HISTORY Semantic-Pragmatic Disorder was originally defined in the literature on Language Disorder in 1983, by Rapin and Allen, although at that time it was classified as a syndrome. They referred to a group of children who presented with mild Autistic features and specific semantic pragmatic language problems. In babyhood, parents often described them as model babies or by contrast babies who seemed to cry too much. Many of these children babbled little or very late and went on using 'jargon' speech much longer than other children of the same age. Their first words were late and learning language was a hard slog. Some had other speech disorders too. Problems were usually first identified between 18 months and 2 years when the child had few if any real words.

    Semantic pragmatic disorder is a disorder in the ability to communicate. Some feel it is part of the autism spectrum and others feel it is a distinct communication disorder. It is often confused with autism and Asperger's due to the social and communication problems.

    Summary
    The WCAG guidelines advise using the clearest and simplest language appropriate. Perhaps this guideline is taken the least seriously from all WCAG , but for some, its lack of implementation can be a true accessibility hazard. Many people require clear and simple language. In this article I want to focus on one disorder where the use of unclear language prevents their participation in many aspects of society. Semantic Pragmatic Disorder (semantic = the relationship between words and their meaning; Pragmatic = making language work in context) or SPD, this is a communication disorder. It is often associated with autism, but is often a disorder on to itself. Non SPD people automatically and unconsciously build memory and knowledge of the connections between words, context and meaning, even for non visual words and non literal senses, like feelings, general sense of time, cues, metaphors, sarcasm. For people with SPD this process is impaired.

    I am the mother of a child with Semantic Pragmatic Disorder. I was asked to write this article for the Hi2u website so that people unfamiliar with this disorder can begin to understand what it means to have SPD. My hope is that it also gives teachers and other professionals involved with a child with SPD an insight to what life can be like for the parents and carers too.
    What is Semantic Pragmatic Disorder ? Semantic Pragmatic Disorder is language/communication disorder, commonly found in children on the Autistic Spectrum. The disorder's validity as a sub type of developmental language disorder, which is where it was originally defined by Rapin and Allen back in 1983, has been questioned on and off since it's introduction because of the autistic features the children often show. The professional community is slowly extending the boundaries of the spectrum to acknowledge disorders such as SPD and high functioning autism that can blend into the realms of "normality".
    Children affected by this disorder have difficulties in the area of the "triad of impairments", typically associated with Autistic Disorders, ie. social interaction, communication and imagination. Their autistic features, however, are mild, but it is important to recognise these when supporting the child.

    What is Semantic Pragmatic Disorder?
    ‘Semantic Pragmatic Disorder’ is a term which has been used, mostly in the UK and by speech and language therapists, for the last 10 -–15 years.
    The term was originally used to describe a developmental language disorder, affecting two aspects of communication: ‘semantics’ and ‘pragmatics’.
    Problems with the meanings of language may be called ‘semantic’ difficulties, whilst difficulties with the appropriate use of language are referred to as ‘pragmatic’.
    Semantic Pragmatic Disorder is not an illness. It is a developmental disorder which prevents the person from using language in an appropriate manner and creates difficulty in understanding the meaning of the spoken word.
    In recent years, there has been debate about whether or not ‘Semantic Pragmatic Disorder’ is an accurate diagnosis or merely a descriptive term for the nature of communication difficulty found in verbal people with autism.
    Recent research findings suggest that the disorder does belong within the autistic spectrum, with a single underlying cognitive impairment producing both Semantic Pragmatic Disorder and autistic features.
