11.18 Herpes Zoster (Shingles)
Presenhtion: Patients complain of pain, paresthesia, or an itch that covers a specific dermatome and then develops into a characteristic rash. Prior to the onset of the rash, zoster can be confused with pleuritic or cardiac pain, cholecystitis, or ureteral colic. Approximately 3-5 days from the onset of symptoms, an eruption of erythematous macules and papules will appear, first posteriorly then spreading anteriorly along the course of the involved nerve segment. In most instances grouped vesicles will appear within the next 24 hours. Herpes zoster most often occurs in the thoracic and cervical segments.
What to do:
  • Prescribe acyclovir (Zovirax) 800mg q4h (five times a day, skip a dose at night) or famiclovir (Famvir) 500mg tid x7d.
  • Prescribe analgesics appropriate for the level of pain the patient is experiencing. Anti-inflammatory medications may help, but narcotics are often required (e.g., Percocet q4h).
  • Cool compresses with Burow's solution will be comforting (e.g Domeboro powder, 2 pkts in 1 pint of water).

What is shingles?
Shingles (herpes zoster) is a viral infection of the nerve roots . It causes pain and a band of rash that spreads on one side of your body. Shingles is most common in older adults and people who have weakened immune systems because of stress, injury, certain medications, or other factors.
What causes shingles?
Shingles is caused by the reactivation of the virus (varicella-zoster virus) that causes chickenpox . After you recover from chickenpox, the virus lies dormant (inactive) in your nerve roots. In some people, it stays dormant for the rest of their lives. In others, the virus reactivates when disease, stress, or aging weakens the immune system The cause of reactivation of the virus is unclear; however, once it does reactivate, it causes shingles, not another case of chickenpox. Most people who get shingles will not get the disease again, although it does come back in a few people.

Doctors call it postherpetic neuralgia or PHN. It's caused by nerve damage left behind by a case of shingles. Shingles itself comes from reactivation of a chickenpox virus, varicella zoster. The virus travels down nerve fibers to cause a painful skin rash. When the rash goes away, the pain usually goes with it. But for 12% to 15% of people the pain remains. If your shingles pain lasts eight to 12 weeks after the rash goes away, you're part of an "unfortunate minority," says pain researcher Andrew S.C. Rice, MD, of Imperial College, London. "Among people with PHN, some have their pain resolve in the first year to 18 months after the shingles rash goes away," Rice tells WebMD. "But if they have pain longer than that, it is not going to go away on its own. In either case, a person must deal with the pain."

    Overview Shingles is caused by the varicella-zoster virus, the same virus that causes chickenpox. After you've had chickenpox, the virus lies dormant in your nerves. Years later, the virus may reactivate as shingles. Although painful, typically shingles isn't a serious condition. Sometimes, however, the rash can lead to a debilitating complication called postherpetic neuralgia. This condition causes the skin to remain painful and sensitive to touch for months or even years after the rash clears up. Early treatment can help shorten a shingles infection and reduce the risk of complications.

Chicken Pox/Shingles What is chicken pox?
Chicken pox is a common disease caused by the varicella zoster virus which is a member of the herpes virus family. It is very contagious and usually occurs during childhood. Adults can get the disease if they did not have it in childhood. What are the symptoms? Chicken pox most often begins with a slight fever, body aches and loss of appetite. Within 1-2 days, the rash appears usually starting on the chest or back. Initially the rash begins as red spots which then rapidly form blisters and spreads to the rest of the body. The blisters open and form a crust or scab within a few days. The rash can continue to break out for 4-5 days as older lesions crust and heal. Itching can accompany the rash along with fever, swollen lymph nodes, sore throat and general body aches. It is unusual to have chicken pox more than once. Is chicken pox contagious? Chicken pox is contagious through direct contact with the fluid in the blisters and by airborne droplets that are inhaled. Incubation period is 7-21 days. A person is contagious from 1-2 days before the rash appears and until all the lesions form crusts.

My wife has been going through a very frustrating and, at times, very painful problem for over a month since first being diagnosed with shingles. She had a doctor's prescription covering the first week of shingles. Since then, the skin eruptions under her right breast and around to her back have dried out, but the redness still appears. She has been treating the areas with zinc oxide, an ice pack and a lotion called Sarna, which has given her some relief from itching and other discomforts. I know shingles is a virus that affects the nerves, but I'm surprised at how little information is readily available for help. Could you please explain this disease? A: Shingles, known medically as herpes zoster, is caused by the varicella-zoster virus, the same one that causes chickenpox in children. The virus remains dormant in the nerves for years, but gets reactivated in about 20% of the general population at some time later in life. About 70% of cases occur in people over the age of 50, and up to 50% of people who live to be 80 will experience shingles. All races and genders are affected equally by shingles that can occur at any season or time of the year.

Please write about shingles. My bout with shingles apparently attacked my sciatic nerve and my rash was on my butt and ankle. For three months hence, I have had severe pain in my buttock and leg. Could there be something else wrong? My doctor tells me this is the way shingles work. A: Shingles, or herpes zoster, is a reactivation of the virus that causes chicken pox. If you have had chicken pox, you have that virus lying dormant in the nerve cells of your body. In most people, the virus stays dormant and never bothers them again. But in others, usually older people or people with medical illness, the virus "wakes up" and causes a blistering rash, similar to the chicken pox rash, but only on parts of the body. The involved area corresponds to the nerve that now has the active virus. Shingles causes numbness, itching or severe pain followed by clusters of blister-like lesions in a strip-like pattern on one side of the body. The pain can persist for weeks, months or even years after the rash heals.

A patient information handout on shingles, written by the authors of this article, is provided on page 2447. H erpes zoster results from reactivation of the varicella-zoster virus. Unlike varicella (chickenpox), herpes zoster is a sporadic disease with an estimated lifetime incidence of 10 to 20 percent. The incidence of herpes zoster increases sharply with advancing age, roughly doubling in each decade past the age of 50 years. Herpes zoster is uncommon in persons less than 15 years old. In a recent study, patients more than 55 years of age accounted for more than 30 percent of herpes zoster cases despite representing only 8 percent of the study population. In this same study, children less than 14 years old represented only 5 percent of herpes zoster cases. The normal age-related decrease in cell-mediated immunity is thought to account for the increased incidence of varicella-zoster virus reactivation. Patients with disease states that affect cell-mediated immunity, such as human immunodeficiency virus (HIV) infection and certain malignancies, are also at increased risk. Chronic corticosteroid use, chemotherapy and radiation therapy may increase the risk of developing herpes zoster.
