Herpes Zoster (Shingles) I. Definition: Shingles (herpes zoster) is a nerve infection caused by the chicken-pox virus. Shingles results from reactivation of the chicken-pox virus that remained in your body since you had chicken poxperhaps many years ago. II. Causes:
  • Herpes zoster (shingles) is caused by the chicken-pox virus.
III. Symptoms:
  • The rash of shingles begins as red patches that soon develop blisters, often on one side of the body. The blisters may remain small or can become large. They heal in two to four weeks. They may leave scars. Many patients mistakenly believe that "nervousness" causes shingles. this is wrong; shingles is a viral infection of a nerve and has nothing to do with being "nervous." Shingles is often painful, this is because the virus travels along the nerve to get to the skin resulting in inflammation and damage to the nerve.
III. Treatment:
  • Acyclovir helps to control the skin eruption. It is not a cure, but helps to heal the rash, provided treatment is started early. Acyclovir is taken by mouth. Even taking acyclovir may not eliminate discomfort, burning sensations, or pain. If the discomfort is mild, take Tylenol or a similar mild painkiller.

If you want to know about the very common (but often feared) herpes simplex virus - the cause of genital herpes - this on-line resource can help you. The HVA can answer any question you may have. What other 'herpes' sites don't tell you
  • Most people have herpes simplex - 70% have facial herpes (cold sores) and 10% have genital herpes in the UK. These figures are even higher in other countries including the USA and the developing world. Three quarters don't know they have it: 1 in 4 will have no symptoms; 2 in 4 will have only mild symptoms and are unlikely to be diagnosed; 1 in 4 will have more noticeable symptoms and will be diagnosed. Herpes simplex is seldom of any medical importance - some people are quite ill when they catch it but so are some people who catch flu.

    Famciclovir is used to treat shingles (herpes zoster) as well as herpes simplex virus infections which can cause painful blisters and lesions. Famciclovir reduces the severity of outbreaks and prevents the development of new blisters. Famciclovir has also been shown to be effective for preventing genital herpes outbreaks. Managing side effects
    Famciclovir has few side effects. The main side effects seen in studies were headache and nausea. Dosage
    For shingles the recommended dosage is 500 mg every 8 hours for 7 days. For genital herpes simplex infections, the recommended dose is 125 mg twice a day for 5 days. The dosage used to prevent genital herpes outbreaks is 500 mg twice a day. In people with reduced kidney function, the dosage should be reduced. The amount of dose reduction is dependent on how well the kidney is functioning. How long it may take to work
    Famciclovir is usually given for 7 days to treat shingles and 5 days to treat genital herpes simplex infections. Managing side effects and drug interactions
    Other drugs such as probenecid that are processed through the kidneys may increase the amount of famciclovir in the body. This might increase the chances of getting side effects from famciclovir.

    Shingles Overview
    Shingles is a reactivation of the herpes zoster virus (varicella-zoster virus, or VZV). This same virus causes the childhood illness chickenpox. The chickenpox virus (varicella) remains in a dormant state in the body in the root of nerves that control sensation. In about 1 out of 5 people, the virus "wakes up," often many years after the chickenpox infection. The virus then travels along a sensory nerve into the skin causing a painful rash known as shingles.
    • Shingles is derived from the Latin and French words for belt or girdle, reflecting distribution of the rash in a broad band. This band is usually only on one side of the body and represents a dermatome Anyone who has had the chickenpox infection or vaccine can get the herpes zoster virus that causes shingles. Older people and those with cancer, HIV, or organ transplants have a decreased ability to fight off infection and a greater chance of getting shingles. The majority of people with shingles, however, are healthy. No special tests need to be done to see if your immune system is strong.

    Anyone with the varicella-zoster virus in their body can be at risk for getting shingles.
    Right now there is no way of knowing who will get the disease. But, there are things that make you more likely to get shingles.
    • Advanced age . The risk of getting shingles increases as you age. People have a hard time fighting off infections as they get older. The chance of getting shingles becomes much higher by age 70. Trouble fighting infections . Your immune system is the part of your body that fights off infections. Age can affect your immune system. So can an HIV infection, cancer, cancer drugs, radiation treatments, or organ transplant. Even stress or a cold can weaken your immune system for a short time and put you at risk for shingles.
