Many different diseases can cause diarrhea and bloody diarrhea. Shigellosis is a bacterial disease, also called "Bacillary dysentery," that is found throughout the world. It is especially prevalent in developing countries where sanitation methods are poor. This disease is passed from person to person, and can be stopped by frequent and thorough hand washing, and good sanitation. Shigellosis is a continuing problem for American troops and travelers stationed in or visiting areas of the world endemic for Shigella. Antibiotics are effective against shigellosis but the ongoing development of antibiotic resistant Shigella requires continual emphasis on alternative therapies and efforts at primary prevention. What is Shigellosis?
Shigellosis is an acute intestinal infection caused by one of the four serotypes of Shigella bacteria. The severity of the illness depends on the Shigella type and the nutritional status of the patient. Shigella dysenteriae 1 is often associated with serious disease and severe complications that include toxic megacolon and the hemolytic-uremic syndrome.
How Shigellosis Is Spread?
The germs leave the body in the stool (bowel movements) of an infected person and infect another person when hands, food or objects (toys, pens, etc.) contaminated with the stool, are placed in the mouth. Hand washing is the best way to prevent the spread of Shigella.

What is shigellosis?
Shigellosis is a bacterial infection that affects the intestines. It is a fairly common disease. Who gets shigellosis?
Anyone can get shigellosis but it is recognized more often in young children. Children in day care centers, travelers to certain foreign countries, institutionalized people, and active homosexuals are at greatest risk. How is the shigella bacteria spread?
Shigella bacteria are found in the intestines and stool of infected people who, in turn, may contaminate food or water. The bacteria are spread by direct contact with an infected person, by eating or drinking contaminated food or water, or by contact with a contaminated object. What are the symptoms?
People infected with the Shigella bacteria may have mild or severe diarrhea (often with traces of blood or mucous); abdominal cramping, nausea, and vomiting. Some infected people may not show any symptoms. How soon do symptoms appear?

Shigellosis
Shigellosis is an infectious disease caused by a group of bacteria called Shigella . Most who are infected with Shigella develop diarrhea, fever, and stomach cramps starting a day or two after they are exposed to the bacterium. The diarrhea is often bloody. Shigellosis usually resolves in 5 to 7 days. In some persons, especially young children and the elderly, the diarrhea can be so severe that the patient needs to be hospitalized. A severe infection with high fever may also be associated with seizures in children less than 2 years old. Nonhuman primates are the only important reservoir for Shigella infection in animal facilities, although zoonotic transmission of the organism from other laboratory research animals has been recorded under unique circumstances. Nonhuman primates can harbor several Shigella spp. that are pathogenic for humans, including S. flexneri, S. sonnei, and S. dysenteriae Infection with the gram-negative, non spore-forming bacilli of Shigella spp. is extremely common among nonhuman primates. The most frequently found species is Shigella flexneri . Shigellosis is the most frequently identified cause of diarrhea in these animals, and is a significant zoonotic disease. Acute shigellosis in nonhuman primates is often fulminant and fatal. It is characterized by diarrhea with mucus and blood, prostration, edema of the face and neck, and rectal prolapse. Animals that recover from an episode often remain asymptomatic carriers and, although they may have negative fecal cultures, they may also have recurrent acute episodes in times of stress.

Moyer, Ph.D. Among the national health objectives of the year 2000 is the goal of reducing the number of cases of diarrheal diseases in children attending daycare centers by 25%. Children are at increased risk for diarrheal diseases because they engage in oral exploration of their environments and their toileting hygiene is imperfect. It is estimated that 25% of preschool children attend day care. While there are many infectious agents causing diarrhea in children, one of the most difficult diseases to control from the epidemiological perspective is shigellosis. Shigellosis is an acute disease characterized by abdominal pain and diarrhea, often containing blood, mucus and pus. by Nelson Moyer, Ph.D., D(ABMM), Program Manager The Centers for Disease Control and Prevention (CDC) recently released the annual tabulation summary from the National Shigella Surveillance System for the year 1999. Shigella isolates from the fifty states and the District of Columbia are reported to CDC electronically as they occur using the Public Health Laboratory Information System (PHLIS). At CDC, the data are tabulated and summary reports are prepared for state epidemiologists and laboratory directors. Clinical Features
  • The onset of clinical illness is often characterized by toxemia malaise , and high fever followed by several hours of watery diarrhea. The dysenteric phase which follows is manifested by lower abdominal cramps, tenesmus and the passage of scanty stools of blood and mucus. Reiter's syndrome is a late complication of S. flexneri infection, particularly among patients having the histocompatibility antigens HLA-B27. The hemolytic-uremic syndrome is a potential complication associated with S. dysenteriae infection.
Transmission
  • Transmitted primarily through the fecal-oral route. The ingestion of as few as 10 Shigella organisms can cause overt clinical illness.Detection of schistosome eggs in feces, urine, or rectal biopsy specimen. Less commonly transmitted by contaminated food or water. Houseflies may serve as mechanical vectors in many undeveloped areas.

Shigellosis
Shigellosis is a diarrheal illness caused by the Shigella group of bacteria. Infection is spread by the fecal-oral route. Only a few bacteria are needed to cause an infection and, unlike many of the diarrheal agents in child care settings, shigella may spread through groups of children who are toilet trained as well as through groups of children who are in diapers. Depending on the infectious dose, infection with shigella may be very mild or it may result in severe bloody diarrhea, fever, cramping, nausea and vomiting. Numerous outbreaks have been reported from child care settings. Children may spread infections acquired in child care facilities to their parents and siblings and whole families may be ill within a matter of days. Deaths have been reported from this illness and it is one of the more serious infections providers are likely to encounter in the child care setting. If you suspect a case of shigellosis in your child care facility: Contact your state or local health department. Prompt intervention may help prevent the spread of shigellosis to others and your health department should be in a position to give assistance and advice. Exclude the ill child and any children who subsequently develop diarrhea from child care until they no longer have diarrhea and have been shown to be free of the

What is shigellosis?
Shigellosis is an infectious disease that causes diarrhea and may be easily spread. About 1,100 cases of shigellosis were reported in Canada in 1999. It is more common in summer than winter. Children ages 2 to 4 are most likely to get shigellosis.
What causes shigellosis?
Shigellosis is caused by the bacterium shigella. The disease is spread when the bacteria in feces (stool) or on soiled fingers is ingested. This happens when basic hygiene and hand-washing habits are inadequate or when food is contaminated and then eaten.
  • Shigellosis is particularly likely to occur among toddlers who are not fully toilet trained. Family members and playmates of infected children are also at high risk of becoming infected. Shigella bacteria are present in the diarrhea of people with shigellosis and for 1 or 2 weeks after symptoms have stopped. Food may become contaminated by infected food handlers who do not wash their hands with soap after using the bathroom.
