Short bowel syndrome is a group of problems affecting people who have had half or more of their small intestine removed. The most common reason for removing part of the small intestine is to treat Crohn's disease. Diarrhea is the main symptom of short bowel syndrome. Other symptoms include cramping, bloating, and heartburn. Many people with short bowel syndrome are malnourished because their remaining small intestine is unable to absorb enough water, vitamins, and other nutrients from food. They may also become dehydrated, which can be life threatening. Problems associated with dehydration and malnutrition include weakness, fatigue, depression, weight loss, bacterial infections, and food sensitivities. Short bowel syndrome is treated through changes in diet, intravenous feeding, vitamin and mineral supplements, and medicine to relieve symptoms.

DEFINITION OF SHORT BOWEL SYNDROME Short bowel syndrome is a collection of signs and symptoms occurring after surgical intestinal resection characterized by weight loss and malabsorption of fluids and micro- and macronutrients. The normal small intestine is approximately 600 cm in length and the colon is approximately 150 cm. There are exceptions to such length limits as described above. Estimates of the length of the remaining small bowel and its health are not always accurate from surgical, endoscopy, or radiographic reports. Also, the remaining intestine can compensate or adapt with time. The results of this can be observed clinically in the form of patient weight gain, reduced intestinal fluid losses, and stabilization of micronutrients. These changes usually are clinically obvious approximately 3 months after intestinal resection and may continue for years.

What is going on in the body?
The small bowel carries food from the stomach to the large bowel, which attaches to the rectum and anus. It plays an important role in digestion. Vitamins, minerals, salts, and important fats are absorbed through the small bowel.
Removal of part of the small bowel decreases the area to absorb these nutrients. Many people can tolerate removal of up to 50% to 60% of the small bowel without having problems. However, when more than this amount or a key part of small bowel is removed, symptoms often occur. These symptoms make up what is called the short bowel syndrome.
What are the signs and symptoms of the condition?
The main symptoms of this condition are abdominal pain, or distress , and diarrhea . Malabsorption, or poor absoption, of important nutrients may lead to weight loss , low blood counts including anemia , and vitamin deficiencies.

Short Bowel Syndrome page 248 The most common cause of massive resection of the small bowel is small bowel ischemia, due to thrombosis or embolism of the superior mesenteric artery, thrombosis of the superior mesenteric vein, or low flow in the splanchnic vessels. Less commonly, volvulus, strangulated hernias, Crohn's disease, neoplasm and trauma necessitate massive resection. Two major types of diarrhea can develop after massive ileal resection. One is induced primarily by malabsorbed bile acids, and the other by malabsorbed fat. When the ileal resection is small (less than 100 cm), hepatic synthesis of bile acids is sufficient to compensate for increased fecal losses. The luminal concentrations of bile acids are maintained within the micellar range, and significant steatorrhea does not occur. However, with inadequate absorption in the terminal ileum, bile acids enter the colon, impairing electrolyte and water absorption. Thus the term "bile acid diarrhea" is applied to this circumstance.

Short bowel syndrome or SBS is one of the most serious complications of surgical treatments for Crohn's disease. It is a result of the loss of a significant part of the small intestine (small bowel) due to disease or surgery. Crohn's disease is the leading cause of Short Bowel Syndrome. It can also be caused by other diseases or traumatic injury to the small intestine. The most serious effect of Small Bowel Syndrome is malabsorption. Malabsorption is the inability to properly absorb nutrients and fluid from the foods you eat. The symptoms of malabsorption include weight loss, chronic diarrhea, dehydration, abdominal bloating, fatigue, depression and fat in the stool. Small Bowel Syndrome may have a major impact on the quality of life of those who suffer from it. When part of the small intestine is removed, the remaining intestine adapts to compensate for this loss. It is possible that your body may not adapt quickly enough or sufficiently to restore all lost function. The degree of complications following surgery of the small intestine, also known as resection, is affected by what portion is taken out. There are three sections in the small intestine: the duodenum it the first part, which connects to the stomach, the jejunum is the middle part, and the ileum is the last part, which connects to the colon. The bulk of digestion and absorption take place in the duodenum and the first part of the jejunum. In fact, the entire jejunum can be removed without much disability except for lactose intolerance, because the ileum can compensate for this loss.
