November 2003
I have checked the archives, and all I found on sciatica during pregnancy was recommendations for people to treat it in some way. I am looking for advice on what I, myself, can do to minimize the pain. I do not have the shooting-pain-down-one's- leg type of sciatica that I have heard others describe, but rather a near-constant ache in my right buttock. In the last few days it's become much more painful in the evenings, so intense that I sometimes can't move between standing and sitting. So... what can I do? Are there any stretches or exercises that would help? Do certain positions of sitting or sleeping make it any better or worse? Any advice appreciated! Sarah I had a bad case of sciatica when I was pregnant. Try elevating your feet when you are sitting. Get up and walk around every half hour. I think the key is to keep moving. I know it seems very painful to do that, but it does get easier. See if a warm/hot pad on the painful area helps. I thought it was helpful at night since you are not able to move much in bed. Feel better! Judy I've had pain in my upper right thigh since I hit 4.5 months. At first, everyone said it was sciaticaincluding my doctors and physical therapists. However, it felt very different from the descriptions of sciatica that I had readit's not shooting and it's more like constant numbness, tingling, and burning pain at the same time. Standing for more than 10 minutes is excruciating, walking is hard after a couple blocks, sitting can also be incredibly painful. Lying down even is uncomfortable at times.

Diagnosis
Sciatica What is sciatica? Sciatica is pain resulting from irritation of the sciatic nerve.Sciatica pain is typically felt from the low back to behind the thigh and radiating down below the knee. The sciatic nerve is the largest nerve in thebody and begins from nerve roots in the lumbar spinal cord in the low back and extendsthrough the buttock area to send nerve endings down the lower limb. What are causes of sciatica? While sciatica ismost commonly a result of a disc herniation directly pressing on the nerve, any cause ofirritation or inflammation of this nerve can reproduce the symptoms of sciatica. These cause include irritation of the nerve from adjacent bone, tumors, muscle, internal bleeding, infections, injury, and other causes. How is sciatica treated?

New Way To Diagnose Sciatica May Point To A Different Cause
LOS ANGELES (February 1, 2005) – For the last 70 years, a damaged disc in the lower back has been widely accepted as the most common cause of sciatica – a condition where the sciatic nerve is pinched, causing pain to radiate down the leg. As a result, treatment for sciatica is based on diagnosis of a damaged disc, despite the fact that nerves cannot be viewed with routine imaging tests. Consequently, over one million patients each year undergo magnetic resonance imaging scans (MRI) for sciatica and many are told there is no obvious cause for their pain. Back Pain and Sciatica
WHAT ARE LOW BACK PAIN AND SCIATICA?
Most people will have at least one backache during their lives, and many will live with recurrent or prolonged back problems. While discomfort can affect any area of the back, pain most often afflicts the lower part, which supports most of the body’s weight. Indeed, low back pain is among the most common reasons why Americans visit the doctor, and it is a leading cause of work-related disability. In many backaches that cause significant disability, the pain or numbness radiates down the leg or into the foot, a condition known as sciatica.
The Spine
The back is highly complex, and pain may result from damage or injury to any of various bones, nerves, muscles, ligaments, and other structures. Still, despite sophisticated techniques that provide detailed anatomical images of the spine and other tissues, the cause of most cases of back pain remain elusive. Vertebrae.

Sciatica
Sciatica is irritation of the sciatic nerve that passes down the back of each thigh. The sciatic nerve arises from the lower spine on either side and travels deep in the pelvis to the lower buttocks. From there it passes along the back of each upper leg and divides at the knee into branches that go to the feet. Sciatica typically causes pain that shoots down the back of one thigh or buttock. • Herniated disc or slipped disc the cushions between the bones of your spine bulge out of place
  • Disc degeneration, caused by againg or arthritis
  • Spinal stenosis narrowing of the spinal canal in the lumbar area
  • Spondylolisthesis - slippage of a bone in the low back
  • Sprain or strain of muscles or ligaments in the area
In rare cases:
  • Benign or malignant tumors
  • Infections
  • Blood clots
  • Metabolic problems like diabetes
  • Toxins such as excessive alcohol
Risk Factors Age: 25-45

Sciatica
The term sciatica is often used to describe pain in the leg from a back problem. The name comes from the greek word ischion which means hip joint, after the area which the pain is often felt or was presumed to come from. The term is often used inaccurately to decribe pain in the leg for any reason, however sciatica is very specific and usually caused by a specific and identifiable problem. Most leg pain is not sciatica.
What is sciatica?
True sciatica is a form of nerve root irritation, also called radicular (root) pain/irritation. This is a better term to use because nerve root irritations can occur in other parts of the body and these cannot be called sciatica. Sciatic pain is found in two main age groups, the younger adult and the older adult. The causes of the problem are different in the two groups. Sciatica is severe leg pain which goes down the leg in a particular area, usually a clearly defined narrow strip of skin. There is no vagueness about the feeling. It is often sharp, shooting or surging in nature and worse below the knee and in the foot. It can be disabling and severe enough to badly disturb sleep. The sufferer may be forced to lie down as the pain is too severe to stand or sit.

The largest nerve in your body has a very devious twistand when you have a pain in that nerve, it can really get around. Sciatica, pain in the sciatic nerve, can radiate from the buttocks down the back of the leg to the knee, even as far as the big toe. "People with sciatica often say their back pain is bad but their leg pain is worse ," says Loren M. Fishman, M.D., a physiatrist and rehabilitation medicine specialist at Flushing Hospital Medical Center in New York City. Often the hip pain is far more severe on one side than the other. When you've got pain like that, you'll need a hands-on diagnosis before anything else, Dr. Fishman says. Once the doctor has ruled out a disk problem or fracture, he may be able to find out whether tight buttocks muscles are causing your pain by compressing the sciatic nerve.

Sciatica is a pain in the leg due to irritation of the sciatic nerve . The pain generally goes from the front of the thigh to the back of the calf, and may also extend upward to the hip and down to the foot . In addition to pain, there may be numbness and difficulty moving or controlling the leg.
    Causes of sciatica
The most common cause of sciatica is a " slipped disc " (a herniated disc in the spine ). Other conditions that can cause or aggravate sciatica include roughening and enlarging of the vertebrae , tension in the piriformis muscle infections , and tumors Sciatica may also be experienced in late pregnancy as the result of the uterus pressing on the sciatic nerve
    Treatment
Most cases of sciatica can be effectively treated by physical therapy and appropriate changes in environment (for example cushioning, or chair and desk height). Chiropractic manipulation often helps. In approximately 10-20% of cases, surgery is required to correct the problem.

    118. Sciatica
sciatica. Definition. The long and very large nerve named sciatic which suppliesthe muscles of the Phase A Used in acute and disabling sciatica.

SCIATICA Definition The long and very large nerve named sciatic which supplies the muscles of the thigh, leg, and foot, and the skin of the leg, running the entire length of the leg becomes inflamed to cause sciatica. The pain is felt in the thigh and other areas associated with the sciatic nerve, buttocks, knee, leg, and even foot in severe cases. It may be caused by a slipped disk, or an inflammation in or around the vertebral bones. An abnormal condition in nearby blood vessels may press on the nerve. Hardening of the arteries can cause reduction of blood supply and pain in the nerve. Poor muscle tone, or contracted ligaments, may also cause a problem with sciatica. Prevention 1. Learn to hip bend not waist bend. 2. Learn not to sit on one foot. 3. Avoid a one-sided twist to do any commonly done task, such as reaching for toilet tissue; getting out of bed; or reaching for the telephone. Change your furniture around to avoid a constantly repeated twist to the same side. 4. Avoid sustained flexion of the back as stooping to cook, write, or drive a car. Avoid long sitting. Do not put your hamstrings (large muscles at the back of the thighs) on a prolonged stretch, as in sitting while the knees are straight in front of you such as in driving a car for hours. 5. Get out of bed correctly. A side bend in getting out of bed may eventually lead to pain on the opposite side in sciatica.
