How to Avoid Sciatica
Sciatica is severe pain in the leg caused by the compression or irritation of the sciatic nerve. Causes can be a herniated disk, poor posture, back strain, obesity, fractures, tumors and degenerative spine disease. The largest nerve of the body, the sciatic nerve runs from the end of the spinal cord to the feet. Steps: Maintain a healthy body weight, especially as you age. Lugging around extra pounds can speed up the degeneration of your bones and joints. Eat a healthy diet rich in green leafy vegetables and fiber. Cut out saturated fats. Reduce your daily stress and learn to relax. Stress and tension keep the body and nerves on overload. Walk or swim for 30 minutes a day, four times a week. Exercise will strengthen your trunk and back muscles, which will decrease the possibility of sciatic pain. Avoid exercises that strain the lower back, especially straight-legged sit-ups and weight lifting without proper lower-back support.

The cause of the nerve dysfunction should be identified and treated as appropriate. In some cases, no treatment is required and recovery is spontaneous. Conservative treatment is usually appropriates if there was sudden onset, minimal sensation changes, no difficulty in movement, no history of trauma to the area, and no evidence on testing of degeneration of the nerve axon.

Verified by National Institutes of Health Clinical Center (CC) February 11, 2005 Sponsored by: National Institute of Dental and Craniofacial Research (NIDCR) Information provided by: National Institutes of Health Clinical Center (CC) ClinicalTrials.gov Identifier: Purpose This study will test the effectiveness of two drugs-nortriptyline and MS Contin (a type of morphine)-to treat pain caused by lumbar radiculopathy, or sciatica. Sciatica results from damage to the lumbar nerve roots, typically causing back pain and sharp, shooting pain down one or both legs. Although sciatica is common, there are no good treatments for it. Tricyclic antidepressants, such as nortriptyline, and opioids, such as morphine, have been effective in treating other kinds of pain from nerve damage. Patients between 18 and 65 years of age who have had sciatica pain daily for at least 3 months may be eligible for this study. Participants will provide a medical history and occupational and other social information. They will undergo a neurological examination, routine blood tests and an electrocardiogram and will fill out three questionnaires providing information on daily functioning and psychological well-being.

The sciatic nerve is formed by nerves from the spine at the junction of the lumbar and sacral regions. The nerve passes from the spine into the pelvis then into the buttock, posterior (back of) thigh and down below the knee. Below the knee the nerve divides into two branches, one going through the posterior (back) calf down the inner side of the ankle, and a second division goes through the outer side of the calf around the top of the ankle and foot to the toes. The important features of sciatic pain are that it goes below the knee, and it is associated with neurological symptoms. The neurological symptoms include a sharp or burning character to the pain; changes in sensation such as tingling, pins and needles or numbness; and association with weakness.

AN ALL-TOO-COMMON HEALTH COMPLAINT The sciatic nerves are the largest and longest nerves of the body, reaching about the size of your thumb in diameter and running down the back of each leg. When these nerves are irritated or affected by the inflammation of nearby soft tissues, doctors refer to this as sciatica. One of the most common causes of sciatic leg pain is the Vertebral Subluxation Complex. It can be accompanied by the bulging or herniation of the soft pulpy discs which separate each spinal bone. This can irritate or put pressure on the sciatic nerve roots as they leave the spinal cord. The result can be an intense pain shooting down either or both legs. In the past, treatment has involved pain medications, muscle relaxers, physical therapy, and even surgery. The chiropractic approach is to use carefully directed and controlled pressure to remove the interference from spinal structures. These chiropractic "adjustments" can be quite effective in reducing nerve irritation and its associated pain. Sciatica, like other health problems that can be traced to the spine, often responds dramatically to the restoration of normal spinal function through chiropractic care.

When you have a pain that goes down the back of your leg or the front of your lower leg, your doctor often diagnoses sciatica. He looks for a pinch of the huge sciatic nerve at the back or by a muscle in your buttocks, called the piriformis. However, most of the time, the MRI test does not show a pinch of the nerve in your back and it doesn't hurt when you touch the piriformis muscle, at a spot two inches from the center of your backside at the height of your hip bone. But your doctor calls it sciatica, tells you that you have pinched nerve in the back and sends you for physical therapy. Researchers in Birmingham England found bacteria called Propionibacterium acnes in 31 percent of people with sciatica. Fifty-three percent of discs removed at surgery contained this bacteria also. If infection really causes sciatica, the treatment would be to take Zithromax, Biaxin or Dynabec antibiotics for many months. We will see if other studies confirm these findings.
