Learn more about our clinical programs and staff here Description Sometimes a child or adolescent continually complains of a discomfort or a pain for which a physician cannot find a cause. The pain or the discomfort, however, is very real to the youngster. The child or adolescent may be suffering from a psychosomatic illness - (Somatoform Disorder) - and may be expressing emotional conflicts through physical complaints. Somatoform Disorders is the relatively new term used to describe a group of involuntary disorders characterized by recurrent and clinically significant physical symptoms that cannot be fully explained by a neurological or generalized medical ("organic") condition. The actual diagnosis in children is rare because the criteria for Somatoform Disorders were established for adults and certain symptoms are not yet experienced by children. Somatoform Disorder must be distinguished from two other diagnoses: Malingering and Factitious Disorders, both characterized by consciously or purposely produced symptoms. Somatoform Disorders diagnosed in children include Conversion Disorder, Somatization Disorder, Body Dysmorphic Disorder, Hypochondriasis, and Pain Disorder.

The common feature of the Somatoform Disorders is the presence of physical symptoms that suggest a general medical condition but are not fully explained by a general medical condition, by the direct effects of a substance, or by another medical/psychiatric disorder e.g. Panic Disorder. The symptoms must cause clinically significant distress or impairment in social, occupational, or other areas of function. The Somatoform Disorders include: Somatization Disorder, Conversion Disorder, Pain Disorder, Hypochondriasis, Body Dysmorphic Disorder, Undifferentiated Somatoform Disorder, and Somatoform Disorder NOS. Factitious Disorders and Malingering are both characterized by the intentional production or feigning of physical or psychological symptoms. In Malingering, however, symptoms are produced because of a clear external incentive, e.g., to avoid an undesirable deployment or to obtain monetary compensation, while in Factitious Disorder, symptoms result only from a wish to assume the sick role. IX. POST TRAUMATIC STRESS AND SOMATOFORM DISORDERS
A. Psychiatric Morbidity
Psychiatric morbidity due to service during the Gulf War was predicted to be low for several reasons: the short duration of the conflict, the relatively low casualties sustained by American forces, and the positive support for the war at home. Examination of records of evacuation during the conflict is one approach to examining the extent of psychiatric morbidity: the Army rate of evacuation for psychiatric reasons translated to only 2.7 per 1,000 evacuations per year.[ ] This very low rate of psychiatric evacuations is in contrast to prior wars in which evacuations for psychiatric disorders in comparison to total evacuations were: 23% in World War II, 10% for Korea, and 7% from Vietnam[ ]. Of the roughly 250 Army personnel evacuated from the Gulf for psychiatric reasons, approximately fifty (20%) were later determined to be disabled for further military service; levels of Post Traumatic Stress Disorder (PTSD2) were found to be very low, with only four of these 50 carrying a diagnosis of PTSD [ ]. Another approach to assessing psychiatric morbidity possibly relating to service in the Gulf is to examine the numbers of service members referred for disability determination due to psychiatric disorders. As of March 1994, approximately 294 soldiers with psychiatric-related diagnoses were referred for disability determinations. Of these, 112 carried the diagnosis of PTSD.[

Somatoform and related disorders Lecturer Larry Goldman, M.D. A significantly large proportion of patients present themselves to primary care physicians with medically unexplained symptoms. Although some of these patients may have unrecognized underlying physical disease, many, if not most, have "somatized" their psychosocial distress. Research has demonstrated that at least 10% of physically symptomatic patients have no evidence of physical disease and that about 25% of patients in primary care practices have some degree of somatization. Further, it has been repetitively demonstrated that a small proportion of patients utilize a disproportionately large percentage of medical care services. Although some of these high utilizers have serious or significantly severe chronic disease many high utilizers have abnormal illness behavior that is remarkably stable year to year and consume a significant proportion of medical care costs. Primary care remains the de facto major mental health care delivery system. Patients in primary care settings, however, infrequently identify themselves as in need of mental health care services and instead use a physical symptom as a "ticket of admission" to the doctor's office. This form of somatization has been labeled by Katon and colleagues as the culturally sanctioned idiom of psychosocial distress. Failure by the physician to recognize somatization results in inappropriate and ineffective treatment of the underlying psychosocial problems as well as the ordering of expensive diagnostic procedures or even employment of unnecessary surgical interventions.
