Alternate Names : Dandruff, Cradle Cap Attribution Seborrheric dermatitis is a chronic, inflamed redness and scaling of the skin. It can involve the scalp and face. When this condition occurs in infants, it is sometimes called cradle cap. In adults, seborrheic dermatitis of the scalp is called dandruff. What is going on in the body? The cause of seborrheic dermatitis is not known. The skin of the scalp, and sometimes the face, becomes inflammed and flakes off. What are the causes and risks of the condition? The cause of seborrheic dermatitis is unknown. It tends to run in families. Some individuals have an overgrowth of yeast organisms that is linked with this condition. Those with neurologic disorders, immunodeficiency disorders such as HIV , and extended hospital stays can have severe cases of seborrheic dermatitis.

Seborrheic Dermatitis and Dandruff
(Sept 1999) Seborrheic dermatitis typically arises after puberty and tends to become milder during later life. A variant of seborrheic dermatitis called cradle cap occurs in newborns and usually goes away without treatment during the first year of life. Safe and effective medicines Dandruff is the term used to describe an exceptionally common scalp condition characterized by flaking and scaling. Intermittent episodes of fine, dry, white flaking and mild itching of the scalp are the hallmark of dandruff. Contrary to popular belief, regular washing of the scalp and hair actually results in improvement of the dryness and scaling ( dandruff products ). Dandruff can be thought of as either a mild variant of seborrheic dermatitis or a separate condition that is capable of worsening into true seborrheic dermatitis. The key distinction between dandruff and seborrheic dermatitis is that there is no visible inflammation associated with dandruff. Seborrhea is a term used to describe skin that is excessively oily. It is only when irritation, redness and scaling develop on oily skin that seborrheic dermatitis is present.

Itching: Causes and Treatment
Itching is a sensation common to a number of skin disorders, including psoriasis. It is a common sensation not specifically associated with any particular skin disorder, and may sometimes seem entirely random - for example, the spontaneous “itchy spot” on the nose, cheek, etc. It is a frequent manifestation of systemic diseases such as chronic renal disease, biliary obstruction, cirrhosis, some types of thyroid disease, Hodgkin’s Lymphoma, and some cancers. Itching is so common one would think it is well understood. To the contrary, itching is not well understood at all. There isn’t even a good definition of itching. The working definition of itching from a standard dermatology textbook is, “a sensation that causes a desire to scratch.” The medical term for itching is pruritus. By using medical terminology we can get a step away from the popular conception of itching as a minor or even funny phenomenon. When pruritus is severe, it is anything but humorous. As anyone with severe pruritus can attest, the sensation closest to severe pruritus is severe pain. Pain and pruritus do share some neurophysiologic pathways. Pruritus seems to occur in two phases:

Diagnosis/Definition
  • Chronic superficial inflammatory disease of the skin. Predilection for the scalp, eyebrows, nasolabial creases, ears, sternal area, groin and gluteal crease. Characterized by scant, loose, dry, moist or greasy scales and by crusted, pinkish or yellowish patches of various shapes and sizes; by remissions and exacerbations; and more or less by itching. On the scalp manifests as dry powdery dandruff or an oily type with erythema and thick crusts. Frequently spreads beyond the hairy scalp. On and in the ears, seborrheic dermatitis is frequently mistaken for otitis externa. There is scaling in the aural canals, around the auditory meatus and in the postauricular region or under the lobe. In these areas the skin often becomes red, fissured and swollen.
Initial Diagnosis and Management
  • Evaluation: A focused history and physical examination. Areas to be examined should include the scalp, ears and mid-face.

About Dandruff and Scaling Scalp Conditions What is dandruff?

Dandruff, the most common scaling scalp condition, is simply a build up of dead skin cells on the scalp. Skin cells are being shed from our scalps all the time, but are usually shed as very fine particles which we don't even notice. Dandruff sufferers produce skin cells on the scalp more quickly than normal, and because more of them are being produced, they clump together to form visible white flakes. Dandruff is also linked with the presence of higher than normal levels of a natural yeast called 'malassezia furfur' . We all have small quantities of this on our scalp with no ill effect, but in dandruff sufferers the yeast thrives on the clumped skin cells, making the condition even worse.
What are the symptoms of dandruff?
The excess skin cells fall off the scalp into the hair as white flakes. They may then fall out of the hair on to the shoulders. For many, one of the worst symptoms of dandruff is intense itching. This is thought to be caused by increased levels of malassezia furfur which, in turn, leads to inflammation and itch on the scalp.

What is seborrhoeic dermatitis

What does seborrhoeic dermatitis look like?
Within the scalp, seborrhoeic dermatitis causes ill-defined dry pink or skin coloured patches with yellowish or white bran-like scale. It may spread to affect the entire scalp. Seborrhoeic dermatitis is common within the eyebrows, on the edges of the eyelids (blepharitis), inside and behind the ears and in the creases beside the nose. It can result in pale pink round or ring shaped patches on the hairline. Sometimes it affects the skin-folds of the armpits and groin, the middle of the chest or upper back. It causes salmon-pink flat patches with a loose bran-like scale, sometimes in a ring shape (annular). It may or may not be itchy and can be quite variable from day to day. Seborrhoeic dermatitis
