bifida is derived from the Latin words 'spina' meaning spine and 'bifida' meaning split or divided. The spine is made up of separate bones called vertebrae, which normally cover and protect the spinal cord. In spina bifida, some of these vertebrae are not completely formed but are split or divided and the defective spinal cord and its coverings usually protrude through the opening. There are different categories of spina bifida including: Occulta
The outer part of the vertebrae is not completely joined. The spinal cord and covering (meninges) are undamaged. There are often tufts of hair found at the site of the defect.
Meningocele
The outer part of the vertebrae is split but the spinal cord is normal. Only the meninges is damaged and pushed out through the opening. Myelomeningocele
