Abstract Short-bowel syndrome is defined as a malabsorption state caused by intestinal resection. The symptomatology of patients with short-bowel syndrome covers a spectrum from chronic watery diarrhea to severe panmalabsorption, depending upon length and site of remaining intestine. The majority of patients can be managed on oral intake with supplements ]. A subset of patients, usually with jejunostomies and a short jejunal remnant (less than 200 cm) have high stomal outputs and require parenteral fluid and electrolyte supplements ]. Another subset with even shorter jejunal remnants develop severe nutrient malabsorption and need long-term parenteral alimentation. Cyclosporine absorption in two patients with short-bowel syndrome
R Roberts, IS Sketris, I Abraham, ML Givner, and AS MacDonald We present the cases of two patients with short-bowel syndrome who failed to achieve therapeutic cyclosporine serum concentrations on oral drug but were successful on intravenous administration. One patient received cyclosporine after renal transplantation for renal failure secondary to enteric oxalosis; the second received cyclosporine for active Crohn's disease. The rapid bowel transit time was the critical factor in limiting cyclosporine absorption in both cases. In studying oral and intravenous pharmacokinetic profiles, we support a zero-order kinetic model for oral cyclosporine absorption.

I rritable bowel syndrome (IBS) is a benign, chronic symptom complex of altered bowel habits and abdominal pain. It is considered a functional type of bowel disorder. That is, no organic or structural cause can be detected (using currently available diagnostic modalities) to explain its symptoms. It is the most common functional disorder of the gastrointestinal (GI) tract. Other disorders in this group include functional dyspepsia, functional anorectal pain, and noncardiac chest pain. The many forms and presentations of IBS can make the diagnosis challenging, and its functional nature can make a satisfactory treatment regimen difficult. See page 1856 for definitions of strength-of-evidence levels contained in this article.

Irritable bowel syndrome is the commonest bowel disorder. It is also known as spastic colon, colitis, spastic bowel, nervous colitis or mucous colitis. About 20% of people have it at some time throughout their life and at any time about 5% of adults have symptoms consistent with IBS.
What is Irritable Bowel Syndrome?
If you have at least three months of
  • abdominal (stomach) pain with one or more of the following
    • relief by defaecation (opening the bowels, passing stool)
    • change in frequency
    • change in consistency of stool (waste/motions)
    and
  • irregular pattern of defaecation for at least two days a week which involves at least three of:
    • altered stool frequency
    • altered stool form (hard/loose)
    • altered stool passage (straining/urgency/sense of incomplete evacuation [emptying of the bowel])
    • passage of mucus (slime)
    • bloating or feeling of abdominal distension (swelling)
    then you have Irritable Bowel Syndrome. Similar symptoms include loose or watery stools, rushing to the toilet, needing to go to the toilet just after going, and needing to loosen clothes after eating, passage of too much wind or gas.
