The primary care physician has always been heavily involved in the care of patients who have somatic symptoms without objective medical findings. As their gatekeeper role expands, family physicians must focus on safely managing the somatizing patient. In the past, this patient could hop from one private physician to another whenever the patient decided that the physician must either be a "quack" or was just not smart enough to see how sick he or she was. The patient's departure was often accompanied by relief on the part of the physician. Under the managed care system, however, the patient will be assigned a primary care physician, and the somatizing patient and the physician will have to learn to work with each other. The patient will not be able to "doctor shop," and the physician will not be able to order endless tests and procedures.

What is somatoform disorder?
People with somatoform disorder have a number of different symptoms that typically last for several years. Their symptoms can't be traced to a specific physical cause. In people with somatoform disorder, medical test results are either normal or don't explain the person's symptoms. People who have somatoform disorder often become very worried about their health because they don't know what is causing their health problems. The symptoms of somatoform disorder are similar to the symptoms of other illnesses. People with this disorder may have several medical evaluations and tests to be sure that they don't have another illness. Return to top
What are the symptoms of somatoform disorder?
Symptoms of somatoform disorder may include frequent headaches, back pain, abdominal cramping and pelvic pain. Other symptoms include pain in the joints, legs and arms, and chest or abdominal pain. Somatoform disorder may also cause gastrointestinal problems, such as nausea, bloating, vomiting, diarrhea and food intolerance. Sometimes somatoform disorder makes it painful for a person to urinate, even if he or she doesn't have an infection. It can also cause problems with sexual function for both men and women.

Chronic urethritis is an inflammation (irritation with swelling and presence of extra immune cells) of the urethra (the tube that carries urine from the body) that continues for weeks to months.

Causes
Chronic urethritis is usually caused either by a bacterial infection or structural problem (narrowing of the urethra). It may also be associated with a variety of systemic diseases, including emotional disorders. Urethritis may occur in either women or men. The major symptoms of urethritis, painful urination and urinary frequency , are quite common. Among people experiencing such symptoms, 50% to 75% have either a first-time or recurrent bacterial infection of the urethra or bladder ( cystitis ), up to 10% are women with vaginitis , and the remainder are either men or women with urethral syndrome (symptoms present without any evidence of bacterial infection). Organisms that cause a variety of sexually transmitted diseases are included among the group of possible causative agents. Importantly, sexually transmitted diseases such as chlamydia and gonorhea may cause urethritis. Other common bacterial organisms that may be responsible for this condition include E. coli

For most of my childhood and adolescence I believed that I had an enormous, elephantine skull. I didn't. Actually, I am told that my head is unusually small in comparison to my body. This is especially true after I put on another 20 kilos in weight. So, why was I wrong about the size of my head for such a long and critical period of my life? I am a cerebral narcissist. I derive my Narcissistic Supply from people's reactions to my intellectual achievements - real or fictitious. No wonder that I exaggerated the dimensions of the site and exclusive source of my life sustaining gratification. Children draw adults as giants. Budding cerebral narcissists misconstrue the size of their skulls. Possessing a distorted physical self-image is called a Body Dysmorphic Disorder. All narcissists have it to some degree. Somatic narcissists are especially prone to misjudge their bodies - either positively or negatively. They believe themselves to be physically irresistible, exuding sex and energy, statuesquely shaped, and, in general, stunning hunks. This grandiose self-image rarely corresponds with reality, though.
