as published in the International Primer Rickets is a disease of the bones and muscles and is due to vitamin D and calcium deficiency ( view photo ). 1 It was first described in the 17th century 2 and causes delayed growth, weakening and bowing of weight-bearing bones, ineffective tooth enamel formation, and muscle weakness due to low levels of calcium. One of the classic physical manifestations of rickets is frontal bossing (enlarged forehead) and box-like shape of the head. Rickets is a prevalent health problem in children all over the world, not just for children living in orphanages. It is one of the top five medical issues in children living in China (malnutrition, rickets, anemia, lead poisoning, and asthma).3 Children living in orphanages do not get enough calcium and vitamin D in their diet and they lack exposure to sunlight, decreasing the skin's ability to produce adequate vitamin D.4, 5 The muscle weakness (hypotonia) due to the lack of calcium can cause the child to appear floppy. When the child arrives in the U.S., the child may appear developmentally delayed as a result of the decreased muscle tone and weakness of the bones due to rickets. Lack of stimulation and lack of exercise can also contribute to muscle weakness, especially muscles of the abdomen (belly). When a baby lies in the supine position (lying on the back) chronically, the abdominal (belly) muscles are weak and malnutrition can cause muscle wasting. Positioning on the belly improves this condition swiftly. Improved nutrition with vitamin D and calcium supplemented foods (formula, milk, milk products, green leafy vegetables, calcium supplemented juices) and vitamins, exposure to sunlight, and exercise rapidly improves the child's muscle tone and bone strength.

Rickets is a failure of the proper development (mineralization) of growing bone ( view photo ). It is probably present in 100% of all children adopted from China, even when it is not apparent to the observer. The predominant cause of rickets in orphanages abroad is nutritional deficiency of vitamin D due either to inadequate direct exposure to ultraviolet rays in sunlight or to inadequate intake of vitamin D, or both. Sunlight is necessary for the formation of vitamin D in the human body and vitamin D is necessary to make bone in the human body. Inadequate calcium and phosphorus in a child's diet also contributes to the development of rickets. When a child first arrives from China, rickets can be diagnosed by a skilled pediatrician who performs a thorough physical exam and orders a blood test called a "rickets screen". The rickets screen consists of alkaline phosphatase, calcium, and phosphorus. The calcium, phosphorus, and alkaline phosphatase levels in the blood may help support the diagnosis of rickets. The child with mild rickets may have no abnormalities in the rickets screen and a normal diet will effectively reverse any weakness in bones and muscles. If the child has bow legs, raised bony bumps along the ribs (rachitic rosaries), frontal bossing of the skull, and other deformities of the bones, rickets might be the cause. Some children have no obvious bony deformities, but still may have weak bones and muscles from a mild form of rickets.

Vitamin D is necessary for proper bone growth, but it is not found in many foods naturally. It is synthesized in the skin with exposure to sunlight, but many babies do not get much exposure to the sun for a number of reasons, such as not spending time outdoors and wearing clothes, hats, or sunscreen. The darker an infant's skin, the more sun exposure is needed to synthesize vitamin D. Although breastfeeding is the ideal form of nutrition for infants, it does not supply the vitamin D needed for healthy bone growth. Nutritional rickets is a condition in which children's bones are too soft and do not develop properly. Thanks largely to vitamin D supplementation of infant formula and recognition of the need for sunlight, rickets had become largely a thing of the past in the United States. However, cases of nutritional rickets among babies have been on the rise recently.

X-Ray Vitamin D may be absorbed from food by the intestines or may be produced by the skin when the skin is exposed to sunlight. In its active form, vitamin D acts as a hormone to regulate calcium absorption from the intestine and to regulate levels of calcium and phosphate in the bones. Sunlight is important to skin production of vitamin D, and environmental conditions where sunlight exposure is limited may reduce this source of vitamin D. Lack of vitamin D production by the skin may occur with indoor confinement or working indoors during the daylight hours, or in climates with little exposure to sunlight.

Rickets is caused by the failure of osteoid to calcify in the growing person or animal. Failure of osteoid to calcify in the adult is called osteomalacia. Rickets occurs when the metabolites of vitamin D are deficient. Less commonly, dietary deficiency of calcium or phosphorus may produce rickets. Vitamin D (cholecalciferol [vitamin D-3], a steroid compound) is formed in the skin under the stimulus of ultraviolet light. Ultraviolet light was the only significant source of vitamin D until early in the twentieth century, when ergosterol (vitamin D-2), which is contained in fish liver oil or as an irradiated plant steroid, was discovered. Ergosterol can be taken orally with good effect.
I have raised 6 children via macrobiotics. No rickets. I have not seen any cases of rickets in my 30 years of macrobiotics.

WHO IS AT RISK?
All babies who receive only breastmilk for 6 months or more, all breastfed babies of color, premature babies, and infants weaned to vegan diets.
WHAT ARE THE SIGNS OF RICKETS?
  • Weakness,
  • unable to stand or walk,
  • slow growth,
  • seizures,
  • irritability.
HOW CAN YOU PREVENT RICKETS IN YOUR BABY?
The easiest way to prevent rickets is sun exposure. Since sun is limited in the Northwest, Vitamin D should be given once a day for the first 2 years.
HOW DO YOU GET VITAMIN D?
Vitamin D is available at stores without a prescription. It comes in a preparation with other vitamins (vitamins A and C) called Tri-vi-sol. If you are on medical coupons, ask your doctor or WIC office for a prescription for Vitamins written as "medically necessary for exclusively breastfed infants to prevent rickets." Yes, this is still possible. When someone has a deficiency of vitamin D their bones become weak and soft, which means they may develop an abnormal shape and are certainly at risk of breaking easily. In children vitamin D deficiency is called rickets; in adults it's known as osteomalacia. Bones need calcium to make them strong. However, to absorb calcium (for example from dairy food, fortified bread and cereals) the body also needs vitamin D. Some of the vitamin D we need comes from our diet. Few foods actually contain vitamin D but those such as fish, eggs, and milk are quite a good source. The body also manufactures vitamin D when the skin is exposed to sunlight. Children who are deficient in vitamin D may not grow to their full potential and they sometimes have knock-knees or bowed legs. Adults may suffer pain in the bones and have difficulty climbing stairs or getting up from a squatting position. This condition, and the potential problems described above, can be prevented and treated by eating a diet with enough vitamin D and ensuring the skin gets an adequate exposure to sunlight.

There is widening of the growth plate. The femoral and tibial metaphysis are frayed and cupped. Diagnosis: Rickets Discussion: Rickets is described as a group of diseases demonstrating similar pathologic, radiologic, and histologic abnormalities. Pathologically, it results from an interruption in the orderly development and mineralization of the growth plate, so changes are more obvious in regions of most active growth, i.e. the costrochondral junction of the ribs, the distal femur, proximal humerus, both ends of the tibia and distal radius and ulna. This results in widening of the growth plate and zone of provisional calcification which becomes irregular. Disorganization and "fraying" with widening and cupping of the metaphyis. The deformities caused by rickets exhibit different patterns depending on the child's age when the disease develops. In infants, craniotabes is a manifestation. During infancy and childhood, long bone abnormalities predominate with bowing deformities. Later on scoliosis, basilar invagination, and triradiate configuration of the pelvis may be seen.

Etiology of rickets can be divided into two main categories: 1. Diminished/ineffective production of active Vit D. This includes

Most of us thought that rickets was a thing of the past, but to the surprise and concern of many, rickets is on the rise again in developed countries such as the U.S. We believe rickets is on the rise for several reasons: Babies may be getting less exposure to sunlight, and sunlight is an important source of vitamin D. Today, fewer doctors recommend that pregnant women and infants take vitamin D, meaning that many nursing mothers and their infants may be deficient in this nutrient. Finally, health professionals have long wanted to see more minority mothers breastfeed their babies, but, ironically, the increased number of minority babies being breastfed is associated with increases in cases of rickets.
Most of the rickets cases in countries like the U.S. in the past ten years have been in breastfed infants and children who have dark skin, or who stay fully clothed for religious or social reasons. These babies, who get little sunshine on bare skin, or whose dark skin protects them from the sun, are the babies most likely to get less vitamin D from sun exposure than they need. These breastfed babies have another risk, since their mothers' breast milk is also more likely to be deficient in vitamin D.
