The phenomenon of somatization, which results in unexplained physical complaints, is ubiquitous in primary care settings although it often goes unrecognized. Medical training emphasizes the identification and treatment of organic problems and may leave physicians unprepared to recognize and address somatoform complaints. As a process, somatization ranges from mild stress-related symptoms to severe debilitation. Patients at the low end of the spectrum often respond to simple reassurance, but patients who are more impaired require interventions specifically designed to avoid unnecessary exposure to dangerous, costly and frustrating diagnostic procedures and treatments. (Am Fam Physician 2000;61:1073-8.) I n patients with somatoform disorders, emotional distress or difficult life situations are experienced as physical symptoms. Patients who somatize present with persistent physical complaints for which a physiologic explanation cannot be found. Failure to recognize this condition and manage it appropriately may lead to frustrating, costly and potentially dangerous interventions that generally fail to identify occult disease and do not reduce suffering.

Previous Next POEM Care Recommendation Letter for Somatization Disorder Clinical Question: Does a care recommendation letter improve outcomes for patients with somatization disorder? Setting: Outpatient (primary care) Study Design: Randomized controlled trial (nonblinded) Synopsis: Somatizing patients have many unexplained symptoms; evaluation of these symptoms consumes significant health care resources and is frustrating for the patient and the physician. In this study, 2,902 consecutive primary care patients were screened for the presence of at least three unexplained physical symptoms with an 11-item questionnaire derived from the Diagnostic and Statistical Manual of Mental Disorders , 3d ed., rev. (DSM-III-R) criteria for somatization disorder. A more detailed face-to-face interview was used to classify the patients (based on DSM-III-R criteria) into somatoform disorder (13 or more lifetime unexplained physical symptoms; n = 88), abridged somatization disorder (at least four unexplained lifetime physical symptoms in men or six in women; n = 183), or multisomatoform disorder (three or more current unexplained symptoms from a list of 15 in a patient with a two-year history of somatization; n = 111). The diagnostic classifications of somatoform disorder, abridged somatization disorder, and multisomatoform disorder were not mutually exclusive; 188 patients (6.4 percent) of the 2,902 patients were classified in at least one of these groups. Thus, on a typical day of outpatient care seeing 24 patients, a physician might see one or two of these patients, so this classification has face validity.

SYMPTOMS
A history of many physical complaints beginning before age 30 years that occur over a period of several years and result in treatment being sought or significant impairment in social, occupational, or other important areas of functioning. Each of the following criteria must have been met, with individual symptoms occurring at any time during the course of the disturbance:
  • four pain symptoms: a history of pain related to at least four different sites or functions (e.g., head, abdomen, back, joints, extremities, chest, rectum, during menstruation, during sexual intercourse, or during urination)
  • two gastrointestinal symptoms: a history of at least two gastrointestinal symptoms other than pain (e.g., nausea, bloating, vomiting other than during pregnancy, diarrhea, or intolerance of several different foods)
  • one sexual symptom: a history of at least one sexual or reproductive symptom other than pain (e.g., sexual indifference, erectile or ejaculatory dysfunction, irregular menses, excessive menstrual bleeding, vomiting throughout pregnancy)
  • one psuedoneurological symptom: a history of at least one symptom or deficit suggesting a neurological condition not limited to pain (conversion symptoms such as impaired coordination or balance, paralysis or localized weakness, difficulty swallowing or lump in throat, aphonia, urinary retention, hallucinations, loss of touch or pain sensation, double vision, blindness, deafness, seizures; dissociative symptoms such as amnesia; or loss of consciousness other than fainting)

Alternate Names : Briquet's Syndrome Somatization Disorder Treatment The goal of treatment is to help the person learn to control the symptoms. There is often an underlying mood disorder which can respond to conventional treatment, such as antidepressant medications. A supportive relationship with a sympathetic health care provider is the most important aspect of treatment. Regularly scheduled appointments should be maintained to review symptoms and the person's coping mechanisms. Test results should be explained. It is not helpful to tell people with this disorder that their symptoms are imaginary.

However, as researchers study the connections between the brain, the digestive system, and the immune system, somatization disorders are becoming better understood and should not be seen as "faked" conditions which the patient could end if he or she chose to do so. The symptoms are generally severe enough to interfere with work and relationships and lead the person to visit the doctor and even take medication. A lifelong history of "sickliness" is often present. However, despite thorough investigation, no specific underlying physical cause is ever identified to account for the symptoms. Stress often worsens symptoms.

DSM IV Criteria Many physical complaints resulting in treatment being sought or significant functional impairment. Onset is before the age of 30. Physical Complaints History of pain related to at least 4 sites or functions. Two GI symptoms. One sexual symptom. One symptom suggestive of a neurological condition (pseudoneurological). Symptoms can not be explained by organic etiology or symptoms are in excess of what is expected from the medical evaluation. Symptoms are not intentionally produced. Clinical Features of Somatization Disorder Somatization disorder is chronic problem, and it often causes frequent seeking of medical treatment or multiple concurrent treatments. Patients undergo multiple procedures, surgeries, and hospitalizations. The disorder often begins during adolescence. Frequently encountered symptoms include nausea, vomiting, extremity pain, shortness of breath, and pregnancy or menstruation associated complaints. The frequency and severity of symptoms may vary with level of stress.

Somatoform Home Somatoform disorders represent a group of disorders characterized by physical symptoms suggesting a medical disorder, however no general medical condition, other mental disorder, or substance is present. Psychological conflicts may be translated, or converted, into physical problems, pain, or complaints. Individuals suffering from a somatoform disorder are not consciously aware of the psychological factors which are in operation, and they are not "making up" their physical symptoms. In conversion disorder, a psychosocial conflict or need is converted into dramatic physical symptoms that affect voluntary motor or sensory functioning. The symptoms often seem neurological, such as paralysis, blindness, or loss of feeling (anesthesia).

What is somatization disorder?
Somatization disorder is a psychological disorder (listing 300.81 in the DSM-IV, the Diagnostic and Statistical Manual of Mental Disorders). It is a variety of somatoform disorder, a condition in which a patient reports physical symptoms which cannot be explained by a known general medical disorder . These symptoms must be significant enough to cause distress and impair the person's functioning.
The diagnostic criteria
The diagnostic criteria for somatization disorder require a history of multiple physical complaints beginning before age 30 which resulted in seeking treatment or impaired functioning.
