A patient information handout on sexual dysfunction in women, written by the author of this article, is provided on page 141 S exuality is a complex process, coordinated by the neurologic, vascular and endocrine systems. Individually, sexuality incorporates family, societal and religious beliefs, and is altered with aging, health status and personal experience. In addition, sexual activity incorporates interpersonal relationships, each partner bringing unique attitudes, needs and responses into the coupling. A breakdown in any of these areas may lead to sexual dysfunction. See editorial
on page 52.
Primary care physicians, skilled in the treatment of medical and psychologic disorders, often feel unqualified to treat patients with sexual dysfunction. However, with an understanding of sexual functioning and application of general medical and gynecologic treatments to sexual issues, sexual dysfunction may be effectively approached with the same skills. The latter includes obtaining a complete patient history, conducting a physical examination, application of basic treatment strategies, providing patient education and reassurance, and recommending appropriate referral when indicated. FROM THE FAMILY PRACTICE INQUIRIES NETWORK Which Antidepressant Is Best to Avoid Sexual Dysfunction? Searchable Question In patients being treated for depression, which antidepressants have a low risk of sexual side effects? Evidence-Based Answer Bupropion (Wellbutrin), nefazodone (Serzone), amitriptyline (Elavil), and moclobemide (Manerix, a reversible inhibitor of monoamine oxidase type A not available in the United States) have been shown to cause less sexual dysfunction than selective serotonin reuptake inhibitors (SSRIs). [Strength of recommendation: B, based on individual randomized controlled trials (RCTs)] Among SSRIs, fluvoxamine (Luvox) may cause less sexual dysfunction than sertraline (Zoloft). [Strength of recommendation: B, single RCT] No other differences between or within classes of antidepressants have been demonstrated in RCTs. Evidence Summary The incidence of sexual side effects between different antidepressants in adults with depressive or anxiety disorders has been reported by 25 RCTs, most of which were included in two recent descriptive systematic reviews. [References 1 and 2Evidence level 1A] Bupropion led to less sexual dysfunction (or to more sexual satisfaction) than sertraline or fluoxetine in four trials. The number needed to harm (NNH), that is, the number of patients who have to take the drug for one patient to experience sexual dysfunction, ranged from two to 17, depending on the type of dysfunction.

Most women and men at one time or another experience some alteration in sexual response, and an occasional problem in this regard is not an indication of dysfunction. Even a single episode of altered response or failure to perform sexually can cause unnecessary concern and may even perpetuate the problem, since anxiety about sexual performance can diminish sexual functioning. Based on the National Health and Social Life Survey of 1,749 women, 43 percent experienced sexual dysfunction. U.S. population census data reveal that 9.7 million American women ages 50-74 self-report complaints of diminished vaginal lubrication, pain and discomfort with intercourse, decreased arousal, and difficulty achieving orgasm. Female sexual dysfunction is clearly an important women's health issue that affects the quality of life of many of our female patients. Until recently, there has been little research or attention that focuses on female sexual function. As a result, our knowledge and understanding of the anatomy and physiology of the female sexual response is quite limited. Based on our understanding of the physiology of the male erectile response, recent advances in modern technology, and recent interest in Women's Health issues, the study of female sexual dysfunction is gradually evolving. Future advances in the evaluation and treatment of female sexual health problems are forthcoming.

Female sexual dysfunction has basically been neglected because no therapy was available. With the advent of Viagra by Pfizer Pharmaceuticals , effective therapy for some forms of female sexual dysfunction may become available to the general public. Female sexual dysfunction can be classified into five basic areas including: 1) Desire problems 2) Arousal difficulties 3) Lubrication factors 4) Pelvic congestion 5) Orgasmic difficulties. For males, desire problems are minimal and usually related to hormonal difficulties. For females, desire problems occur in more than 33 percent of dysfunction cases. This is probably related to the more complex psychological aspects of female sexuality. On the other hand, many of these patients can be helped. In males, desire problems make up only five percent of all sexual dysfunction. Arousal, lubrication, and pelvic congestion issues together probably represent about half of all female sexual problems and, fortunately, is the area in which pharmacology in the near future appears to offer the greatest hope.

Sexual dysfunction is broadly defined as the inability to fully enjoy sexual intercourse. Specifically, sexual dysfunctions are disorders that interfere with a full sexual response cycle. These disorders make it difficult for a person to enjoy or to have sexual intercourse. While sexual dysfunction rarely threatens physical health, it can take a heavy psychological toll, bringing on depression, anxiety , and debilitating feelings of inadequacy. Description
Sexual dysfunction takes different forms in men and women. A dysfunction can be life-long and always present, acquired, situational, or generalized, occurring despite the situation. A man may have a sexual problem if he:
  • Ejaculates before he or his partner desires Does not ejaculate, or experiences delayed ejaculation Is unable to have an erection sufficient for pleasurable intercourse Feels pain during intercourse Lacks or loses sexual desire.
A woman may have a sexual problem if she:
  • Lacks or loses sexual desire Has difficulty achieving orgasm Feels anxiety during intercourse Feels pain during intercourse Feels vaginal or other muscles contract involuntarily before or during sex Has inadequate lubrication.

Researchers at Yale School of Medicine and the Albert Einstein College of Medicine have found that female sexual dysfunction (FSD) affects 48.2 percent of women in a new study and that these women had decreased sensation in the clitoris, which increased the risk of sexual dysfunction.
"There is a paucity of data available on FSD and this study brings attention to the possibility of a neurological cause for the dysfunction," said lead author Kathleen Connell, M.D., assistant professor in the Department of Obstetrics, Gynecology & Reproductive Sciences at Yale School of Medicine. Connell said previous epidemiological studies have shown that about 10 million women between the ages of 50 and 74 report abnormal sexual complaints, including decreased desire, inability to reach orgasm and increased pain with intercourse. In contrast to data on men, Connell said clinical trials evaluating the physiologic mechanisms responsible for sexual function in women are few, despite reports of other investigators, which suggest that sexual dysfunctions may be more common in women than men.

While it is natural to look for medical diseases or hormonal deficiencies as the sole cause, most sexual dysfunction is a combination of both physiological and psychological factors. In a recent continuing medical education article, Petok WD: A practical approach to evaluating female sexual dysfunction. OBG Management 1999;11:68-77, outline the six most commonly seen female sexual dysfunctions.
What are the most commonly seen sexual problems that women have? Petok lists six commonly seen sexual problems that present to doctors and sex therapists. They are:
  • Hypoactive sexual desire disorder (HSDD) - lack of sexual desire with a sort of passive apathy, not seeking out sex but not actively resisting when a partner initiates sex.
  • Sexual aversion disorder (SAD) - an active aversion to all genital sexual contact which may manifest itself as revulsion, fear or anger.
  • Female sexual arousal disorder (FSAD) - rare, if ever, thoughts about sex or getting "turned on" except in perhaps a theoretical way. Lay persons would term this frigidity.
