    Sarcoidosis Sarcoidosis (SAR-coy-DOH-sis) is an inflammatory disease characterized by granulomas (small rounded outgrowths made up of blood vessels, cells, and connective tissues) that can produce many different symptoms. It is generally a chronic disease, lasting for several years or a lifetime. Some people, however, may have a type that only lasts a few months. Some people may have no symptoms; others may have symptoms that include: skin, lung, and eye problems; arthritis; myositis; fever; fatigue; and weight loss.     Definition: Sarcoidosis is a disease of unknown cause in which inflammation occurs in the lymph nodes, lungs, liver, eyes, skin, and/or other tissues.
    Causes, incidence, and risk factors: The cause of the disease is unknown. Tissue samples from affected organs show clusters of immune cells (macrophages, lymphocytes, and multinucleated giant cells). These clusters are called granulomas. Possible causes include a hypersensitive response to some factor in the environment, a genetic predisposition, or an extreme immune response to infection. The incidence varies widely according to race and sex. The disease is most common among North Americans of African heritage and Northern European Caucasians. Women of African heritage are affected more than men of African heritage. The onset of the disease usually occurs in people between 30 and 50 years old. Sarcoidosis is very rare in young children.

    Diagnosis
    Sarcoidosis What is sarcoidosis? Sarcoidosis is a disease that results from inflammation of tissues of the body. It can appear in almost any body organ, but most often starts in the lungs or lymph nodes. The cause of sarcoidosis is unknown. The disease can appear suddenly and disappear. Or it can develop gradually and go on to produce symptoms that come and go, sometimes for a lifetime. As sarcoidosis progresses, small lumps, or granulomas, appear in the affected tissues. In the majority of cases, these granulomas clear up, either with or without treatment. In the few cases where the granulomas do not heal and disappear, the tissues tend to remain inflamed and become scarred (fibrotic). Sarcoidosis was first identified over 100 years ago by two dermatologists working independently, Dr. Jonathan Hutchinson in England and Dr. Caesar Boeck in Norway. Sarcoidosis was originally called Hutchinson's disease or Boeck's disease. Dr. Boeck went on to fashion today's name for the disease from the Greek words "sark" and "oid," meaning flesh-like. The term describes the skin eruptions that are frequently caused by the illness.

    A 34-year-old man was admitted to the hospital with the presenting symptoms of shortness of breath and weight loss.
    Imaging Findings
    PA and lateral radiographs
    High resolution CT

    Posteroanterior (PA) and lateral radiographs show prominence of both hila ( arrow ), widening of the right paratracheal stripe ( arrow ), and obliteration of the aortopulmonary window ( arrow ). Multiple small nodules are visible in both lungs. The lung volumes are decreased. High resolution computed tomography (HRCT) confirms miliary-like nodules in a subpleural, perihilar, and bronchovascular distribution. Fissures are thickened. Paratracheal ( arrow ), bilateral hilar ( arrow ) and subcarinal ( arrow ) lymph nodes are also demonstrated.

    • Stage I: Hilar and mediastinal lymph node enlargement
    • Stage II: Lymphadenopathy and parenchymal disease
    • Stage III: Parenchymal disease only
    • Stage IV: Pulmonary fibrosis
    Intrathoracic lymphadenopathy is the most common finding in sarcoidosis occurring in over 85% of patients. The most common pattern is right paratracheal and bilateral hilar adenopathy (the "1,2,3 sign"). Unusual patterns of lymph node enlargement occasionally occur. While not common, paratracheal, subcarinal, aortopulmonary window, and other mediastinal lymph node groups can be involved in the absence of hilar disease.

Sarcoidosis and African American Women Sarcoidosis is a relatively common disease affecting people of all ages and sexes. Cochrane reviews are regularly checked and updated if necessary. Background: Pulmonary sarcoidosis is a common condition with an unpredictable course. Oral (OCS) or inhaled steroids (ICS) are widely used in its treatment, but there is no consensus about when and in whom therapy should be initiated, what dose should be given and for how long. Corticosteroids given for several months have deleterious side-effects so it is important to know whether they have any maintained benefit in pulmonary sarcoidosis. Objectives: To determine the randomised controlled trial (RCT) evidence for the benefit of corticosteroids (oral or inhaled) in the treatment of pulmonary sarcoidosis. Search strategy: MEDLINE, EMBASE and CENTRAL were searched using predefined terms. Bibliographies of retrieved RCTs and reviews were searched for additional RCTs. Pharmaceutical companies and authors of identified RCTs were contacted for other published and unpublished studies. Searches are current as of May 2004.

Sarcoidosis of this disease is a multisystem granulomatous disease of unknown etiology that predominantly affects the lungs. The many forms and presentations and the lack of a single diagnostic test can make the diagnosis challenging. Physicians have limited guidance for the treatment of sarcoidosis because of a lack of high-quality randomized controlled trials (RCTs). Sarcoidosis is an acquired idiopathic, multisystem disorder characterized by noncaseating granulomas in the involved tissue. This condition of unknown origin is most common in African Americans and northern Europeans. The organs most commonly affected are the lungs, lymph nodes, skin, and eyes. [1-4] Sarcoidosis typically causes no significant derangements, but on occasion the granulomas can become so large or numerous that they impair the function of the affected organ.

    What is sarcoidosis?
    Sarcoidosis is an inflammatory disease that can attack any organ of the body. However, it is most frequently found in the lungs. Sarcoidosis is characterized by the presence of small areas of inflammed cells called granulomas. Granulomas can be found inside the body, for example, on the walls of the bronchi and bronchioles (breathing tubes) in the lungs. They can also appear as sores on the skin. What are the symptoms of sarcoidosis? Sarcoidosis causes a variety of symptoms or no symptoms at all. Patients suffering from pulmonary sarcoidosis may have a dry cough, shortness of breath and chest pain. There also can be fatigue, weakness and weight loss. For those cases where the disease appears outside the lungs, symptoms can include fever, painful joints, scaly rashes, and bloodshot eyes.
