Shy-Drager syndrome The Shy-Drager Syndrome is a form of multiple system atrophy in which autonomic failure - notably sexual and bladder dysfunction, postural hypotension and loss of sweating - is associated with parkinsonism. The pathogenesis involves a disruption of the cells in the intermediolateral cell column of the spinal cord and the sympathetic ganglia. Sometimes neural destruction is more widespread, resulting in, for example, cerebellar signs.

    What is Shy-Drager Syndrome?

Is there any treatment? Shy-Drager is often difficult to treat because of the fluctuations in blood pressure. The general treatment course is aimed at controlling symptoms. Anti-Parkinson medication, such as L-dopa, may be helpful but should be used with caution because it can lower blood pressure, causing blackouts. To relieve low blood pressure, dietary increases of salt and fluid may be beneficial. Medications to elevate blood pressure such as corticosteroids may cause side effects and should be carefully monitored by a physician. Alpha-adrenergic medications, metoclopramide, ergotamine derivatives, and indomethacin are useful in many cases. Sleeping in a head-up position at night may reduce headaches and morning dizziness. An artificial feeding tube or breathing tube may be surgically inserted for management of swallowing and breathing difficulties. In rare cases, a pacemaker may be implanted to correct heart irregularities.
    What is the prognosis?

Treatments of Shy-Drager Syndrome discussion: Orthostatic hypotension in Shy-Drager syndrome is treatable, but there is not known effective treatment for the progression central nervous system degeneration. The general treatment course is aimed at controlling symptoms. Antiparkinsonian medication, such as L-dopa, may be helpful. To relieve low blood pressure while standing, dietary increases of salt and fluid may be beneficial. Medications to elevate blood pressure, such as salt-retaining steroids, are often necessary, but they can cause side effects and should be carefully monitored by a physician. Alpha-adrenergic medications, non-steroidal anti-inflammatory drugs, and sympathomimetic amines are sometimes used. Sleeping in a head-up position at night reduces morning orthostatic hypotension. An artificial feeding tube or breathing tube may be surgically inserted for management of swallowing and breathing difficulties.

    Causes and Risk
Multiple system atrophy (MSA) is a rare degenerative condition. Its symptoms are similar to those of Parkinson's disease in that patients may move slowly, be tremulous, and have a shuffling gait. However, in MSA there is more widespread neurologic damage and damage to the autonomic nervous system (the portion of the nervous system that controls involuntary functions). There is progressive degeneration of the nervous system, with damage to all parts of the nervous system, including the brain. The cause is unknown. MSA develops gradually and is most often diagnosed in men older than 60.

Multiple system atrophy (MSA) with postural hypotension, meaning that you get low blood pressure when you stand (and possibly faint), even if you have high blood pressure the rest of the time. Treatment is possible for some management of a nasty set of symptoms (constipation, impotence in men, urinary incontinence, impaired speech, difficulties with breathing and swallowing, and inability to sweat), but it's incurable.

A patient with Shy-Drager syndrome exhibited a partial defect in antidiuretic hormone (ADH) release, and cluster breathing, an indication of pontomedullary respiratory center damage, with a normal CO2 response curve. This extends the spectrum of abnormalities associated with the degenerative disease of the central nervous system. The presence of a pontomedullary respiratory pattern without an impaired CO2 response curve suggests that neurons that determine respiratory rhythm function independently from those that function as chemoreceptors.

(Multiple System Atrophy with Postural Hypotension; Striatonigral Degeneration)
by Debra Wood, RN Definition Causes Risk Factors ... Organizations
    Definition
Shy-Drager syndrome is a degenerative disorder of the nervous system. Multiple areas of the nervous system can be affected, such as the parts that control motor and involuntary functions. The motor system controls movement and balance. The involuntary, or autonomic, nervous system controls body functions (for example, blood pressure and bowel and bladder function). Shy-Drager syndrome may cause postural hypotension. Postural hypotension is a drop in blood pressure after changing position.
    Causes
It is unknown what causes the degeneration or nerve loss in Shy-Drager syndrome.
    Risk Factors
A risk factor is something that increases your chances of getting a disease or condition. Risk factors for Shy-Drager syndrome include:
    • Age: Middle age or older Sex: Male
    Symptoms
Symptoms associated with the involuntary nerves usually occur first. Motor problems similar to those found with
