SHIGELLOSIS What is shigellosis? Shigellosis is an infectious disease caused by a group of bacteria called Shigella . Most people who are infected with Shigella develop diarrhea, fever and stomach cramps a day or two after being exposed to the bacterium. The diarrhea is often bloody. Shigellosis usually resolves in five to seven days. In some persons, especially young children and the elderly, the diarrhea can be so severe the patient needs to be hospitalized. A severe infection with high fever also may be associated with seizures in children younger than 2 years of age. Some persons who are infected may have no symptoms at all, but may still pass the Shigella bacteria to others. What sort of germ is Shigella The Shigella germ is actually a family of bacteria that can cause diarrhea in humans. These microscopic living creatures, which can pass from person to person, were discovered 100 years ago by a Japanese scientist named Shiga, for whom they are named. There are several kinds of Shigella bacteria but only two are common in the United States.

What is shigellosis?
Shigellosis is a bacterial disease caused by infection with Shigella bacteria. Shigellosis can affect anyone; however, children, people with poor immune systems, and the elderly are at the greatest risk.
How do you catch shigellosis?
Shigellosis is passed from person to person by the faecal-oral route, by direct or indirect contact with faecal material. This commonly occurs if hands are not washed properly, particularly after going to the toilet or changing nappies, and as a result of sexual contact. Shigella infections may also be acquired from eating food contaminated with the bacteria. Flies can also carry Shigella and can contaminate food. A person can have Shigella present in their stools for some weeks and remain asymptomatic, and still pass on infection to others.

Shigellosis (pronounced: shih-guh- lo -sus) is an intestinal infection caused by Shigella (pronounced: shih- geh -luh) bacteria. The bacteria produce toxins that can attack the lining of the large intestine, causing swelling, ulcers on the intestinal wall, and bloody diarrhea. Symptoms can range from just watery diarrhea to bloody diarrhea, fever, and abdominal pain. Shigella bacteria can contaminate food and water supplies, especially in areas where the sanitation is not adequate. However, most of the time the bacteria are spread when a person comes into contact with another person's stool (poop). That's one of the reasons why it's important to wash your hands after using the rest room. It takes as few as 10 of these bacteria to cause an infection. That means that

    Disease reporting
    Health-care providers and clinical laboratories are required to report cases and suspect cases of Shigellosis to local health departments within one working day of identification.
    Cases are subject to restriction on school and day-care attendance, food handling, and patient care while in the communicable stage of the disease, or for the duration of any diarrhea and/or vomiting.
    In general, restrictions on cases with Shigellosis or Shiga-toxigenic Escherichia coli infection shall not be lifted until results of licensed laboratory tests of two consecutive approved fecal specimens collected not less than 24 hours apart show no identifiable pathogens. If sufficient measures have been taken to prevent transmission, or the disease is no longer communicable, worksite, child-care and school restrictions can be removed at the discretion of the local public health authority; school restrictions can be removed by a school nurse or health care provider; and health-care facility restrictions can be removed by the facility's infection control committee.

    What is Shigellosis?

    What are the symptoms of Shigellosis?
    The most common symptoms of this disease are diarrhoea (which may contain blood, mucus and pus), fever, stomach cramps, nausea and vomiting. After the bacteria are taken in by mouth, it usually takes between one and three days before you become ill. To trace the cause of the illness, it is necessary to know what you ate and drank and where you were in the three days before you became ill. Illness usually lasts from four to seven days. Rarely, infection with some types of Shigella bacteria can be life threatening.
    Where are Shigella found?
    Shigella bacteria are only found in humans.
    How does Shigellosis spread?
    Shigellosis occurs when Shigella bacteria are taken in by mouth and the most common way this happens is by person-to-person spread. People with shigellosis have Shigella bacteria in their faeces. If these people do not wash their hands properly after going to the toilet, then contaminated hands can spread the bacteria to surfaces and objects which will be touched by other people. Contaminated hands can also spread the bacteria to food which may be eaten by other people.

    The genus Shigella consists of four species:
    • Group A: Sh. dysenteriae Group B: Sh. flexneri Group C: Sh. boydii Group D: Sh. sonnei
    Groups A, B and C are further divided into approximately 40 serotypes, designated by numbers.
    Identification
    Clinical features
    Shigellosis is characterised by an acute onset of diarrhoea, fever, nausea, vomiting and abdominal cramps. Typically the stools contain blood, mucus and pus, although some persons will present with watery diarrhoea. Complications include toxic megacolon and reactive arthritis. Rarely haemolytic uraemic syndrome can occur. The infectious dose required to produce disease is low and may be as few as ten organisms. Illness is usually self-limited and lasts from several days to weeks with an average of four to seven days. The severity of infection depends on host factors such as age and nutritional status and the serotype. Infections with Sh. sonnei usually result in a short clinical course and low case fatality rate. In contrast, Sh. dysenteriae is often associated with serious disease and a high case fatality rate.
