Vasomotor Rhinitis (VMR) or idiopathic non-allergic rhinitis Vincent St Aubyn Crump - Article Written: January 2003 Rhinitis is defined as an inflammation of the nasal mucosa and it is characterised by nasal obstruction, runny nose (rhinorrhea), sneezing, and (itching) pruritus. The causes of rhinitis can be broadly categorised into 3 headings: Allergic Rhinitis Seasonal - Hay Fever
Persistent (perennial) eg House dust mite-induced Infectious Rhinitis (eg the common cold). Children, particularly young children in school or day care centres, may have from eight to 12 colds each year. Viral infections are self-limiting and usually last 7-10 days. Non-allergic, Non-Infectious eg Vasomotor Rhinitis (or Irritant rhinitis). "Vaso" means blood vessels and "motor" refers to the nerves, which innervates nasal tissue and the blood vessels. This is sometimes referred to as idiopathic non-allergic rhinitis. It is estimated that up to 10% of the population suffers from non-allergic rhinitis. Mixed Allergic and Non-allergic Rhinitis probably account for the majority of cases. This is an important category to recognise, as allergen avoidance measures only, will give sub-optimal improvements.

What is allergic rhinitis?
Allergic rhinitis (often called hay fever) is an overreaction of the immune system to particles in the air that you breathe. The immune system normally protects the body from viruses and bacteria by producing antibodies to fight them. With allergic rhinitis, the immune system starts fighting essentially harmless substances, like dust mites and pollen ( allergens ), as though they were trying to attack your body. This overreaction (

Non-Allergic Rhinitis
Not everyone with a snuffly nose has an allergy. Some have a sinus infection. Others have polyps or twisted nasal anatomy. In non allergic rhinitis (vasomotor rhinitis), there appears to be an imbalance between the function of nerves supply the blood vessels and glands in the nose.
IMPORTANT The information provided is of a general nature and should not be used as a substitute for professional advice. If you think you may suffer from an allergic or other disease that requires attention, you should discuss it with your family doctor. What is non allergic rhinitis? Not everybody with a blocked or snuffly nose has allergy. Some will have a sinus infection. Others will have a blocked nose due to twisted anatomy, or an overgrowth of the lining of the nose and sinuses known as polyps. A dry nose sometimes develops with age, or after sinus surgery. In non-allergic rhinitis, there appears to be an imbalance in the function of nerves that make mucous glands secrete fluid and which cause blood vessels to swell or contract. Patients may complain of blockage, runny nose, postnasal drip and even sneezing and itching. The condition usually lasts for years. What makes it worse?

3. Allergic rhinitis affects 20 to 40 million people in the United States annually, including 10% to 30% of adults and up to 40% of children. 4. The severity of allergic rhinitis ranges from mild to seriously debilitating. 5. The cost of treating allergic rhinitis and indirect costs related to loss of workplace productivity resulting from the disease are substantial. The estimated cost of allergic rhinitis based on direct and indirect costs is 2.7 billion dollars for the year 1995, exclusive of costs for associated medical problems such as sinusitis and asthma. Rhinitis is also a significant cause of lost school days. 6. Risk factors for allergic rhinitis include: (1) family history of atopy; (2) serum IgE 100 IU/mL before age 6; (3) higher socioeconomic class; (4) exposure to indoor allergens such as animals and dust mites; (5) presence of a positive allergy skin prick test. Rhinitis is reported to be a very frequent disease, although data regarding the true prevalence of rhinitis are difficult to interpret. Most population surveys rely upon physician-diagnosed rhinitis for their data, and this may give rise to a much lower reporting of rhinitis. Some population studies have been done with questionnaires administered to the subjects followed in many cases by telephone interviews to try to make a specific diagnosis of rhinitis. These studies may reflect a more accurate prevalence of rhinitis but probably still underreport this disease. Most epidemiologic studies have been directed towards seasonal allergic rhinitis, or hay fever, since this symptom complex with its reproducible seasonality is somewhat easier to identify in population surveys. Perennial allergic rhinitis is more difficult to identify because its symptom complex may overlap with chronic sinusitis, recurrent upper respiratory infections, and vasomotor rhinitis.

Hay fever and rhinitis
Every year millions of people in the UK experience the discomfort of hay fever. Some people only have symptoms during the summer. Others have hay fever-like symptoms all year round (called 'perennial allergic rhinitis'). This information describes the symptoms and causes of both conditions. It offers practical advice on how to avoid your triggers and find the treatment that's right for you.
Case study: Doreen Moore and her daughter Claire
I developed hay fever three years ago and have had the summer sneezes ever since. I tend to get an itchy nose and my eyes water. I take antihistamines as soon as my symptoms start. I'm quite lucky because if I take my antihistamines quickly enough they really do keep my hay fever under control. My ten-year-old daughter Claire isn't so lucky. Claire has severe hay fever. From early April she takes mild antihistamines and then takes increasingly strong ones as the summer goes on. She also has to use a nasal spray and eye drops. Unfortunately, pollen is a trigger for Claire's asthma too so she has to adjust her asthma medication during the summer. Claire's hay fever has a big impact on her own life and ours. We've all had to learn how to adapt. For example, Claire's eyes run and her nose itches as soon as she steps outside, and she's learned that she's better off reading indoors while the rest of us are out in the garden.

rhinitis otitis media can be caused by airborne and food allergy.
http://www.nutramed.com/ent/rhinitis.htm
Home Modular Nutrition Logon Eye, Ear, Nose and Throat The ear, nose, and throat are the most common target organs for food allergens. Congestion or inflammation of the nose (rhinitis), sinuses (sinusitis), and throat (pharyngitis) may be due to airborne irritants and allergens; however, food allergy may be the undiagnosed cause of these common problems. Food allergy is suggested by these respiratory symptoms: nasal stuffiness, snoring, increased mucus flow in nose and throat and recurrent sore throats. Recurrent middle ear "infections" are very common in the first five years of life and may be eliminated by proper diet revision. Milk, wheat, and egg white allergy are the most common cause of respiratory symptoms, but many foods do this. Otitis Media Allergic Eye Symptoms Asthma Antihistamines ... More on Food Allergy Allergic Rhinitis Allergic rhinitis must be one of the most obvious environmental diseases. Hay fever is the prototype of type I allergy and the mechanism was thought to be a straight forward histamine-mediated response to inhaled antigen. The early immediate responses is characterized by sneezing and anterior nasal discharge, often profuse. While there are patients who just have hay fever attacks as discrete events and asymptomatic between pollen or dust exposures there is larger group of patients who develop more chronic symptoms. With continued antigenic exposure a chronic cell-mediated inflammation takes over. The chronic state is more difficult to characterize and understand. Airborne antigens must be absorbed into the mucosa and processed. If the antigens are derived from large structures such pollens, then obviously antigens must be extracted from the pollen as proteins or peptides which are then ingested and presented by macrophages.

Allergic Rhinitis
Betty Wray, MD and Allergy-Immunology Section Allergic Rhinitis – Diagnosis Introduction: Allergic rhinitis is extremely common affecting 14-20% of the general population, and 60-70% of atopic patients. It is seen more commonly if there is a family history of allergy, and begins from infancy to young adulthood. Allergies can be classified as seasonal (e.g. tree pollens in spring, grass in late spring and summer, ragweed in late summer and fall), perennial (house dust mites, molds, animal dander, foods) or irritants (smoke, gas fumes). Pathogenesis: Allergens bind to specific IgE on surface of mast cells in respiratory tissue. Bridging of IgE triggers release of chemical mediators such as histamine, leukotrienes, C4, D4. Evaluation: The history is the most important part of the evaluation. Symptoms such as rhinorrhea, nasal congestion, paroxysms of sneezing, postnasal drip, pruritus of eyes, nose, soft palate and ears, allergic salute, snorting and sniffing, seasonal and dietary variations/exposures.

Hay fever (seasonal allergic rhinitis)
This factsheet is for people who have hay fever. Sneezing, a blocked or runny nose and itchy eyes are common symptoms of hay fever, also called seasonal allergic rhinitis. This condition affects 15 to 20 percent of people in the UK and, like other allergic conditions such as eczema, seems to be becoming more common, especially in children.
What is hay fever?
Hay fever is an allergic reaction to pollen. Pollen is an allergen, the term given to any substance that can cause an allergic reaction. Your allergic reaction happens when your body's immune system mistakes the allergen for a harmful invader. Most people are not sensitive to allergens.

Allergic and Non-Allergic Rhinitis
Do you suffer from a runny or stuffy nose much of the time? In the U.S., about 40 million people do. This problem is known as rhinitis. Approximately 40 million people in the U.S. have rhinitis. There are several types of rhinitis.
What Are the Types of Rhinitis?
Allergic Rhinitis
If you sneeze and have a runny or stuffy nose during the spring, summer or fall allergy seasons, you may have seasonal allergic rhinitis or hay fever. Hay fever is the most common type of allergy problem. It mainly affects the eyes and nose. Hay fever symptoms include sneezing, itching, runny or stuffy nose and red, watery eyes. Rhinitis can be a problem all year or only some of the year. It can be a problem when inside or when outside. Allergy symptoms are caused when someone has a problem when around a certain substance. These substances are called allergens. They can be inside, such as cats or dust. They can be outside such as tree and grass pollen in the spring and weed pollen and molds in the summer and fall. Hay fever is mainly an allergy caused by outdoor allergens.
