Rheumatological conditions are amongst the most prevalent pathologies affecting older people causing disability and incurring very large direct and indirect healthcare costs. The report was produced in response to the National Service Framework (NSF) for older people. BELFAST, NORTHERN IRELAND. Systemic lupus erythematosus (SLE) is a chronic inflammatory disease. It can manifest itself via a photosensitive facial rash, fatigue, anorexia, weight loss, and night sweats and can progress to life-threatening involvement of the heart, lungs, kidneys or central nervous system. Flare-ups of SLE are typically followed by periods of clinical remission. Fish oils and copper have both been found useful in the treatment of other inflammatory diseases, so researchers at the University of Ulster decided to see if supplementation with one or both of these would help alleviate SLE symptoms. Their clinical trial involved 52 SLE patients who were randomly assigned to receive 3 grams/day of fish oil providing 540 mg/day of EPA (eicosapentaenoic acid) and 360 mg/day of DHA (docosahexaenoic acid), 3 mg/day of copper in the form of a copper di-glycinate amino acid complex, both fish oil and copper, or a placebo. The study lasted 24 weeks and participants were assessed at baseline, 6, 12 and 24 weeks. The researchers found that disease activity at 24 weeks, as measured by the SLAM-R score, was significantly less in the groups that had supplemented with fish oil than in the placebo and copper only groups. They conclude that supplementation with fish oil may be effective in favourably modifying the symptomatic disease activity in SLE.

Arthritis Nearly 40 million Americans have arthritis, which causes joints to become inflamed or swollen. There are more than 100 types of arthritis that affect the joints and connective tissues (tendons and ligaments) of the body. The left photo shows advanced rheumatoid arthritis that has casued swelling in all of the joints of the hand and wrist. Arthritis sets off a painful, destructive cycle in which inflammation - part of the body's natural defense system - causes tissue damage that the body tries to deal with by creating more inflammation. The inflammation causes swelling, pain, redness and a loss of motion in the joint. Ultimately, it can change the bones and connective tissues, reducing their ability to function. What is Rheumatoid Arthritis?
Rheumatoid Arthritis (RA) is the most debilitating type of arthritis because it can cause deformity and disability. It affects more than 2 million Americans of which 200,000 are juveniles. RA's onset usually occurs between the ages of 20 and 50, but is known to affect older adults too. Women are affected three times as often as men. RA is an immune disorder termed seropositive. This means in many patients with RA the rheumatoid factor (M class) can be identified in their blood (serum). Not everyone with who tests positive for the rheumatoid factor develops the disease. Researchers indicate the disease cannot be inherited but the susceptibility toward development could be, although this has not been proven. The actual cause is unknown. Joints are lined in a membrane called the synovium. In rheumatoid arthritis, the white blood cells, whose normal duty is to provide immunity, turn against the synovial membrane causing inflammation. In turn, inflammation triggers the release of chemicals causing the synovial membrane to thicken. Destructive enzymes in the synovial fluid create a granulated membrane (pannus) that covers the normal joint surface. Over time, these chemicals and enzymes can destroy joint cartilage, bone, tendons, and ligaments. As these structures are compromised, the joint loses it shape and alignment.

What is Rheumatoid Arthritis? What are the symptoms? Who gets RA? What causes Rheumatoid Arthritis? What are some of the available treatments for RA? While there is sadly no cure for Rhumatoid Arthritis, most therapies for reducing symptoms are generally fairly to very effective at easing pain and slowing the progression of the disease. Most treatments center around preventing disability and loss of quality of life as well as providing continual maintenance care to slow (in some cases very significantly) the progression of the disease. How is RA diagnosed? Only your doctor can tell you for sure that you have RA, but the most common methods of diagnosis include an analysis of symptoms, your medical history, lab tests, a physical exam, x-rays, and possibly a test for the rheumatoid factor. The rheumatoid factor is a test that detects an antibody that is found in roughly 80% of people with RA. Please be aware that there is currently no single test that will tell you if you do in fact have Rheumatoid Arthritis. Treating Rheumatoid Arthritis With Tumour Necrosis Factor Doesn't Make Sense
By Dr. Paul Emery Rheumatoid arthritis is one of the most common autoimmune diseases, with a prevalence of about 1%, and it is perhaps the most common reversible disability in the Western world. After 10 years, 50% of people with rheumatoid arthritis in employment no longer work, most losing their jobs in the first 12 months after diagnosis.1 Excessive amounts of the pro-inflammatory cytokines, tumour necrosis factor, TNF interleukin-1 (IL-1), and interleukin-6 (IL-6), mediate most of the pathogenic features of rheumatoid arthritis. Infliximab, the chimeric antibody to tumour necrosis factor , and etanercept, a fusion protein p75 tumour necrosis factor receptor immunoglobulin, have been shown to be very effective in reducing the chronic symptoms and signs of rheumatoid arthritis in patients who fail to respond to conventional treatment with disease modifying drugs.2 Both these molecules produce response rates which are at least as high as those seen with other treatments given for milder disease. Importantly, these drugs have been shown to be effective in patients who were thought to be resistant to all treatment. Before these new drugs such patients were left to deteriorate, resulting in cachexic individuals with destroyed joints, a picture all too familiar to physicians.

Introduction Rheumatoid arthritis affects about 1 percent of our population and at least two million Americans have definite or classical rheumatoid arthritis. It is a much more devastating illness than previously appreciated. Most patients with rheumatoid arthritis have a progressive disability. More than 50% of patients who were working at the start of their disease are disabled after five years of rheumatoid arthritis. The annual cost of this disease in the U.S. is estimated to be over $1 billion. There is also an increased mortality rate. The five-year survival rate of patients with more than thirty joints involved is approximately 50%. This is similar to severe coronary artery disease or stage IV Hodgkin's disease. Thirty years ago, one researcher concluded that there was an average loss of eighteen years of life in patients who developed rheumatoid arthritis before the age of 50.

Mortality at 10 years is 26.7% vs 15% for the average population. Aetiology of RA The Synovium is the centre of the abnormality in the musculoskeletal manifestations of Rheumatoid Arthritis. This is the lining tissue in the joint, and is similar in structure to the lining around the tendons - the tenosynovium, and the lining surrounding Bursae - which are sacs of fluid containing structures that lubricate tendons as they attach to bone. An antibody is a protein made by the B cell lymphocyte of the body, under the stimulus of the T lymphocyte cell.

Search strategy: Using the Cochrane search strategy, studies were found by screening: 1) The MEDLINE CD-ROM database from 1966 to June 2002 and 2) the database from the Cochrane 'Rehabilitation and Related Therapies' Field, the Pedro database up to June 2002. Also, 3) reference checking and 4) personal communications with authors was carried out to retrieve eligible studies. Date of the most recent literature search: June, 2002 Selection criteria: Studies were eligible if they were randomised controlled trials (RCTs) comparing balneotherapy with any other intervention or with no intervention.Included participants all suffered from definite or classical rheumatoid arthritis (RA) as defined by the American Rheumatism Association Criteria (ARA) or by the criteria of Steinbrocker. At least one of the WHO/ILAR core set of endpoints for RA clinical trials had to be among the main outcome measures.
