Professional and public interest in female sexual dysfunction has recently been sparked by developments in several areas including the investigation of new drug treatments. Approximately 43 percent of American women suffer from sexual dysfunctionperhaps an actual number as high as 50 million American women. The highest proportion occurs in young women between the ages of 18 and 29. (Study by Edward O. Laumann, PhD; Anthony Paik, MA; Raymond C. Rosen, PhD published in JAMA 2/9/99.) The lack of desire appears to be the number one, most significant complaint, followed by arousal and lubrication dysfunctions. Twenty-one percent of those surveyed experienced physical pain during sex, while 27 percent reported that intercourse was not enjoyable. Sixteen percent admitted to sexual anxiety. The prevalence of female sexual dysfunction continues to be documented and the topic has generated discussions in both the medical and lay communities (60 Minutes, Larry King, Newsweek, Oprah).

Our answers to "Good Morning America" questions Female Sexual Dysfunction:
by Jennifer Berman, MD
and Laura Berman, PhD Female Sexual dysfunction is age-related, progressive and highly prevalent affecting 30-50 percent of women . Based on the National Health and Social Life Survey of 1,749 women, 43 percent experienced sexual dysfunction. U.S. population census data reveal that 9.7 million American women ages 50-74 self-report complaints of diminished vaginal lubrication, pain and discomfort with intercourse, decreased arousal, and difficulty achieving orgasm. Female sexual dysfunction is clearly an important women's health issue that affects the quality of life of many of our female patients. Until recently, there has been little research or attention that focuses on female sexual function. As a result, our knowledge and understanding of the anatomy and physiology of the female sexual response is quite limited. Based on our understanding of the physiology of the male erectile response, recent advances in modern technology, and recent interest in Women's Health issues, the study of female sexual dysfunction is gradually evolving. Future advances in the evaluation and treatment of female sexual health problems are forthcoming.

Sexual difficulties can begin early in a person's sex life or they may develop after an individual has previously experienced enjoyable and satisfying sex. A problem may develop gradually over time, or may occur suddenly as a total or partial inability to participate in one or more stages of the sexual act. The causes of sexual difficulties can be physical, psychological, or both. Emotional factors affecting sex include both interpersonal problems (such as marital/relationship problems, or lack of trust and open communication between partners) and psychological problems within the individual ( depression , sexual fears or guilt, past sexual trauma, and so on).

Selective Serotonin Reuptake Inhibitors And Sexual Dysfunction: A Historical Review Of The Development Of Evidence For An Adverse Effect Anita K. Wagner, Pharm.D., M.P.H. ), announced a joint venture with Icos to develop a new impotence drug, a "product [that] is being talked about as a next-generation Viagra." The companies indicated that they also intend to develop treatments for women who have sexual dysfunction.(2) These excerpts from the general news media reflect several of the insights patients, clinicians, researchers, and drug manufacturers have gained over the 12 years since FDA approval of fluoxetine into one of the SSRI’s disturbing adverse effects: SSRI affect different aspects of men’s and women’s sexual functioning, at higher rates than originally detected; these effects are not easily separated from the underlying disorders for which they are prescribed; patients prefer not to suffer from adverse sexual effects; identification of drugs to counteract them have provided additional evidence for the drug-associated sexual effects and contributed to further understanding of the neurophysiology of sexual dysfunction. In this paper, I will attempt to chronicle the development of evidence for SSRI-induced sexual dysfunction. Published reports were identified in January 1999 through a search of citations in the National Library of Medicine’s Medline

Psychology works
Most women have questions or experience some concerns about their sexuality at some point in their lives. When these concerns start to interfere with sexual activity, pleasure and satisfaction, a psychologist can be an excellent resource for women. How common is sexual dysfunction? Recent research studies in the U.S. and Europe estimate that more than 1 out of 3 women live with sexual dysfunction! Most of these women are very distressed about their problems with sexual function and satisfaction, and about the effects the sexual problem may have on their relationships. The most frequently reported problem is lack of interest in sex. This is followed by difficulties experiencing orgasm, difficulties with sexual arousal, not finding sex pleasurable, and experiencing pain and/or anxiety with sexual activity. The number of men who experience sexual dysfunction increases with age. For women, the opposite is true. With the exception of lubrication difficulties, the number of women who experience sexual dysfunction decreases with age. Female Sexual Dysfunctions Desire

  • Difficulties with physical and/or subjective sexual arousal Difficulties lubricating Difficulties sustaining arousal

Go to PDF Version Female Sexual Function/Dysfunction There are several stages that outline the body's response to sexual stimulation. These stages are fluid, and very individual. Although most females will progress through the stages in order, the amount of time spent in each stage can vary dramatically. FUNCTION Stage One - Excitement
  • Blood begins to build up (vasocongestion) in the blood vessels of the genital region. A slippery fluid seeps through the lining of the vagina, functioning as a lubricant. The clitoris begins to swell somewhat and may elongate slightly. The nipples become harder and erect and there is a darkening of the skin through the neck, breasts and upper abdomen during sexual excitement; this is termed the "sex flush." General muscular tension builds in the body as heart rate and blood pressure increase.
Stage Two - Plateau
  • The outer third of the vaginal wall becomes swollen with blood, narrowing the space within the vagina slightly. Swelling seems to create the tension that is an important precursor of orgasms, and is termed the "orgasmic platform." The clitoris retracts under its foreskin so that it no longer receives any direct stimulation.
