Overview
Pectus excavatum (PE) is defined as an abnormal formation of the rib cage where the breastbone caves in, resulting in a sunken chest appearance. Many doctors describe PE as a cosmetic defect or a congenital defect that is correctable only through surgery. But is it? This web site looks at the environmental factors that may influence pectus excavatum and the other rachitic skeletal features that often occur along with it, such as bowed limbs, scoliosis and flat feet.

What are osteomalacia and rickets?
These are disorders caused by insufficient levels of vitamin D in the body. They are really the same condition; rickets is the name used when it occurs in children whereas osteomalacia is the term used for adults.
Where do we get vitamin D from?
There are two sources of vitamin D:
  • a person's diet
    formation in the skin through exposure to sunlight.
There are few dietary sources of vitamin D. The best ones are fatty fish such as salmon and sardines, and margarines supplemented with vitamin D.
Milk contains added vitamin D in the USA but not in the UK. Most people in the UK get most of their vitamin D from exposure of the skin to sunlight.

Osteomalacia
Osteomalacia means "soft bones". Osteoid is the bone protein matrix, composed primarily of type 1 collagen. When there is insufficient mineral or osteoblast dysfunction, the osteoid does not mineralize properly, and it accumulates. Many cases of osteomalacia are related to abnormalities in vitamin D When the newly formed bone of the growth plate does not mineralize, the growth plate becomes thick, wide and irregular. This results in the clinical diagnosis of rickets, and is seen only in children because adults no longer have growth plates. When the remodeled bone does not mineralize, osteomalacia occurs, and this happens in all ages. Most of the hereditary causes of osteomalacia appear during childhood and cause rickets. The following table shows a list of causes of osteomalacia and some of the biochemical abnormalites that can be seen: Osteomalacia may be suspected on a clinical basis, especially when there is a family history or lab abnormalities listed on the table above. In uncertain cases, the most reliable way to establish the diagnosis is with an UNDECALCIFIED bone biopsy.     U.S. health officials say lack of sunlight and too little milk are contributing to a resurgence of rickets By Christy Oglesby CNNfyi ATLANTA, Georgia (CNN) Societal and environmental changes such as long commutes, smog and day care are contributing to the resurgence of childhood rickets a bone softening disease that health officials thought they had all but eliminated 70 years ago. Atlanta health officials have detected about 10 cases of the rare disease, said Dr. Norman Carvalho of Children's Healthcare of Atlanta, a children's hospital. But there may be more, he said, because the disease's rarity may make it unrecognizable to physicians. And if doctors do ascertain that a patient has rickets, they are not required to report it to the state health department. Carvalho was the leader in a Georgia study on malnutrition that will appear in the April issue of the journal Pediatrics. The study, conducted from 1997 to 1999, found that about one in 200,000 children in Georgia is hospitalized with rickets.

    Vitamin D deficiency exists when the concentration of 25-hydroxy-vitamin D (25-OH-D) in the blood serum occurs at 12 ng/ml (nanograms/milliliter), or less. The normal concentration of 25-hydroxy-vitamin D in the blood serum is 25-50 ng/ml. When vitamin D deficiency continues for many months in growing children, the disease commonly referred to as rickets will occur. A prolonged deficiency of the vitamin in adults results in osteomalacia. Both diseases involve defects in bones. Description
    Vitamin D is a fat-soluble vitamin, meaning it is able to be dissolved in fat. While some vitamin D is supplied by the diet, most of it is made in the body. To make vitamin D, cholesterol, a sterol that is widely distributed in animal tissues and occurs in the yolk of eggs, as well as in various oils and fats, is necessary. Once cholesterol is available in the body, a slight alteration in the cholesterol molecule occurs, with one change taking place in the skin. This alteration requires the energy of sunlight (or ultraviolet light). Vitamin D deficiency, as well as rickets and osteomalacia, tends to occur in persons who do not get enough sunlight and who fail to eat foods that are rich in vitamin D. Once consumed, or made in the body, vitamin D is further altered to produce a hormone called 1,25-dihydroxy-vitamin D (1,25-diOH-D). The conversion of vitamin D to 1,25-diOH-D does not occur in the skin, but in the liver and kidney. First, vitamin D is converted to 25-OH-D in the liver; it then enters the bloodstream, where it is taken-up by the kidneys. At this point, it is converted to 1,25-diOH-D. Therefore, the manufacture of 1,25-diOH-D requires the participation of various organs of the bodythe liver, kidney, and skin.
