Irritable bowel syndrome (functional bowel)
Definition
Irritable bowel syndrome refers to a complex disorder of the lower intestinal tract. It is mainly characterized by a pattern of symptoms that is often worsened by emotional stress. The condition involves hypersensitivity to pain in the gut, combined with altered bowel habits resulting in diarrhea, constipation, or both.

Causes
Irritable bowel syndrome is characterized by a combination of abdominal pain and altered bowel function. There are many possible causes. For instance, there may be a disturbance in the muscle movement of the intestine or a lower tolerance for stretching and movement of the intestine. There is no abnormality in the structure of the intestine. Irritable bowel syndrome (IBS) can occur at any age, but often begins in adolescence or early adulthood. It is more common in women. Predisposing factors may include a low-fiber diet, emotional stress, use of laxatives, a bout of infectious diarrhea, or other temporary bowel inflammation.

5-aminosalicylic acid (5-ASA) A commonly prescribed anti-inflammatory medication considered of great value in the treatment and control of inflammatory bowel diseases, particularly ulcerative colitis. It is a member of the aminosalicylate group of medications, and is also known as mesalazine. Abdomen The lower part of the trunk of the body, between the chest and the pelvis. It contains many parts of the digestive system including the stomach, small and large intestine, and the liver. Acute A short-acting health problem of sudden or rapid onset. Alimentary Pertaining to food. Alimentary canal The passage along which food passes for digestion, also known as the gastrointestinal tract. It includes the mouth, stomach, small intestine and large intestine (colon and rectum – also called the gut). Anus The opening at the lower end of the intestine through which the solid or sometimes fluid (diarrhoea) waste from digested food is excreted as faeces. Anti-inflammatory Tending to alleviate or prevent inflammation.

Background: Irritable bowel syndrome is a frequent gastrointestinal disorder of which the pathophysiological basis is still not completely understood. Among the various proposed mechanisms, colonic motility may play a role, at least in a subset of patients. Aims: To investigate the regular colonic contractile patterns in patients with irritable bowel syndrome. Our working hypothesis was that an increase of these patterns in such condition might represent a motility equivalent of the so-called 'spastic colon'. Patients and methods: Twelve patients of both genders were investigated by means of 24 h colonic manometry, and the results were compared to those from 20 healthy controls. Regular contractile patterns (with frequencies ranging from 2 to 8 per min) were calculated for the entire recording period and in single colonic segments. Results: Overall, colonic frequency patterns were significantly more frequent in patients than controls (12.6% vs 6.4% of the total contractile activity, P

    Irritable Bowel Syndrome (Spastic Colon Roland Larrabee, M.D. Irritable bowel syndrome (IBS) is a chronic (long-lasting) disorder of the large intestine (colon). It is the most common gastrointestinal disorder. It affects twice as many women as men and usually begins in early adult life. Although IBS can cause much distress, it does not lead to life-threatening disease. Another term for IBS is spastic colon. The cause of IBS is not fully understood. Most symptoms of IBS probably result from abnormal muscle movement or spasm of the lower part of the colon. Sometimes the spasm delays bowel movements. At other times it may lead to more rapid passage of bowel movements, resulting in diarrhea. In either case the spasm usually causes abdominal cramps. A specific food may trigger attacks. Spasms may also be triggered by stress or depression because muscle movement in the colon is controlled by the nervous system. The most common symptoms include:

    The "spastic colon" syndrome: therapeutic and pathophysiologic considerations
    F Lechin, B Van Der Dijs, A Bentolila, and F Pena Low doses of d-amphetamine plus propranolol rapidly improved the abdominal pain in 165 "spastic colon" patients. Concomitantly, these drugs reduced the sigmoidal hypertonicity and the rectal inhibition found in the manometric studies performed in some of those patients. The sigmoidal tone and phasic activity were also decreased by anticholinergic drugs. These results suggest that a cholinergic-serotonergic hyperactivity of the myenteric plexus may be responsible for the "spastic colon" syndrome.

    The contents of the large bowel are the primary determinant of the health and function of the intestine, and a spastic colon is no exception to this rule. Few physicians have even an elementary understanding of the influence that foods can have on the body: they are unprepared to deal with health and disease from this fundamental perspective.
    Possibly it is too simple and obvious a fact that the contents of the large intestine will determine whether or not the stool will be large or small, hard or soft, and if defecation will be painful or not, and whether or not blood and mucus will accompany passage of the stool. (This is a clear case of not seeing the forest for the trees.) Could the fear that the very foods the physicians and dietitians themselves eat and feed to their families can also be the cause of their patient's illness be too personal a threat for most health professionals? The "high-tech" drug solution would be much more easy to accept for most men and women of science.
    Most people with irritable bowel syndrome improve almost as soon as they begin to eat a starch-based diet that provides all types of fresh or fresh-frozen plant foods.

    Colon Therapy
    T herapies that relate to the large intestine, or bowel (colon), have always been the stepchildren of medicine. From the inception of medical treatment, complaints related to colon activity have been prevalent. The use of cathartics and laxatives dates from time immemorial and the enema in various forms was described in humanity's very earliest therapeutic writings. Yet to this day, very few doctors are anxious to do any constructive work on this poor, maligned and misunderstood part of the body.
    It is a rare person who doesn't have some difficulty with the colon at some time in his life. If these problems are met and corrected early, they will not assume the severe characteristics of the diseases that can afflict the large bowel.
    The organs of elimination, particularly those for the discharge of urine and feces, have always been regarded as dirty or unclean and weren't to be discussed in polite company. I have often thought that if fecal matter had a delicate odor and a good commercial use, the colon and its attendant difficulties might be viewed in an entirely different light. Our Creator apparently was of a somewhat different opinion. Therefore, since it doesn't seem possible to change the nature of the colon's product, we should attempt to do the next best thing: change our view of this whole process of the elimination of the body's solid waste.
    Fermentation and Putrefaction
    B acterial action on food in the large bowel may be divided into two types-fermentation and putrefaction. Fermentation is the action of bacteria on starches or carbohydrates. By this action, the gases methane, hydrogen, and carbon dioxide are produced. Also organic acids, such as butyric, acetic and lactic acid are formed. If the degree of fermentation is minor, there is little or no distress or knowledge by the individual of this process. On the other hand, if the circumstances are such that there is excessive fermentation, large amounts of gas are formed which may cause distention of the large bowel. This excessive gas formation causes either pain in the more severe cases or at least the passing of the objectionable gas. Where digestion is weak, or where the individual has a digestive idiosyncrasy, certain foods tend to cause more fermentation than others. The common offenders are the various types of beans, onions, peanuts, cabbage, cucumbers, and dried fruits.

More information Irritable bowel syndrome is a catchall name for a variety of ailments also referred to as spastic colon, nervous colon, spastic bowel, mucous colitis and irritable colon. It is the most common intestinal disorder but is not life-threatening. The syndrome affects millions of Americans, mostly women. While there is no cure, adding dietary fiber and decreasing fat intake can alleviate some symptoms, as can certain medications. "It's one of the most embarrassing diseases out there," says the 29-year-old Dallas resident. "I can't even go to a party because I may need to go to the bathroom and I might have only two seconds to get there or I'll have an accident. It's too humiliating." Her condition is called irritable bowel, also sometimes known as colitis, spastic colon or functional bowel disease. It falls under a syndrome of ailments believed to affect 22 million Americans, most of them women.
