Saethre-Chotzen Syndrome or Acrocephalosyndactyly, Type 3
Features, which are variable, include: short and broad skull, facial asymmetry, widely spaced eyes with drooping eyelids, prominent nose and protruding jaw, soft tissue webbing of the areas of the fingers closest to the palm, usually only involving two or three digits. In some cases: small stature, hearing defects, squint, kidney defects, heart defects. Mild to moderate learning difficulty may occur but intellect is unimpaired in most cases. The syndactyly requires surgery only for cosmetic reasons. Autosomal dominant inheritance proposed. (See Craniosynostosis; Skull, Bones of the; Sutures of the Skull)

Pragmatics This section of the resource guide will address issues related to students that perform adequately on typical standardized tests of oral language (ex. CELF-3, The Word Test-R, PPVT-3, TOLD-P-3, etc.) yet have difficulty communicating effectively with others. The child’s grammar, vocabulary, articulation and fluency are judged to be adequate but the way they use language socially is odd. Because a standardized test that addresses conversational difficulties does not exist, Kyrene SLPs struggle to place these students in the speech-language program because they do not meet current eligibility guidelines. After a thorough review of the research in this area, the eligibility guidelines have been updated with additional information. (July 2001) Pragmatics has been defined as follows: "Pragmatics is the area of language function that embraces the use of language in social contexts (knowing what to say, how to say it, and when to say it- and how to "be" with other people). (Caroline Bowen, 2001) "Pragmatics involves the three following aspects of language use: 1. the study of discourse and conversational skills; 2. the study of the relationship between pragmatics and other levels of language; and 3. the study of the situational determinants of the use of language." (Neville, 1990).

Non-literal language comprehension, using a task of metaphor comprehension, was investigated in 18 children with specific language impairment (SLI), including a sub-group identified as having semantic-pragmatic disorder. The SLI children were compared to a normally-developing control group of children, matched on receptive language age. No difference in group mean scores was found between the SLI children and the control group , however differences in the developmental patterns were found. The sub-group identified as having semantic-pragmatic disorder performed as well as the other SLI children and the control children on the measure of non-literal language comprehension. The issue of whether formal and structured test situations reveal the language difficulties that children diagnosed as having semantic-pragmatic disorder show in conversational situations is discussed.

General Description
Language exists in the mind of each of us as an individual, yet, as is shown by the fact that we can understand each other, it is also socially constructed. We look at the surprisingly lengthy course of language acquisition in a normally developing child, and compare this with some of the ways in which language is affected by developmental problems and disorders. Language can provide insights into many aspects of the functioning of the brain, and allows us to pose a number of questions. Is language different in kind from other areas of cognition, or do general learning processes also apply to language? If language is most frequently represented in the left hemisphere of the brain, is there any effect on language when the right hemisphere is damaged? Are the brains of people who speak two or more languages organised differently from those of people who are monolingual? What is the nature of the interaction between language and memory? Finally, we ask whether language is unique to humans, or does it also appear in non-human species?

It is posted with permission, February 2002. Terms commonly used to diagnose a speech or language disorder Acquired Brain Injury (ABI): An accident or trauma that causes injury to the brain. Acquired Brain Injury can also be called a Traumatic Brain Injury (TBI) and it may be caused by penetrating injuries, such as in a car accident, or a Closed Head Injury (CBI) when the brain is damaged but there is no outward sign of trauma. The nature of the speech and language disorder depends on the type and extent of the injury.

Semantics is the aspect of language function that relates to understanding the meanings of words, phrases and sentences, and using words appropriately when we speak. Children with semantic difficulties have a very hard time understanding the meaning of words and sentences. This is sometimes apparent from their unusual responses when they are told to do something, and sometimes it is revealed by the questions they ask, and the things they say about words. There is an example here of 12 year old Nerida's interpretation of the word "acquire". In the example, she was unable to detect from the context that she was being asked what "acquire", rather than "a choir" meant. People with semantic processing difficulties have particular trouble with abstract words like 'curious' or 'vague', words that relate to feelings and emotions such as 'embarrassed' and 'anxious', and words that refer to status (for instance 'expert' or 'authority') or degree (for example, 'essential' or 'approximate'). They have difficulty with idioms, sayings and slang expressions, often taking them literally or interpreting them oddly. For example, when asked if he enjoyed spending time with his friends, a 14 year old with semantic processing problems replied, "I don't see how you can spend time, and I certainly don't see how you could enjoy it because spending time is not something you can do. You can only actually spend money".

There are a number of diseases which cause children to display some of the symptoms of autism. Heller's Disease
Normal development to age 3 or 4, then abrupt onset of fretfulness, negativism and anxiety. Regression of mental development and gradual loss of speech.
LKS "Landau-Kleffner Syndrome"
Also Acquired Childhood Epileptic Aphasia. Very rare disorder. Typical profile: normal development and age appropriate language first 3-7 years; Loss of receptive language while retaining some expressive language; "Telegraphic" speechfew verbs; Suspicion of deafness; Child frustrated, puzzled by change in himself; Autistic-like behaviors; normal or above
normal non-verbal IQ score; abnormal EEG, with or without seizures. Some practitioners suspect that some cases of Childhood Disintegrative Disorder or "late outset" autism may be Landau-Kleffner Syndrome.
Rett's Syndrome
a neurological disorder that occurs only in girls. Unlike autism, girls initially show normal development, then revert. The initial symptoms include some that are associated with autism.
