Cutaneous Indications for HIV Testing Seborrheic Dermatitis Seborrheic dermatitis is a common benign dermatosis, present in 3 to 5% of young adults. The clinical presentation is a pink or red inflammatory eruption involving the scalp, eyebrows, nasolabial folds, post-auricular areas, external auditory meatus, and beard areas. In more severe cases, the chest and intertriginous area may be involved. The involved skin may be edematous with a yellowish scale. The patient may be asymptomatic or may complain of severe pruritus associated with this condition. The etiology of seborrheic dermatitis is unclear. What is known is that there is often a genetic predisposition to this condition. Pityrosporum (yeast species) may play a role in the pathogenesis of this condition, as may increased sebum production. There exists an almost paradoxical tendency for autoimmune or inflammatory conditions to arise in the profound immunodeficient state of HIV disease. For example, the onset of more severe or diffuse seborrheic dermatitis in a patient may herald progression of HIV disease. This presentation occurs typically at a CD4+ count of 500 or less. Patients who have had a history of mild seborrheic dermatitis ("dandruff") may experience a sudden worsening of the condition. Seborrheic dermatitis is more prevalent in the setting of HIV disease and is often more severe in its presentation than in a healthy population. Histopathologic differences may also be observed.

Last update: September 9, 2005 Conditions We Treat Seborrheic dermatitis What is Seborrheic Dermatitis? Seborrheic dermatitis is a noncontagious inflammation of the upper layers of the skin. Most common in infants younger than three months and among adults ages 30 and older, seborrheic dermatitis is slightly more common in men than in women. Symptoms include red, itchy, inflamed skin covered by scales that may appear white, yellowish or gray. When skin starts reproducing itself faster than normal, the extra skin cells build up and flakes (dandruff) appear. This condition often appears on the scalp, near the nose and eyebrows, and around the ears, breastbone, groin and armpits.

Abstract
Seborrheic dermatitis is a chronic inflammatory disease that mainly affects seborrheic areas of skin. An inflammatory response to the yeast Pityrosporum ovale has been thought to be important in the etiology of the condition. Therefore, topical antifungals and corticosteroids have been the mainstay of treatment. The recent development of topical macrolactam immunomodulators has offered a useful, safe alternative to corticosteroids in the treatment of various inflammatory skin disorders. We report successful treatment of seborrheic dermatitis with pimecrolimus.
Case report
Figure 1 Figure 2 Scattered scaly patches on forehead, nasal bridge, and scalp line (Fig. 1). One month followup showing complete clearance (Fig. 2). An 18-year-old woman with no significant past medical history presented with a 1-year history of asymptomatic lesions on her face. On physical examination, there were scattered, scaly patches on her forehead, nasal bridge, and scalp line (Fig. 1). She had been treated with ketoconazole cream, ketoconazole 2% shampoo, and hydrocortisone 1 % cream without improvement. A skin biopsy confirmed the diagnosis of seborrheic dermatitis. The patient was prescribed pimecrolimus cream twice daily. On followup at 1 month the eruption had completely cleared (Fig. 2).

Seborrheic Dermatitis
In seborrheic dermatitis, the skin is usually itchy, red, and flaky. It often affects the scalp (causing dandruff) and the central part of the face or chest. Seborrheic dermatitis can also affect skin on other parts of the body. Dandruff is common among adults of all ages. Seborrheic dermatitis of the face and chest becomes more common as people age.
Causes and Symptoms
The cause is unknown, but seborrheic dermatitis may be a reaction to an otherwise harmless fungus. Seborrheic dermatitis is more common among people who have certain brain or nerve disorders (such as Parkinson's disease), those whose immune system is weakened (for example, by some cancers or by use of corticosteroids for a long time), and those who take certain drugs (such as chlorpromazine or haloperidol). In older people, seborrheic dermatitis most commonly affects the scalp, eyebrows, eyelids, the skin between the nose and upper lip, the part of the face where the beard grows, and the area just behind the ears. The disorder can also affect the middle of the chest and the area between the shoulder blades.

Dermatitis is a broad term covering many different disorders that all result in a red, itchy rash. The term eczema is sometimes used for dermatitis. Some types of dermatitis affect only specific parts of the body, whereas others can occur anywhere. Some types of dermatitis have a known cause; others do not. However, dermatitis is always the skin's way of reacting to severe dryness, scratching, a substance that is causing irritation, or an allergen. Typically, that substance comes in direct contact with the skin, but sometimes the substance is swallowed. In all cases, continuous scratching and rubbing may eventually lead to thickening and hardening of the skin.

Seborrheic dermatitis appears as red, inflamed skin covered by greasy or dry scales that may be white, yellowish, or gray. It can effect the scalp, eyebrows, forehead, face, folds around the nose and ears, the chest, armpits (axilla), and groin. Dandruff and cradle cap are mild forms of seborrheic dermatitis, they appear as fine white scales without inflammation.
The cause of seborrheic dermatitis is unclear, though it is has been linked to genetic or environmental factors. Pityrosporum ovale , a species of yeast normally found in hair follicles, has been proposed as one possible causative factor. A high fat diet and alcohol ingestion are thought to play some role. Seborrheic Dermatitis (Severe Dandruff) I. Definition: Seborrheic dermatitis is a chronic, recurring scaly rash involving the face, ears, eyebrows, scalp, and sometimes the chest. It occurs more frequently as people get older. There is no cure for this condition. II. Causes:
  • A yeast named Malassezia ovalis that grows on the skin causes seborrheic dermatitis. People prone to seborrheic dermatitis are usually genetically susceptible to this condition. Seborrheic dermatitis is usually worse in the colder months.
III. Treatment:
  • Treatment is a matter of control rather than cure. Seborrheic dermatitis usually responds to topical therapy. The treatment depends on its site as well as its severity. Sebulex Shampoo , which can be purchased without a prescription, should be used every other day to wash the scalp. The more often you shampoo the less likely you are to have this problem. Other good shampoos are Head and Shoulders and Selsun Blue Shampoos that contain conditioners can make one more prone to this condition. If you use a shampoo with conditioner alternate it with one that has no conditioner. Hair pomade can contribute to this problem. The use of Sebizon Lotion with your favorite pomade is an effective treatment for this problem.
