Symptoms
The most common characteristic of the somatoform disorder is the appearance of physical symptoms or complaints of such without any organic basis. Such dysfunctional symptoms tend to range from a specialized sensory or motor diability to hypersensitivity to pain. Four major somatoform disorders exist: conversion disorder (also known as hysteria), hypochondriasis, somatization disorder, and somatoform pain disorder.
Types
Conversion Disorder Primary symptom is often a lack or change in physical functioning. The diseased often react with an attitude of indifference, showing an amazing lack of concern. However, the primary symptoms which may include such serious ailments as blindness, amnesia and paralysis, are used as a defense mechanism by the person to escape from a stressful situation. In addition, there may be an awareness of the gains possible through the use of the symptom, which may prolong the symptom. Symptoms are grouped as follows: Sensory Symptoms : These include anesthesia, excessive sensitivity to strong stimulation (hyperanesthesia), loss of sense of pain (analgesia), and unusual symptoms such as tingling or crawling sensations.

Somatization and Somatoform Disorders
Somatization is where mental factors such as stress cause physical symptoms. Somatoform disorders are a severe form of somatization where physical symptoms can cause great distress, often long-term. However, people with somatoform disorders are usually convinced that their symptoms have a physical cause.
Somatization What is somatization?
When physical symptoms are caused by mental or emotional stress it is called 'somatization'. For example, many people have occasional headaches caused by mental stress. But, stress and other mental health problems can cause many other physical symptoms such as: chest pains, tiredness, dizziness, back pain, feeling sick, diarrhoea, period problems, etc. The term 'psychosomatic' means something similar to somatization, but includes other things. See separate leaflet called 'Psychosomatic Disorders'. How can the mind cause physical symptoms
The relationship between the mind and body is complex and not fully understood. When we 'somatize', somehow the mental or emotional problem is expressed partly, or mainly, as one or more physical symptoms. However, the symptoms are real and are not imagined. You feel the pain, have the diarrhoea, etc. How common is somatization?

    printer friendly version Somatoform disorders are characterised by the primary problem being a physical presentation or concern secondary to a psychological problem. They are an important group of disorders to be aware of since patients with them often present to GP’s and physicians rather than psychiatrists.
    Somatisation disorder.
    • Dramatic presentation of multiple, recurrent and frequently changing physical symptoms. Many negative investigations have usually been carried out in the years prior to referral to a psychiatrist. The disorder is more common in women than men and usually starts in early adult life.
    Hypochondriasis.
    • A persistent belief in the presence of at least one serious physical illness despite negative physical findings and reassurance. Alternatively the patient may have a persistent preoccupation with a presumed deformity or disfigurement.

Children and adolescents with physical illness have a progressive course, with deteriorating school performance, visual loss, fatigue, posture abnormalities, and a variety of symptoms that suggest a medical disorder. Psychogenic disorders, on the other hand, usually have a rapid onset, with a family history of similar presentations, a clear discrepancy between the physical findings and the patient's complaint, and some conscious or unconscious motivation for the maintenance of these symptoms (see Table 1 ). Patients have an inappropriate level of disability and distress and increased utilization of health care services.
