Lin Gorenkoff. Selectively mute children do not speak to people beyond the immediate family. This includes neighbours, relatives not seen regularly, teachers, classmates and the participants in any group activity, whether unstructured play groups or organized sports or hobbies. Within the family their conversation is typically age-appropriate, quite often to the extent that they are considered to be chatterboxes. The problem can go unnoticed in the pre-school years and, sometimes even during the first years in primary grades when teachers or facilitators assume the child is shy and that the parents are aware of the child's lack of verbal participation. It may become apparent when the child begins to attend nursery school or other form of structured play but may easily go unnoticed at that stage. For a child not exposed to organized preschool group activities, the first indications of the problem may not be noticed until kindergarten or beyond. Many children - especially when the problem is diagnosed early, and they are given the support and understanding that is needed - are able to speak comfortably in the classroom by the end of the school year. However, when the problem is especially persistent, dismissed or unnoticed in over-crowded classrooms and, consequently, not treated, it can continue into high school years.

TREATMENT OPTIONS SUGGESTIONS FOR TEACHERS ... HOME SELECTIVE MUTISM IS THE CHILD'S RESPONSE TO SOCIAL ANXIETY. The Diagnostic and Statistical Manual of Mental Disorders, fourth Edition (DSM-IV) defines selective mutism as:
  • Consistent failure to speak in specific social situations (in which there is an expectation for speaking, e.g., at school) despite speaking in other situations.
  • The disturbance interferes with educational or occupational achievement or with social communication.
  • The duration of the disturbance is at least 1 month (not limited to the first month of school).
  • The failure to speak is not due to a lack of knowledge of, or comfort with, the spoken language required in the social situation.
  • The disturbance is not better accounted for by a Communication Disorder (e.g., Stuttering) and does not occur exclusively during the course of a Pervasive Developmental Disorder, Schizophrenia, or other Psychotic Disorder.
Selective mutism was thought to affect less than one percent of all children until recent studies demonstrated that between one and two per cent of primary school childrem meet the above diagnostic criteria. Originally called

Among the vast range of anxiety induced disorders that exist, Selective Mutism may be the most disabling to its victims. It has been estimated that approximately one in a thousand children suffer from this presumed psychiatric ailment wherein the ability to speak is limited to the household or other areas of comfort. Public places and schools elicit so much anxiety within these children that their natural capacity to speak is suppressed. Once a child under five years of age exhibits the behavior described, for over a month, and without having other speech impeding barriers affecting them such as autism or a second language, he or she will most likely be diagnosed with selective mutism.
Doctors, for the most part, lean towards medication for those afflicted with selective mutism; however, other methods are practiced as well.

Developmentally, most teenagers are in middle adolescence about the age of 14, and this is when trying on new identities and behaviors really starts to show. What is not going on is selective mutism The other thing you can bet is that there is not some disease of his larynx (voice box), since he is able to generate a laugh when it suits him. All of this leads to adolescence. How do parents tell troublesome behaviors from just annoying behaviors? Look for red flags - falling grades, withdrawing from previously enjoyed activities (e.g., band, soccer, etc), lack of friendships, not adhering to previously agreed upon rules such as curfews. This syndrome differs from mutism. Children with mutism never speak. Cultural issues, such as recent immigration and bilingualism, should be noted. Fear of Talking (Selective Mutism)
What is selective mutism?
The most common feature of selective mutism is when your child does not speak in social situations, including school, even though he can speak and understand spoken language. It used to be called elective mutism.
What are the characteristics of selective mutism?
You may notice your child is overly shy when she enters preschool or kindergarten and seems unable to talk to others in the new class. You may have noticed that your child had a limited group of people she spoke to before starting school. Then, after your child is in school for several months, you find out that your child is not talking there. Some characteristics of selective mutism are:
  • Continues to not speak in social situations where talking is expected (such as school, Sunday school, play groups, around parents' friends), even though the child can speak in other situations (such as to family members).

    Anxiety Disorders in Children and Adolescents
    Selective Mutism

    Children who refuse to speak in situations where speech is expected or necessary, to the extent that their refusal interferes with school and making friends, may be suffering from Selective Mutism, thought to be a severe form of Social Anxiety Disorder. Onset of Selective Mutism is usually before five years of age, but it often comes to a head when the child enters school. The average age of diagnosis is between 4-8 years old, but these children probably exhibited "extreme shyness" at a much earlier age. For Selective Mutism to be diagnosed, the behavior must persist for at least one month. These children can be very talkative, even boisterous when at home, or in a place where they feel comfortable. Children suffering from Selective Mutism may:
    • Stand motionless and expressionless, turn his/her head, chew or twirl hair, avoid eye contact or withdraw into a corner.

    Definition Selective mutism is a condition occurring in childhood in which a child who is fluent with language frequently fails to speak in certain situations where language is expected. It typically occurs in school or social settings. This pattern of mutism must be observed for at least one month. The first month of school is not included as excessive shyness is commonly observed during this period. Parents often think that the child is refusing to speak, but usually the child is truly unable to speak in particular settings.

Selective mutism is an often misdiagnosed childhood anxiety disorder that, if left untreated, can lead to bigger behavioral problems down the road, KERO 23 reported. Gina Mallon first noticed the quiet change in her daughter Emily back in kindergarten. "She would get very upset if she thought someone heard her voice," Mallon said. Doctors told Mallon her daughter was shy, and she would outgrow it. But when Emily's grades dropped because she wouldn't speak in class, her parents knew better. Emily was finally diagnosed as selectively mute. She can't speak in school because anxiety overwhelms her. "Kids suffering from selective mutism will tell you that they're scared, they don't know why, their words are stuck, their body won't let them get the words out," psychologist Elisa Shipon-Blum said. Shipon-Blum said her daughter Sophia didn't utter a peep in school for three years.

    Selective Mutism:
    Although this is not classified as a child anxiety disorder, selective mutism is often seen around the time that children enter school for the first time, and can be accompanied by anxiety. The essential feature of selective mutism is the persistent failure to speak in specific social situations (e.g., school, with playmates) where speaking is expected, despite speaking in other situations. In order to be diagnosed with selective mutism, the person’s speech refusal must interfere with educational or occupational achievement or with social communication, and must be going on for at least one month. Selective mutism is not diagnosed if the individual’s failure to speak is due solely to lack of knowledge of the language. Primary Symptoms:
    Primary symptoms of selective mutism include persistent failure to speak in specific social situations where there is an expectation for speech. The child should be able to speak in other situations. This disorder is usually first seen in childhood, and is treatable with cognitive-behavioral techniques. Treatment:
