Sexual difficulties can begin early in a person's sex life or they may develop after an individual has previously experienced enjoyable and satisfying sex. A problem may develop gradually over time, or may occur suddenly as a total or partial inability to participate in one or more stages of the sexual act. The causes of sexual difficulties can be physical, psychological, or both. Emotional factors affecting sex include both interpersonal problems (such as marital/relationship problems, or lack of trust and open communication between partners) and psychological problems within the individual ( depression , sexual fears or guilt, past sexual trauma, and so on).

It still remains an exciting, yet sensitive and controversial subject. Although the evaluation and treatment of sexual problems in men have focused almost exclusively on erection, female sexual responses have proved much more difficult to quantify. How do you describe what is "normal"? And what do we diagnose as a "dysfunction," when female sexuality has more than just the mechanical aspect to be taken into consideration. Evaluation of the Patient with Erectile Dysfunction The evaluation of the male with erectile dysfunction consists of three distinct parts, namely, a structured interview, physical examination, and adjunctive testing. While the role of the latter is debated for the general erectile dysfunction population, there are some patients who benefit from identifying a cause of the ED. All patients presenting for the evaluation of impotence should undergo a comprehensive history and a focused physical examination. The purpose of this approach is to define (1) that the patient suffers from sexual dysfunction and to determine the nature of this, (2) that the patient has adequate exercise tolerance to resume sexual relations, (3) that the patient does not have any conditions in his medical or surgical history that represent contraindications to specific therapies and (4) to seek out factors in the patient's medical surgical and sexual history that may indicate an etiology of his sexual dysfunction. History
It is unknown what percentage of patients presenting to a urologist for nonsexual complaints ever get questioned regarding their sexual function. However, it has been estimated that only 30 percent of all men presenting to primary care physicians are asked questions regarding sexual function. The reason for this is most probably multifactorial, with both patient and physician factors playing a role. From a patient standpoint, it is widely appreciated that embarrassment and fear play a major role in a patient¹s reticence to broach this subject. From a physician standpoint, factors interfering with initiating dialogue regarding sexual function include lack of physician time, lack of physician interest, lack of physician knowledge, and cultural and religious issues.

Erection problems associated with radical prostatectomy Over the past two decades the number of cases of prostate cancer has soared. Approximately 50,000 new cases were diagnosed in 1981 and in 1997, more than 200,000 new cases were diagnosed. As the overall incidence of prostate cancer increases, the average age at the time of diagnosis has dropped dramatically from 72.2 years in 1983 to 69 years in 1994. These two factors combined have made sexual dysfunction following radical prostatectomy a far more important issue. Most urologists are aware that erectile dysfunction is frequent following radical prostatectomy (RP). Most fail to appreciate and/or fail to inform their patients regarding the other sexual dysfunctions, which include the absence of ejaculation (although orgasm is generally preserved), and also possible penile curvature or length loss. What Is Sexual Dysfunction?
Sexual dysfunction is a quality-of-life issue for men and women. Men may have libido (sex drive), ejaculatory or erectile difficulties. Some form of sexual dysfunction affects half of all men over age fifty. Women may suffer from difficulties with libido, sexual arousal, orgasmic function or vaginal lubrication. A recent survey in The Journal of the American Medical Association (JAMA) stated that more than 40 percent of women may be affected by these types of sexual dysfunctions. In addition, testosterone deficiency may be present in both men and women.

In contrast to men, whose erectile dysfunction increases with age, women are much less likely to have age-related sexual dysfunction, according to a paper by an expert on sexual relationships at the University of Chicago and based on a new global study to be presented today, October 10. The survey, the Global Study of Sexual Attitudes and Behaviors , funded by Pfizer, Inc., is based on in- person and telephone interviews in 2001 with 27,500 men and women aged 40-80 in 30 countries worldwide. Researchers looked at equal numbers of men and women equally distributed across four decades. The survey asked about a range of health conditions such as hysterectomies, depression and other illnesses, including diabetes, heart disease, and hypertension. The researchers assessed whether aging increased the risk of sexual dysfunction, even when these health conditions were taken into account. The research team examined sexual dysfunction among men and women as well as other topics related to aging and sexuality. The cross-country differences of sexual dysfunction were reported for persons experiencing the problem for two months or more. Among men, the team found increasing problems with erectile dysfunction (ED) as men age. Among women, the researchers looked at these factors: lack of interest in sex, inability to experience orgasm, pain during intercourse, lack of sexual pleasure and lubrication difficulties. The pooled global survey found that 31 percent of women lacked interest in sex, 22 percent were unable to orgasm, 21 percent found sex unpleasurable, 20 percent had trouble lubricating, and 14 percent experienced pain with sex.
