Herniated nucleus pulposus (slipped disk)
Definition: A condition in which part or all of the soft, gelatinous central portion of an intervertebral disk (the nucleus pulposus) is forced through a weakened part of the disk, resulting in back pain and leg pain caused by nerve root irritation.
Alternative Names: Lumbar radiculopathy; Cervical radiculopathy; Herniated intervertebral disk; Prolapsed intervertebral disk; Slipped disk; Ruptured disk
Causes, incidence, and risk factors: The bones of the spinal column, or vertebrae, run down the back connecting the skull to the pelvis. These bones protect nerves as they exit the brain and travel down the back and then to the entire body. The spinal column is divided into several segments the cervical spine (the neck), the thoracic spine (the part of the back behind the chest), the lumbar spine (lower back), and sacral spine (the part connected to the pelvis that does not move). The spinal vertebrae are separated by cartilage disks filled with a gelatinous substance, that provide cushioning to the spinal column. These disks may herniate (move out of place) or rupture from trauma or strain , especially if degenerative changes have occurred in the disk.

GENERAL INFORMATION
DEFINITIONSudden or gradual break in the supportive ligaments surrounding a spinal disk (a cushion separating bony spinal vertebrae).
BODY PARTS INVOLVED
Disks of the neck or lower spine are the most common sites, especially between the 4th and 5th lumbar vertebrae.
IN THE LOWER BACK:
  • Severe pain in the low back or in the back of one leg, buttock or foot (sciatica). Pain usually affects one side and worsens with movement, coughing, sneezing, lifting or straining.
  • Weakness, numbness or muscular wasting of the affected leg. IN THE NECK:
  • Pain in the neck, shoulder or down one arm. Pain worsens with movement.
  • Weakness, numbness or muscular wasting of the affected arm.
    CAUSES
    Weakening and rupture of the disk material, creating pressure on nearby spinal nerves. Rupture of the disk is caused by sudden injury or chronic stress, such as from constant lifting or obesity.
    RISK INCREASES WITH
  • Any sport in which movement causes downward or twisting pressure on the neck or spine. The most common include bowling, tennis, jogging, track, football, racquetball, weight-lifting or gymnastics.

    Low back strikes nearly everyone at some point in life. Although some people must deal with back pain on a daily basis, a large number of people may benefit from treatment. More importantly, back injuries are preventable.
    Causes of Low Back Pain Low back pain may result from a number of causes, such as poor posture and body mechanics, or decreased flexibility and strength. One of the most common causes is fatigued or "pulled" muscles. This can result from overuse, such as excessive yard work. Another common cause is the "Weekend Warrior Syndrome," when an individual that rarely takes part in strenuous activity, performs activities for which they are not properly conditioned. The muscle sustains injuries and may swell and be painful during use. Another cause of low back pain is a bulging or herniated nucleus pulposis, commonly called a "slipped disk." The vertebral disk can be thought of as a stale jelly donut - a jelly-like center surrounded by a tough outer covering. It normally sits between two vertebrae (the back bones), and provides space between the bones to increase their mobility. A "slippage" results when the forces on one side of the disk result in the gel center being forced partially or totally through the outer covering. One study suggests pain may result from the decrease in the overall stability of the vertebral column. This results in decreasing the stability of the bones, which act as the anchors for the muscles attached to them. This would be like removing some of the bricks from the base of a chimney and watching it slowly lean and crumble over months and years. The only accurate way to determine if someone has a slipped disk is by CT scan or MRI.

    What can I do for relief when I've hurt my lower back?
    The best position for relief when your back hurts is to lie on your back on the floor with pillows under your knees, with your hips and knees bent and your feet on a chair, or just with your hips and knees bent. This takes the pressure and weight off your back. If you're resting a hurt back, you may need 1 to 2 days of this sort of rest. Resting longer than this can cause your muscles to weaken, which can slow your recovery. Even if it hurts, walk around for a few minutes every hour.

Thanks to a heady cocktail of hardcore painkillers, muscle relaxants and anti-inflammatory drugs, not only could I not sit down long enough to tap out a bestseller, but I couldn't think, read or do much apart from dribble. I could have donned a hoody and set up a lucrative kerbside pharmacy though.   • The leading cause of back pain is simple muscle strain. Although symptoms may come on suddenly and can be acutely painful, this is actually a problem that develops over a long period of time. When muscles contract, lactic acid and pyruvic acid are produced as byproducts of muscular activity. It is the presence of lactic acid in the muscles that produces the familiar sensation of muscle fatigue following strenuous activity. If high levels of these acidic byproducts accumulate in the muscles, they cause irritation that can eventually turn into pain and interfere with the normal conduction of electrical impulses in the muscle tissue.
