The retina is the part of the eye which transmits visual information to the brain. Retinopathy of prematurity is a disorder of retinal blood vessel development in the premature infant . The severe form is characterized by retinal vascular proliferation , scarring, retinal detachment , and blindness.

Causes, incidence, and risk factors: The blood vessels of the retina begin to develop three months after conception and complete their development at the time of normal birth. When an infant is born very prematurely, the infant's eye development will be disrupted. In infants who develop retinopathy of prematurity (ROP), the vessels grow abnormally from the retina into the normally clear gel that fills the back of the eye. Here, without support, the vessels are fragile and often hemorrhage into the eye. This is followed by scar tissue development which pulls the retina loose from the inner surface of the eye and draws it toward the center of the globe, producing a retinal detachment. This can reduce vision or, if severe, result in complete

General Discussion Retinopathy of prematurity (ROP) is a potentially blinding disease affecting the retinas in premature infants. The retinas are the light-sensitive linings of the insides of the eyes. In infants born prematurely, the blood vessels that supply the retinas are not yet completely developed. Although blood vessel growth continues after birth, these vessels may develop in an abnormal, disorganized pattern, known as ROP. In some affected infants, the changes associated with ROP spontaneously subside. However, in others, ROP may lead to bleeding, scarring of the retina, retinal detachment and visual loss. Even in cases in which ROP changes cease or regress spontaneously, affected children may have an increased risk of certain eye (ocular) abnormalities, including nearsightedness, misalignment of the eyes (strabismus), and/or future retinal detachment. The two major risk factors for ROP are a low birth weight and premature delivery.

Cochrane reviews are regularly checked and updated if necessary. Background: Retinopathy of prematurity (ROP) causes vision loss in many premature infants each year, despite the advances being made in treatment. In the search for ways to prevent the disease altogether, the exposure of the retina to bright ambient light following premature birth has been a natural hypothesis, since the premature infant normally would be in the dark in-utero environment. Several controlled studies have now addressed this theory. Objectives: To answer the question: "Among very low birth weight infants, what is the effect of reducing early environmental light exposure on the incidence of any "Acute ROP", or "Poor ROP Outcomes"? Search strategy: Searches were made of the Cochrane Neonatal Group Register of Controlled Trials, Medline, EMBASE, the Cochrane Library, previous reviews including cross references, abstracts, conference and symposia proceedings, and expert informants. The search terms used were [retrolental fibroplasia or retinopathy of prematurity] and [light or light/ae or lighting or lighting/ae or light/tu or lighting/st]. Liesenhoff' The retinopathy of prematurity (ROP) screening aims at the complete detection of all preterm infants with threshold ROP. Methods. We developed a software running under Microsoft Windows 3.1 or Windows 95 programmed in Microsoft Access" Basic and Microsoft Visual Basic . The sofhvare allows to register all the preterm infant’s personal data (e.g., name, date of birth, birth weight, gestational age), data on risk factors, ROP findings, neonatological and ophthalmological treatment. The baby’s next examination can be scheduled bedside. Checking the due dates of re-examination and evaluation of data can easily be performed. For the screens are designed in Microsoft Windows manner, the sofbvare can be handled intuitively. The third level of normalization in data base architecture and various automatic plausibility checks guarantee utmost integrity of data and efficacy of database. Results: To date, the software has been used routinely for two years in more than 2000 examinations in over 1000 preterm infants.

Two concerns emerged from the CRYO-ROP extensive study on the natural history of ROP and treatment of threshold ROP. The first of these is failure of peripheral retinal ablation to eliminate all, or nearly all cases, of retinal detachment due to ROP. In the CRYO-ROP Study, 26 percent of eyes with threshold disease in zone II and 78 percent of eyes with zone I threshold disease had an unfavorable structural outcome despite treatment. The second concern is that most children who developed threshold ROP disease had visual acuity worse than 20/40 even if the eye had a favorable structural outcome. Since no other treatment has yet been shown to be effective in preventing blindness from ROP, retinal ablation remains the treatment of choice. The ETROP Study will test whether earlier treatment is more effective than treatment at threshold in improving functional (visual acuity) outcome following ROP, as well as determining whether earlier treatment decreases the probability of an unfavorable structural outcome.

Retinopathy of Prematurity Introduction Retinopathy of prematurity (ROP) is a developmental abnormality of the retina and vitreous in preterm infants which involves disordered vascularisation, cellular maturation and cellular differentiation. It is an important cause of visual impairment and the outcome can be improved if the disorder is detected by screening which allows appropriate treatment and follow-up (RCO/BAPM 1995, Schaffer et al 1992 and Watts 1992). This protocol for screening to detect ROP and its treatment, is based on the Guidelines for Care around preterm birth (NHMRC 1996). Incidence and risk factors.
  • In those that develop ROP, it is usually first detected at 30-45 weeks Postmenstrual age (PMA) and reaches stage 3 at a mean PMA of 37 (range 32-50) weeks.
  • If infants have been screened and ROP does not appear until after 36 weeks PMA, it is unlikely to be severe.
Consequences of disease In some infants this proliferative retinopathy progresses to inflammatory, haemorrhagic, and ultimately fibrotic retinal scarring and detachment, which may lead to blindness (Schaffer et al 1992, Watts 1992). There are four stages of acute ROP (see Appendix) and adverse outcome is associated with development of stage 3 and 4. Diagnosis The aims of screening for ROP are to identify ROP which has the potential to reach stage 3; and severe (stage 3) ROP which may require treatment.

What are the most common problems of premature infants?
Some problems are so common you should expect your baby to have them, especially if s/he is more than four to six weeks early. These include: jaundice, apnea, and inability to nipple or breast feed. Other problems are common, but may or may not occur in your baby. These include: anemia, low blood pressure, difficulty with sugar balance, respiratory distress syndrome (RDS), transient tachypnea of the newborn (TTNB), patent ductus arteriosus (PDA), retinopathy of prematurity (ROP), reflux, hernias, and infection. Less common, but very important problems include: air leak (includes Pneumothorax, Pneumomediastinum and Pulmonary Interstitial Emphysema), necrotizing enterocolitis (NEC), bronchopulmonary dysplasia (BPD), intraventricular hemorrhage (IVH) and periventricular leukomalacia (PVL). return to top
