Imaging Sarcoidosis can involve all of the organ systems of the body, but thoracic lymph nodes and the lung parenchyma are most often and most visibly involved. In the U.S. the chest radiograph is abnormal in more than 90% of patients of patients with sarcoidosis. The reported percentage is less in some other countries. Findings on the chest radiograph are a most important feature of sarcoidosis and these findings are often the initial signpost leading to the diagnosis, frequently in an asymptomatic patient. When a patient presents with a grossly abnormal chest radiograph, has no symptoms, has a normal physical examination and no abnormal laboratory tests, sarcoidosis becomes a likely diagnosis. It should be emphasized, however, that no combination of radiograph and clinical findings are pathognomic. Sarcoidosis is a diagnosis of exclusion based on combined radiologic, clinical, and histologic data.
STAGING
The radiographic abnormalities in the thorax are lymphadenopathy, parenchymal lung disease, or both. Several combinations of lymph node enlargement and lung disease have been divided into four categories or stages. These categories are of some limited clinical use since Stages I and II tend to correspond to reversible acute or subacute disease, while Stages III and IV often indicate progressive or permanent lung disease. The term stages, however, is misleading since patients do not necessarily progress from one stage to the others, and there is poor correlation between the appearance of the lung disease and functional impairment.

What Is It? Sarcoidosis (SAR-coy-DOH-sis) is an inflammatory disease characterized by granulomas (small rounded outgrowths made up of blood vessels, cells and connective tissues) that can produce many different symptoms. It is generally a chronic disease, lasting for several years or a lifetime. Some people, however, may have a type that only lasts a few months. What Are the Symptoms? Some people may have no symptoms; others may have symptoms that include:

What Causes It? Although the cause is unknown, suspected causes include:

What It Is Who Gets It ... Links What It Is, What It Isn't Sarcoidosis is a relatively rare autoimmune disease that can affect the skin, lungs, heart, brain and nervous system, eyes, and other organs. Although the disease was first recognized about 100 years ago, little is known about what causes it and there is no cure. As an autoimmune disease, sarcoidosis is similar to diseases like rheumatoid arthritis and lupus in how it behaves and how it is treated. Depending on the severity, treatment may be needed to control what sarcoidosis does. Sarcoidosis can go dormant on its own or with treatment, and its symptoms often do not recur after the initial onset. In a small percentage of cases, sarcoidosis is chronic, requiring on-going treatment, with possible occurrence in other organ systems. Chronic sarcoidosis is apparently cyclic in its activity level, with flares of symptoms and more dormant periods lasting weeks or months. No markers have been identified as indicating how sarcoidosis is going to behave in any individual or how an individual with sarcoidosis is going to respond to treatment, although a physician experienced with sarcoidosis may be able to make a "good guess" based on his or her experience. Whether he or she is willing to voice that guess is another story entirely! Despite a century of research, little is known about sarcoidosis. Research has discovered what it is not. Among other things, it is not a cancer or tuberculosis or AIDS-related. It is not caused by pine pollen or other identifiable agents like pollution, airborne or otherwise. It is not progressive or contagious. You did not get it from someone else and you cannot give it to another person. As far as we know, nothing you did specifically caused your sarcoidosis.

It is most common in young adults between the ages of 20 and 40. The word "sarcoidosis" comes from the Greek word "sarkodes," meaning "fleshy," and the Greek suffix "-osis," meaning "condition." The "fleshy condition" refers to the skin lesions that often form on various parts of the body. Sarcoidosis is a multisystemic disease that can affect a variety of organs. Pulmonary involvement occurs in greater than 90% of patients and is a leading cause of morbidity and mortality from this disease. Many organs can be involved, in fact, one of the most noticeable symptoms is the peculiar skin lesions, erythema nodusum, that develop on the lower legs. Sarcoidosis is a disease of unknown etiology characterized by granuloma formation in a variety of organs. Granulomas are collections of immune cells and disease that result from granulomatous disorders. There are a variety of granulomatous disorders that closely resemble sarcoidosis, including tuberculosis. The close resemblence to tuberuculosis has led to speculation that sarcoidosis is due to an infectious organism. It is the formation of these granulomas and the resulting tissue damage caused by the cells within these granulomas that cause the symptoms of sarcoidosis.

Sandra Conroy, who has sarcoidosis , knows first-hand the frustrations and obstacles that are often encountered by persons with this disease. When she was diagnosed in 1984, information on sarcoidosis was extremely limited. As a result of her extensive research, and using her own patient questionnaire , she developed a computer data base relating to the symptoms and demographics of sarcoidosis patients. She also authored the Sarcoidosis Resource Guide and Directory. In 1992, Ms Conroy founded the National Sarcoidosis Resource Center, which has grown into an international organization with a registry of over 30,000 patients. She has been invited to speak at numerous medical conferences and health forums in the United States and other parts of the world.
