Scoliosis
Scoliosis (sko-lee-O-sis) is a side-to-side curvature of the spine. One type of scoliosis, called idiopathic scoliosis , is common in the general population. (The word "idiopathic" is used to describe a disease having no known cause.) Idiopathic scoliosis affects two to three percent of children, usually just before or during adolescence. An unusual form of scoliosis, dysplastic scoliosis , is seen in NF1 but rarely in the general population. Dysplastic scoliosis is characterized by a sharp curve over just a few vertebrae.
  • When does scoliosis show up? Scoliosis typically appears during early childhood.
    How common is scoliosis among people with NF1? Scoliosis occurs in about 15 percent of children with NF1.
    How is it treated? In less severe cases, a brace can be worn to prevent further curvature of the spine. More severe cases require surgery to correct the curvature.

What Is Scoliosis?
Scoliosis is a medical term taken from a Greek word meaning curvature. This disease often develops during childhood causing the spine to curve laterally (to the side) to the left or right. The spine's normal curves occur at the cervical, thoracic, and lumbar regions. These natural curves position the head over the pelvis and work as shock absorbers to distribute mechanical stress during movement. The normal spine viewed posteriorly (from behind) appears straight from neck to buttocks. However, a scoliotic spine bends to the left or right resembling the letter S or C. Scoliosis is a complex three-dimensional disease. To understand this concept consider that in some cases, as the spine curves abnormally, the involved vertebrae are forced to rotate. If rotation occurs at the thoracic level of the spine, vertebral turning impacts the rib cage and may result in rib prominence on the opposite side of the curve. In severe cases, lung and heart function can be compromised. Fortunately, severe cases of scoliosis (above example) are not as prevalent as smaller curves.

What Is It?
Scoliosis is a medical term taken from a Greek word meaning curvature. This disease is known to develop in adults over the age of 18 causing the spine curve laterally (to the side) to the left or right. Adult scoliosis is caused by: 1. Progression of the disease from childhood. This usually occurs when scoliosis is not treated early or went unnoticed during childhood. The scoliotic curves may be thoracic, lumbar, or both. 2. The asymmetric degeneration of spinal elements. This may be caused by osteoporosis (porous bone), disc degeneration, compression fracture, or a combination. These conditions usually affect the lumbar spine and can affect vertebral height, shape, or basic structural integrity. 3. Combination of numbers 1 and 2.

What is Adult Scoliosis?
The normal, healthy spine is straight when seen from the front or the back (frontal plane). When seen from the side, the healthy spine has several mild curvatures. By obtaining an X-ray of a standing person, the exact contour of the spine can be measured and then compared to normal values to determine if a significant spinal deformity is present. Scoliosis is defined as a lateral deviation of the spine. More precisely, if the spine, seen on an antero-posterior radiograph (abbreviated as "AP", ie. seen from the front), has a curvature that measures more than 10 degrees then a scoliosis is present. (figure 1)
When a curvature, or scoliosis, is mild then one may not be able to detect any abnormality without an X-ray. On the other hand, moderate or severe scoliosis can be noted due to an asymmetry of the back. A tilted waistline and skin folds may be present, as well as a hunching, or protrusion of the back. Frequently, such an asymmetry in the back is more noticeable with leaning forward.
There are many types of scoliosis, and people of all ages can be affected by scoliosis. In adults, scoliosis can result from several conditions, but most types fall into two different categories. In the first category is a person who had a scoliosis as a child/adolescent and the abnormal spine curve has increased into adult life or is becoming painful with aging. The second category of adult scoliosis is seen in patients who have never had a scoliosis as a child but begin to develop an abnormal curvature with aging. The spinal deformity in degenerative scoliosis is usually a mild side curvature involving predominantly the lower levels of the spine.

There are several types of scoliosis. Fortunately, most are rare. The common type is "idiopathic scoliosis," so called because the cause is unknown. It affects about 4% of the population, but is more common among females. Idiopathic scoliosis usually starts between the ages of 10 and 12. As the child grows, there is a chance the curve can progress (worsen). Most curves will not worsen during adolescence and worsening after growth is complete is unusual. Other types of scoliosis include congenital (caused by an abnormally shaped bone that is present at birth); neuromuscular (due to a neuromuscular disease such as cerebral palsy, muscular dystrophy, etc.); acquired (following a fracture, radiation therapy for cancer, etc.); and juvenile. Juvenile scoliosis is similar to adolescent scoliosis, but it is much more likely to progress. Before considering treatments for scoliosis, it is important to know the natural history of the disease (what happens if left untreated). Curves that measure 10 degrees or less are considered "normal" and do not interfere with strength, joint mobility, endurance, or any other body function. They are not true scoliosis, almost never progress, and do not increase the likelihood of developing back pain, arthritis, disc herniation, or any other musculoskeletal problem. Treatment of scoliotic curves of 10 degrees or less is therefore unnecessary. Sometimes these minimally curved spines even get straighter on their own.

In June of 2002, my mom noticed something was not quite right with my back. One shoulder blade seemed to stick out more than the other. So, being a mom, she made an appointment with the doctor.
The doctor looked me over, made me do the Adams forward bend test , and promptly sent me off to see an Orthopedist that specialized in Scoliosis.
So, the next week I was off to see the next doctor, where I got X-Rays , and the confirmation ... I had Idiopathic Juvenile Scoliosis

What is scoliosis?
Scoliosis means that the spine curves from side to side rather than being straight down the back. The spine is made of bones called vertebrae that normally stack one on top of the other in a straight line. The bones in the upper back are called thoracic vertebrae. This is the most common site for scoliosis. The bones in the lower back are called lumbar vertebrae. Scoliosis occurs less often in the lumbar vertebrae. Scoliosis develops gradually. Scoliosis is usually noticed just before or during puberty when a child goes through a growth spurt. Females get scoliosis more often than males. Often parents do not notice the gradual changes caused by scoliosis. The curvature is usually discovered by a health care provider. Occasionally, scoliosis is diagnosed during infancy and is treated sooner.

Everyone's spine has natural curves. These curves round our shoulders and make our lower back curve slightly inward. But some people have spines that also curve from side to side. Unlike poor posture, these curves can't be corrected simply by learning to stand up straight.
This condition of side-to-side spinal curves is called scoliosis. On an x-ray, the spine of a person with scoliosis looks more like an "S" or a "C" than a straight line. Some of the bones in a scoliotic spine also may have rotated slightly, making the person's waist or shoulders appear uneven.
  • One in 10 people will have scoliosis, but only two to three in 1,000 will need treatment.
  • One of the most common signs of scoliosis is a prominent shoulder blade, frequently the right one. One shoulder might also be higher, and the child seems to tilt to one side. One hip might seem higher than another.

    What is scoliosis?
    Scoliosis is a complex twisting of the spine that affects the alignment of the bones from nearly every angle. While one in ten persons will have some curvature, only two or three out of a thousand persons need treatment. Some cases of scoliosis are due to an underlying muscle or nerve problem, or a defect in bone formation. Most scoliosis, however, is called "idiopathic" meaning that the cause is not known. It is known, however, that idiopathic scoliosis:
  • usually develops in young people aged ten to twelve years old;
  • has a tendency to run in families;
  • can usually be prevented from worsening in a growing adolescent by wearing a brace.
    What is a scoliosis brace?
    The brace (orthosis) is a device to hold the spine in a straighter position during the growing years. The brace is prescribed by an orthopaedic specialist and is usually worn nearly full-time until bone growth has stopped. The Boston-style brace ( photo ) is a plastic body jacket that fits snugly around the body to exert pressure on the ribs and back, pushing the spine into a straighter position.     Scoliosis curves greater than 10° affect 2-3% of the population, often children between the ages of seven and ten, with a greater incidence among girls. Curves greater that 20° affect about 1 in 2500 people. Right-sided curves are more common than left-sided curves. Different etiologic theories point to genetic , neuromuscular, hormonal and growth factors, though it appears that the cause is multifactorial. Among Genetic Halflings Scoliosis is very frequent. The vast majority of cases (85%) have unknown causes or are idiopathic In one out of every 1000 cases, surgery may be necessary. Females are nine times more likely to require treatment than males, mostly since they are also more likely to have larger, progressive curves.
