  • The management of somatoform disorders is a confusing and difficult area.
  • There are many iatrogenic complications, including unnecessary and repetitive investigations and surgery, drug dependence and "doctor shopping".
  • Somatisation is a world wide phenomenon and a common presentation of psychological distress.
  • Three main groups of patients are encountered in general practice: those with high levels of somatic symptoms, those with illness fear, and those with somatic presentations of other psychiatric illnesses.
  • Management is straightforward for the acute cases and consists of trying to get the patient to accept a link between the psychosocial conflicts and the symptoms.
  • Management is very difficult for chronic cases, where care, not cure, is the goal, as is an attempt to limit harm to the patient and limit the cost to the health system.

1998 MJA Definitions Somatisation: The tendency to experience, conceptualise and communicate mental states and distress as physical symptoms or altered bodily function. Somatoform disorder: The presence of physical symptoms that suggest but which are not fully explained by a general medical condition, the direct effects of drugs or another mental disorder. The symptoms must cause clinically significant distress, or impairment in social, occupational or other areas of functioning. In contrast to factitious disorders and malingering, the physical symptoms are not intentional. Somatisation disorder: A rare and extreme version of somatoform disorder where the patient over many years seeks medical attention for many physical symptoms with no evidence of organ pathology. The diagnosis of the disorder requires the presence of 14 of 37 potential symptoms for women and 12 for men (Box 3). Somatisation syndrome A partial version of somatoform disorder, in which the patient has fewer symptoms (four for men, six for women).

Definition
The somatoform disorders are a group of mental disturbances placed in a common category in the fourth (1980) edition of the Diagnostic and Statistical Manual of Mental Disorders (DSM-IV) on the basis of their external symptoms. These disorders are characterized by physical complaints that appear to be medical in origin but that cannot be explained in terms of a physical disease, the results of substance abuse, or by another mental disorder. In order to meet DSM-IV 's criteria for a somatoform disorder, the physical symptoms must be serious enough to interfere with the patient's employment or relationships, and must be symptoms that are not under the patient's voluntary control. It is helpful to understand that the present classification of these disorders reflects recent historical changes in the practice of medicine and psychiatry. When psychiatry first became a separate branch of medicine at the end of the nineteenth century, the term hysteria was commonly used to describe mental disorders characterized by altered states of consciousness (for example, sleepwalking or trance states) or physical symptoms (for example, a "paralyzed" arm or leg with no neurologic cause) that could not be fully explained by a medical disease. Somatoform Disorders Somatoform Disorders are those conditions that are suggestive of a physical disorder but upon examination cannot be accounted for by a medical disorder, a substance or other mental disorder. These disorders are usually presented in general medical settings as the patients presume them to be of somatic origin. To warrant the diagnosis of Somatoform Disorder the symptoms must cause significant impairment or distress.
  • Somatization Disorder was formerly called hysteria. Primarily affecting women, it generally has its onset in adolescence or early adulthood and lasts for years. Its symptoms usually include a combination of pain, pseudoneurological, digestive and sexual complaints. Undifferentiated Somatoform Disorder is a milder form of Somatization Disorder lasting at least six months. Conversion Disorder is characterized by unexplained sensory motor deficits which are judged to be psychogenic. Pain Disorder is diagnosed when the primary symptom is pain and psychological factors are presumed to be important in the etiology, maintenance or exacerbation of the condition.

Somatoform disorders
DESCRIPTION: A group of disorders in which there are physical symptoms for which no physical cause can be found. Included in this group are somatization disorder, conversion disorder, and body dysmorphic disorder. Hypochondriasis and chronic pain disorder (somatoform pain disorder) are also classified as somatoform disorders, but are covered in separate topics.
  • Somatization disorder: multiple physical symptoms that recur over a period of several years with no medical basis. The patient has usually consulted numerous doctors for treatment and medications. This disorder was previous called hysteria. The symptoms often involve neurological problems (double vision), gynecological problems (painful menstruation) or gastrointestinal problems (abdominal pain).
  • Conversion disorder: loss or change of a physical function (paralysis of an arm, blindness, seizures). The symptom is not intentionally produced or faked.
  • Body dysmorphic disorder: preoccupation with an imagined defect in a normal-appearing person. The defect may be very minor, but the concern is blown out of proportion. The focus is often on skin wrinkles and blemishes, facial hair, shape of the nose, mouth or jaw.

Somatoform Home Somatoform disorders represent a group of disorders characterized by physical symptoms suggesting a medical disorder, however no general medical condition, other mental disorder, or substance is present. Psychological conflicts may be translated, or converted, into physical problems, pain, or complaints. Individuals suffering from a somatoform disorder are not consciously aware of the psychological factors which are in operation, and they are not "making up" their physical symptoms. The symptoms and pain are real - the cause is simply psychiatric instead of medical. Types of Somatoform Disorders: Body Dysmorphic Disorder:
Individuals suffering from body dysmorphic disorder become deeply concerned about some imagined or minor defect in their appearance. They may go to great lengths to conceal their "defects" or avoid contact with others. Conversion Disorder:
In conversion disorder, a psychosocial conflict or need is converted into dramatic physical symptoms that affect voluntary motor or sensory functioning. The symptoms often seem neurological, such as paralysis, blindness, or loss of feeling (anesthesia).
