Since her student days at the Philadelphia College of Osteopathic Medicine, Elisa B. Shipon-Blum, DO, has had an intense interest in anxiety disorders.
When she entered private practice as a family physician in 1991, she concentrated her practice on pediatrics and childhood anxiety disorders. Early on Dr Shipon-Blum saw children with a disorder known as selective mutism, previously known as elective mutism. This condition aroused her curiosity and suddenly demanded her attention.
"These children were scared. Their parents were confused. Their teachers were concerned. Everybody wanted answers," recalls Dr Shipon-Blum. "Until recently, the disorder was misdiagnosed as autism, as defiance, extreme shyness or even as a mental disorder requiring special education.
"This complete and total misunderstanding of selective mutism hindered the development of a treatment program for patients with the disorder. Most cases are due to severe social phobia, and most of these children have a several strong hereditary predisposition to anxiety and depressive disorders."

Selective Mutism
Selective mutism, formally known as elective mutism, is a disorder of childhood that is characterized by the peristent lack of speech in at least one social situation, despite the ability to speak in other situations.. Onset of selective mutism typically occurs before a child is 5-years-old. However, it is usually first noticed when the child enters school. Specific features of this disorder are described in the 2000 Diagnostic and Statistical Manual of Mental Disorders (DSM-IV-TR) (pp.125-127) as follows:

Selective Mutism
An Integrated Treatment Approach
by Robert L. Schum Selective mutism is a uniquely challenging childhood communication disorder that demands a coordinated treatment approach. Speech-language pathologists are in an excellent position to coordinate intervention among family, classroom teachers, and other clinicians. The Problem Selective mutism is defined as a failure to speak in specific s ocial situations despite speaking in other situations, and it is typically a symptom of an underlying anxiety disorder. Children with selective mutism can speak normally in certain settings, such as within their home or when they are alone with their parents. However, they fail to speak in other social settings, such as at school or at other places outside their home. Other symptoms associated with selective mutism can include excessive shyness, withdrawal, dependency upon parents, and oppositional behavior. Most cases of selective mutism are not the result of a single traumatic event, but rather are the manifestation of a chronic pattern of anxiety. Mutism is not passive-aggressive behavior. Mute children report that they want to speak in social settings, but are afraid to do so.

Selective Mutism Selective mutism is characterized by a consistent failure to speak in specific social situations in which there is an expectation for speaking. Children with selective mutism have the ability to both speak and understand language, but fail to use this ability as a result of what we would consider to be social anxiety in adults. Most children with selective mutism function normally in other areas of their lives. Selective mutism is not a communications disorder and is not part of a developmental disorder. By definition, selective mutism does not include children with conduct disorders, oppositional/defiant behavior, and/or attention-deficit hyperactivity disorder. T he principal problem in children with selective mutism appears to be anxiety . This anxiety (which causes avoidance) seems closest to the definition of social anxiety disorder (social phobia). Indeed, most adults with social anxiety disorder relate strongly and can fully understand selective mutism, even though they themselves were not necessarily children with selective mutism. It also appears that cognitive-behavioral therapy, with the emphasis being on the behavioral component (depending on the age of the child) is the therapy of choice.

References and related books Introduction Those that have worked with selectively mute children have encountered wide variations in their social actions. Some children enjoy contact with others and will play easily, but remain silent. Some have a close friend who often speaks for them by interpreting gestures. Others find all aspects of social situations uncomfortable and do not participate at all. Whatever form the condition takes, it can persist. There are children in the 2nd, 3rd, and 4th grades who have never spoken in school. There are students in high school who have not uttered any or no more than a few words in a school setting.
