Technically speaking, sciatica is a symptom not a diagnosis It is a non-specific term commonly used to describe symptoms of pain radiating downward from the buttock over the posterior or lateral side of the lower limb. It is usually assumed to be caused by compression of a nerve but this is not necessarily so. A common neurolgical cause of this pain is entrapment of the sciatic and/or posterior femoral cutaneous nerves. But the pain may be caused by trigger points in the soft tissue. This latter case is easily overlooked and requires manual palpation of the musculature associated with the hip if the pain is not to be misdiagnosed. Background It is often assumed that there is sciatic nerve root entrapment, resulting in the compression of the nerve. Pain and symptoms being transmitted or referred from the low back to one of the buttocks and down the back of the leg along the pathway of the sciatic nerve. Hence the term sciatica.

The sciatic nerve is a large nerve which comes from your lower back and travels down the back of your legs and then branches out to your feet. It allows you to feel sensations and move muscles in your legs. Sciatica is where inflammation or pressure from the back causes the sciatic nerve to become painful. Sometimes, the function of the nerve can become impaired resulting in weakness or pins and needles. Sciatica can be present with or without backache and can send pain down the back of your leg. Why might it happen in pregnancy?
Contrary to popular belief, there is no evidence to suggest that sciatica is caused by anything specifically related to pregnancy. Pregnancy related changes can cause pelvic pain and back ache, but not sciatica. Sciatica is not caused by the baby pressing on a nerve. In younger people, it is often caused by damage to a disc leading to inflammation around the nerve or direct pressure from a prolapsed or 'slipped' disc. In older people, it may be due to compression from a ligament or bone. Neither of these situations occur more frequently in pregnancy and the chances are that if you do have sciatica during pregnancy, it would have come on whether you were pregnant or not.

Sciatica can be a distracting pain that starts in the lower back and extends down into one or both legs. Chiropractic care has proven to be quite helpful.
What Is Sciatica?
Sciatica is a severe pain in the leg caused by compression, irritation, or inflammation of the sciatic nerve. The sciatic nerves are the largest and longest nerves in the body, reaching about the size of your thumb in diameter, and running down the back of each leg. Each sciatic nerve is composed of five smaller nerves that leave the spinal cord from the lower spinal column, join together and then travel down each leg It then divides into ninny smaller nerves that travel to the thigh, knee, calf, ankle, foot and toes When these nerves are irritated or affected by the inflammation of nearby soft tissues, doctors refer to this as sciatica.
Symptoms Of Sciatica
People with "sciatica" can suffer from a wide range of symptoms. Often the pain will come and go at times, it may be constant, but then it may subside for hours or days. Some people may feel only a dull ache or numbness, which travels down the back into the upper leg. For others, it may be intense "shooting pains" down the leg into the foot and toes. Many factors affect the pain of sciatica. Sitting in one position for long periods of time as when driving or working at a computer can increase the pain. Working out or running, or even simple things like walking, bending, turning or standing up may be difficult and painful. Tennis or golf and other twisting activities can cause sciatica pain to flare up. For some, the pain may be in both legs or change from side to side. For a number of others, back pain may occur before the sciatica itself. In the most severe cases, sciatica can damage reflexes, or even cause a wasting of the calf muscles.

The term "sciatica" is commonly used to describe pain traveling in the distribution of the sciatic nerve . Sciatica is a symptom caused by a disorder occurring in the lumbar spine. The sciatic nerve is the largest nerve in the human body, about the diameter of a finger.
Sciatica Fig 1 Sciatic nerve fibers begin at the 4th and 5th lumbar vertebra (L4, L5) and the first few segments of the sacrum. The nerve passes through the sciatic foramen just below the Piriformis muscle (rotates the thigh laterally), to the back of the extension of the hip and to the lower part of the Gluteus Maximus (muscle in the buttock, thigh extension). The sciatic nerve then runs vertically downward into the back of the thigh, behind the knee branching into the hamstring muscles (calf) and further downward to the feet.

The nerve branches from the spine to the left and right buttocks and then travels down the back of the legs, at about the middle of the thighs, to the feet. Sciatic pain usually radiates from the buttock and is felt primarily in the back of the thigh, though it may extend to the foot. Usually, only one side is affected. Although the pain is felt in the fleshy part of the body, the origin of the pain is a pinching of one or more of the nerves departing the spinal cord in the area of the intervertebral disks. Initially, there may be a temporary prolapse or extrusion of the disk (or disks) that causes the spinal column to squeeze down on the nerves. In cases of destruction of the disk, the pain will become chronic. Pressure can also be applied on the nerve from damage to the vertebra themselves.
