The Shy-Drager syndrome (SDS) is a form of progressive autonomic nervous system failure (PAF) with orthostatic hypotension and associated extrapyramidal involvement that is often mistaken for Parkinson disease. SDS includes olivopontocerebellar atrophy and striatonigral degeneration which is attended by PAF. Eight patients with SDS were studied on a 0.5-T superconducting system utilizing T1- weighted inversion recovery (IR) and T2-weighted spin-echo pulse sequences and also on a 1.5-T system using

Shy-Drager Syndrome
Definition: A progressive neurodegenerative condition of the central and autonomic nervous systems characterized by atrophy of the preganglionic lateral horn neurons of the thoracic spinal cord, which differentiates this condition from other forms of idiopathic orthostatic hypotension (HYPOTENSION, ORTHOSTATIC). This disease is generally considered a clinical variant of MULTIPLE SYSTEM ATROPHY. Affected individuals present in the fifth or sixth decade with orthostasis and bladder dysfunction; and later develop FECAL INCONTINENCE; anhidrosis; ATAXIA; IMPOTENCE; and alterations of tone suggestive of basal ganglia dysfunction. Dear Bavette: Sorry to hear about your patient. Constipation is a problem in Shy-Drager syndrome patients. I would try and increase the bulk in the foods she eats. Increase the whole grains, veggies, and fruits. Try pear juice (contains more of the ruffage stuff than prune juice). If these fail, then try to titrate the ducolox to a level that helps her go to the bathroom without having explosive diarrhea. Let us know how she does. Sincerely, CCF Neuro[P] MD
