Overview Symptoms Treatment Prevention Definition: Multiple physical complaints that suggest physical disorders without a disease or physical basis to account for them.
Alternative Names: Briquet's syndrome
Causes, incidence, and risk factors: In somatization disorders, there tends to be a pattern of multiple and recurrent symptoms and complaints that can involve any body system and often persist for years. Most frequently, the complaints involve the gastrointestinal system, the nervous system, the cardiopulmonary (heart and lungs) system, or the reproductive system. The symptoms begin before the age of 30 and occur more often in females. The symptoms are severe enough to lead to the person to visit the doctor or take medication, and may interfere with the person's life. The cause is not specific but symptoms begin or worsen after losing a job, close relative, or friend, or other loss. A lifelong history of sickliness is often present, and often people with this disorder relate to others only through their symptoms. A greater intensity of symptoms often occurs with stress

According to the diagnostic criteria for somatization disorder given in the revised third edition of the Diagnostic and Statistical Manual of Mental Disorders (DSM-III-R), 13 of 37 specific medically unexplained somatic symptoms must be present to establish the diagnosis. Previous studies have found that the full somatization disorder is relatively rare in patients in primary care settings. However, many patients have multiple medically unexplained somatic symptoms but do not meet all of the DSM-III-R criteria. Whether somatization disorder should be based on a dichotomous concept (with diagnosis depending on the presence of 13 symptoms) or whether it is better viewed as a spectrum (with the full disorder at one extreme) is controversial. Katon and colleagues hypothesized that somatization disorder is a continuum, and that, as the number of medically unexplained somatic symptoms increase, so would the rates of coexisting psychiatric disorders, disability and medical care use.

Although often difficult to recognize, somatization disorder- the presence of multiple physical symptoms but no apparent physical or psychiatric disease- is a common problem in primary care practices. Onset is usually before age 30. The disorder manifests as a myriad of symptoms, including gastrointestinal complaints, various types of pain, cardiopulmonary symptoms, pseudoneurologic complaints, sexual problems and complaints related to the female reproductive system. Vigilance is necessary, because somatization disorder may mimic numerous physical and psychiatric ailments.

Somatization disorder
Definition: Somatization disorder is a chronic condition in which there are numerous physical complaints lasting for years and resulting in substantial impairment that are caused by psychological problems and for which no underlying physical problem can be identified.
Alternative Names: Briquet's syndrome
Causes, incidence, and risk factors: The disorder is marked by multiple physical complaints that persist for years involving any body system. Most frequently, the complaints involve chronic pain and problems with the digestive system, the nervous system, and the reproductive system. The disorder usually begins before the age of 30 and occurs more often in women. Somatization disorder is highly stigmatized and patients are often dismissed by their physicians as having problems that are "all in your head." However, as researchers study the connections between the brain, the digestive system, and the immune system, somatization disorders are becoming better understood and should not be seen as "faked" conditions which the patient could end if he or she chose to do so. The symptoms are generally severe enough to interfere with work and relationships and lead the person to visit the doctor and even take medication. A lifelong history of "sickliness" is often present. However, despite thorough investigation, no specific underlying physical cause is ever identified to account for the symptoms. Stress often worsens symptoms.

Mental Health Disorders Somatoform Disorders Somatization Disorder A very specific set of symptoms defines this somatoform disorder. The individual must have a history of physical complaints beginning before he or she is 30 years old that has caused significant distress or impairment. Among the complaints there must have been at least four pain symptoms, two symptoms involving the digestive tract, one involving sexual functioning, and one symptom involving the nervous system. The symptoms could have occurred at any time prior to the examination. The pain can have occurred at any place, during any activity; similarly, there are no further restrictions on the place or time of the other three classes of symptoms. The second requirement is that the symptoms cannot be adequately explained by any observable medical condition or by the effects of a drug. Finally, it must appear that the symptoms are not produced deliberately by the examinee. Somatization Disorder (SD) is relatively rare, and probably more common in women than in men. No more than 1 man in 500, and perhaps 1 woman in 200, has SD. People with the disorder may consult multiple physicians and receive many tests, and even operations, that turn out to be unnecessary. The disorder is likely to be lifelong; patients who seek or are referred to mental health professionals are difficult to convince that their symptoms have little physical basis, given that the patient did not voluntarily produce the symptoms. Denying the validity of the symptoms simply makes the patient seek another professional who acknowledges the genuineness of the symptoms. Patients' colorful, though often inconsistent, descriptions of their symptoms may initially be convincing to the physicians that they so often consult. Maxmen and Ward

Much of this information is from the DSM IV (Diagnostic and Statiscal Manual of Mental Disorders, 4th edition, published by the American Psychiatric Association), and provided to me by a psychiatric advanced practice nurse. Somatization disorder is characterized by physical complaints occurring over period of several years and resulting in treatment seeking and impairment in functioning. For a diagnosis of somatization disorder, the patient must have 4 pain symptoms, 2 gastrointestinal symptoms, 1 sexual symptom, and 1 pseudo-neurological symptom, AND after a full medical work-up, either the symptoms cannot be fully explained by medical condition or symptoms are in excess of what is expected given the medical condition. Conversion disorder is 1 or more symptoms affecting voluntary motor or sensory function (suggesting a neurological or medical condition), but psychological factors are judged to be associated with symptoms because the symptoms is preceded by a conflict or other stressor. The symptoms are not intentionally produced, cannot be fully explained by medical condition, and cause significant distress or impairment.
