Allergic rhinitis
Dr Adrian Morris Perennial allergic rhinitis is usually triggered by indoor allergens such as house dust mite and pet skin flakes. It's similar to hay fever - the allergen causes inflammation and irritation in delicate linings in the nose and eyes.
In this article
What are the causes? What are the symptoms? What is the treatment? What can be done to prevent allergic rhinitis? Sixteen per cent of the population suffer with allergic rhinitis - of these, eight per cent have perennial symptoms, six per cent perennial and seasonal symptoms and two per cent have seasonal symptoms alone (according to the British Medical Journal, 1998). As with asthma and eczema, the incidence of allergic rhinitis is increasing and it runs in families.
What are the causes?
Perennial allergic rhinitis starts in early childhood and occurs all year round. It's caused by allergy to the droppings of house dust mites or pet skin flakes. Occasionally indoor mould spores and, in rare cases, food allergy can be causes. There is often a family predisposition to developing rhinitis and other allergies.
What are the symptoms?

Back to table of contents Allergic Rhinitis Allergic rhinitis is an IgE mediated inflammatory disease of the nasal mucosa. IgE antibodies are attached to mast cells near mucosal surfaces. When the individual comes into contact with airborne allergens, the mast cells release chemical mediators histamine, leukotrienes, kinins, and prostaglandins. These substances cause the reactions that lead to the allergic symptoms. The initial response is followed by a late chronic inflammatory phase characterized by proliferation of mast cells, and invasion of the mucosa by eosinophiles, basophiles, and the proliferation of lymphocytes. This inflammatory response causes the increased sensitivity to non-specific stimuli like smoke, strong odors, and insecticides. Allergic rhinitis affects about 10-20% of the pediatric population and may be seasonal (pollens and grasses) or perennial (animals, dust mites, cockroaches). Common Symptoms

    Allergic rhinitis
    Definition
Allergic rhinitis, more commonly referred to as hay fever, is an inflammation of the nasal passages caused by allergic reaction to airborne substances. Description
Allergic rhinitis (AR) is the most common allergic condition and one of the most common of all minor afflictions. It affects between 10-20% of all people in the United States, and is responsible for 2.5% of all doctor visits. Antihistamines and other drugs used to treat allergic rhinitis make up a significant fraction of both prescription and over-the-counter drug sales each year. There are two types of allergic rhinitis: seasonal and perennial. Seasonal AR occurs in the spring, summer, and early fall, when airborne plant pollens are at their highest levels. In fact, the term hay fever is really a misnomer, since allergy to grass pollen is only one cause of symptoms for most people. Perennial AR occurs all year and is usually caused by home or workplace airborne pollutants. A person can be affected by one or both types. Symptoms of seasonal AR are worst after being outdoors, while symptoms of perennial AR are worst after spending time indoors. Both types of allergies can develop at any age, although onset in childhood through early adulthood is most common. Although allergy to a particular substance is not inherited, increased allergic sensitivity may "run in the family." While allergies can improve on their own over time, they can also become worse over time.

Rhinitis is inflammation and swelling of the mucous membrane of the nose, characterized by a runny nose and stuffiness and usually caused by the common cold or an allergy. The nose is the most commonly infected part of the upper airways. Rhinitis may be acute (short-lived) or chronic (long-standing). Acute rhinitis commonly results from viral infections but may also be a result of allergies or other causes. Chronic rhinitis usually occurs with chronic sinusitis (chronic rhinosinusitis). Viral Rhinitis: Allergic Rhinitis: Allergic rhinitis is caused by a reaction of the body's immune system to an environmental trigger. The most common environmental triggers include dust, molds, pollens, grasses, trees, and animals. Symptoms include sneezing, runny nose, stuffiness, and itchy, watery eyes. A doctor may diagnose allergic rhinitis based on a person's history of symptoms. Often, the person has a family history of allergies. More detailed information may be obtained using blood tests or skin testing. Avoiding the substance that triggers the allergy prevents symptoms but is often not possible. Nasal corticosteroid sprays decrease nasal inflammation caused by many sources and are relatively safe for long-term use. Antihistamines help prevent the allergic reaction and thus symptoms. Antihistamines dry the mucous membrane of the nose but many of them also cause sleepiness and other problems, especially in older people. Newer ones require a prescription but do not have these side effects. Allergy shots (desensitization) help to build long-term tolerance to specific environmental triggers, but they may take months or years to become fully effective. Antibiotics do not relieve the symptoms of allergic rhinitis.

This is the first of five articles on allergy Preview : "Allergy season" may be somewhat of a misnomer, since for some people, allergies are always in season. But for many, rhinitis and other symptoms are strongly influenced by Mother Nature and are at their worst during the annual periods of pollen overload. Dr Graft describes how to determine the cause of rhinitis, whether allergy related or not, and how to choose the most effective medication on the basis of disease severity and accompanying problems. C omplaints of nasal symptoms are not uncommon among patients visiting primary care physicians. Allergic rhinitis is usually described as seasonal or perennial (depending on the offending allergen), and the two types often coexist. "Nonallergic rhinitis" is an encompassing term for a variety of disorders, some of which are affected by environmental factors although no allergies are present. Allergic rhinitis
Allergic rhinitis is the most common allergic disorder. In a genetically predisposed person, exposure to certain substances can lead to generation of IgE antibodies that are directed against a specific allergen. On re-exposure to the allergen, binding to the IgE antibodies causes release of a host of mediators of allergic response, including histamine, prostaglandins, and leukotrienes. This multiplicity of mediators helps explain why antihistamines alone often do not control symptoms adequately. Binding of these mediators to receptors on blood vessels and other tissues results in clinical symptoms, which may begin within minutes of exposure (ie, an early-phase response). About half of patients also experience recrudescence of symptoms about 4 to 8 hours after exposure to the allergen (ie, a late-phase response) (1).

    Allergic Rhinitis
    Overview
    What is allergic rhinitis?
Allergic rhinitis (often called hay fever) is an overreaction of the immune system to particles in the air that you breathe. The immune system normally protects the body from viruses and bacteria by producing antibodies to fight them. With allergic rhinitis, the immune system starts fighting essentially harmless substances, like dust mites and pollen ( allergens ), as though they were trying to attack your body. This overreaction ( allergic reaction ) causes inflammation and symptoms that affect mainly the nose but also the eyes, ears, throat, and mouth. Your body may not have an allergic reaction the first time you breathe in an allergen such as grass pollen, for example. The next time you inhale that pollen, however, you may develop a stuffy nose and have trouble breathing. This is caused by the response of your immune system. You may have allergic rhinitis for many years (chronic). During this time, you may lose some of your sensitivity to allergens and your reaction will not be as severe. You may also develop complications such as

We and our collaborator, sanofi-aventis , have discovered and are developing several TLR Therapeutic product candidates for the treatment of certain respiratory conditions. Sanofi-aventis has the right to develop up to four of our TLR Therapeutics for the treatment of asthma, allergic rhinitis and chronic obstructive pulmonary disease. Asthma and allergies occur when the immune system mounts an inappropriate Th2 response against normally harmless environmental antigens, such as pollens or peanuts. Coley’s TLR Therapeutic compounds for respiratory diseases stimulate TLR9 and initiate an immune system cascade that mimics the effects of an infection, thereby switching the immune system away from the Th2 state. Once the immune system has switched away from the Th2 state, an inappropriate allergic reaction will no longer occur in response to that stimulus. We believe that this novel mechanism of action orchestrates fundamental modifications in immune system responses and redirects pre-existing hypersensitive immune reactions into more normal responses. Coley’s asthma and allergy product candidates, in development with sanofi-aventis, will be better able to treat the causes, rather than the symptoms, of these conditions and may offer a safer, more natural, effective and convenient method of preventing asthmatic and allergic reactions than current products. Specifically, we believe that our product candidates will be able, through their novel mechanism of action, to:

What is allergic rhinitis? Rhinitis is a reaction that occurs in the nose when airborne irritants (allergens) trigger the release of histamine. Histamine causes inflammation and fluid production in the fragile linings of nasal passages, sinuses, and eyelids.
There is usually a family history of allergic rhinitis.
What are the types of allergic rhinitis? The two categories of allergic rhinitis include:
    • seasonal - occurs particularly during pollen seasons. Seasonal allergic rhinitis does not usually develop until after 6 years of age.
    • perennial - occurs throughout the year. This type of allergic rhinitis is commonly seen in younger children.

What are the causes of allergic rhinitis? The most common causes of allergic rhinitis include the following:
    • pollen
    • dust mites
    • mold
    • animal dander

What are the symptoms of allergic rhinitis? The following are the most common symptoms of allergic rhinitis. However, each child may experience symptoms differently. Symptoms may include:
