Training Tips Runners Knee: The Number One Injury Effecting The Runner
by John W. Zandler, D.C., C.C.S.P. In researching 10,271 athletic injuries treated by a large sports medicine facility running accounted for the largest percentage (37%) of injuries treated. Upon further review of such running injuries, the knee joint was the most commonly involved part of the body. Furthermore, it was found that training errors including a sudden change in training, endurance/intensity, or miles was the most common factor precipitating over use injuries thus resulting in time off from training. Additional factors of injury include structural-anatomical problems resulting in altered body biomechanics and over use of the knee joints, muscles, ligaments and bony elements. Thus, due to the prevalence of knee injuries, the following article will focus on common injuries affecting the knee joint. In determining the origin of runner's knee it has been previously stated that training errors represent the most common factor leading to runner's knee. Additionally, a biomechanical basis for this injury is noted which refers to the altered mechanical ability of the knee joint to perform, thus, resulting in overuse of it's muscle, ligament and bony elements. Such altered mechanics can result from a variety of anatomical/structural factors including underlying leg length imbalances, muscle strength-flexibility imbalances, excessive pronation!,supination, flat feet and previous traumatic injury to the knee possibly resulting in an underlying degenerative-arthritic problems including the knee-patella femoral joints. These factors either individually or in various combination result in altered knee mechanics which places additional stress and resultant microtrauma to the knee's bony and ligamentous elements.

From physical therapist Dave Lupia Runners Knee: This is an injury related to the complex of the quadriceps, femur, patella (kneecap) and patellar tendon. Symptoms: Can include a dull aching pain in the lower kneecap. It can be severe after runnings or after extended periods of sitting. Causes: Wide hips, knock knees, dislocating patella, high patella, weak inner quadriceps muscle, tight hamstrings or calf muscles, pronation of the feet. Treatment: Reduce or stop running, allowing time to rest and heal. R.I.C.E. rest, ice, compression, elevation. Cross train with non-impact activities like biking, elliptical training or swimming. Overpronators should get a proper foot assessment and invest in quality running shoes. Strengthen quadriceps, primarily the interior aspect to assure proper tracking of the kneecap. Increase flexibility in posterior muscles, especially hamstrings and calves. Consult a physician or physical therapist if the problem is not resolved in two weeks.

The knees of female athletes are very vulnerable to injury, especially ACL tears. Here's what to do about it.
When it comes to fitness training, it is sometimes difficult to find a female-specific topic because the same principles and practices apply to both women and men. Nevertheless, there are other gender-specific issues, and one important one that was discussed recently in the Journal of Sport Rehabilitation is the fact that women are more likely to suffer knee injuries. And, most seriously, women have a greater risk of suffering a non-contact anterior cruciate ligament (ACL) tear. An ACL tear is one of the most serious injuries in sport, normally requiring surgery and a long period of rehabilitation. A non-contact tear occurs without impact with another person or object ('ACL injuries in the female athlete', Journal of Sports Rehabilitation, 6, pp 97-110, 1997).
In the review article, Ireland, Gaudette and Crook, three sports doctors from Kentucky, discuss research that shows greater incidence of ACL injuries in female athletes. Comparing within the same sports, such as soccer and basketball, females suffer more matchplay knee injuries and the injuries are usually more serious. For example, a female basketball player is five times more likely to suffer a non-contact ACL tear than a male basketball player of the same level. Further evidence comes from a survey completed by athletes who competed in the 1988 US Olympic trials. Thirteen out of 64 females suffered an ACl tear compared to only three out of 80 males. In addition, there was a greater need for surgery with the female ACL tears.

There are many common injury presentations for runners. Go figure, but there seems to be a fixation with the idea that we are literally running our knees into the ground with every step we take. Turns out that knee injuries are not inevitable and, though common, are easily corrected if properly managed. Knee pain need not deter you from your ideal running program.
Runner's Knee
(Chondromalacia of the patella) Description: Pain around and sometimes behind the kneecap. One of the most common injuries among runners, runner's knee most often strikes as runners approach forty miles per week for the first time. Even after taking a few days off, the pain seems to come right back, sometimes even intensifying, after the first few miles of the next run. The pain often feels worst when running downhill or walking down stairs, and the knee is often stiff and sore after sitting down for long periods. You might hear a crunching or clicking sound when you bend or extend your knee. The sure-fire test for runner's knee: sit down and put your leg out on a chair so that it's stretched out straight. Have a friend squeeze your leg just above the knee while pushing on the kneecap. She should push from the outside of the leg toward the center. At the same time, tighten your thigh muscle. If this is painful, you're looking at runner's knee.

The knee is subject to enormous workloads during sports activities. The muscles that surround the knee provide mobility and strength to the legs. When athletes run or jump, the kneecap alone often endures forces of 1000 to 1500 pounds. The entire knee joint is under massive stress with these kinds of activities, especially when there are imbalances in muscle strength or flexibility. In time, overuse injuries, such as runner's knee, may develop. The main symptom of runner's knee is a dull, aching pain under or around the kneecap. It is felt when the kneecap repeatedly grinds against the thigh bone (femur), the bone behind the kneecap. This causes the cushioning behind the knee cap to break down. The break down of this cushioning is progressive. First, it softens. Then, it might fray, crack, crater, or tear. The pain is most noticeable going down stairs or hills. Unequal leg length, turned-in thighbones, and flat feet can increase the risk for developing runner's knee. Nearly 30 percent of runners eventually develop runner's knee. Skiers, cyclists, soccer players, and people who participate in high-impact aerobics classes may also get runner's knee.
