DESCRIPTION: Retinopathy of prematurity (ROP) is a proliferative disorder of the retinal blood vessels in premature infants. The normal retinal vascularization occurs nasally at approximately 36 weeks of gestation and temporally at approximately 40 weeks of gestation.

  • 65.8% of infants weighing less than 1251 grams at birth, and 81.6% of those weighing less than 1000 grams

In infants who develop retinopathy of prematurity (ROP), the vessels grow abnormally from the retina into the normally clear gel that fills the back of the eye. In newborns born very prematurely, growth of the blood vessels supplying the retina may stop for a period of time. When growth resumes, it occurs in a disorganized fashion. During disorganized rapid growth, the small blood vessels may bleed and eventually lead to scarring.     Retinopathy of prematurity: A systematic review of the literature
    The Fetus and Newborn Committee, Canadian Paediatric Society OBJECTIVE: To assist in the diagnosis of retinopathy of prematurity (ROP) to facilitate treatment in a timely manner to help prevent blindness.
    DATA SOURCES: Systematic review using MEDLINE including the following key words, “retinopathy of prematurity”, “retrolental fibroplasia”, “blind”, “blindness”, “vision screening”, “cryotherapy”, “cryosurgery”, “laser” and “ablative therapy”. The bibliographies of the references found using the above techniques were scanned for references missed in the primary search.
    DATA SELECTION: Eight population-based studies examining the incidence and severity of ROP were identified. Other studies of ROP were included because they contributed to an understanding of the natural history, treatment or long term outcome of ROP.

        What is retinopathy of prematurity?
        Retinopathy of prematurity (ROP) is a disorder of the blood vessels of the retina (the light sensitive part of the eye). ROP is most common in premature babies. Generally, the more premature the baby and the lower the birthweight, the greater the risk for developing ROP. This disorder has in the past been called retrolental fibroplasia.

    Visual disability and blindness from severe retinopathy of prematurity (ROP) may be prevented with timely treatment during the evolution of stage 3 ROP.1 Accordingly, infants at risk must be examined to discover and to follow the development of ROP.
    The Canadian Ophthalmological Society, the Canadian Association of Pediatric Ophthalmologists and the Canadian Paediatric Society recommend the following guidelines for screening premature infants for ROP. Risk group
    • Infants with a birth weight of 1500 g or less
    Examiners The examiner should be a skilled observer in the recognition of ROP, usually an ophthalmologist. Schedule of assessments
    • The first examination should be performed 4 to 6 weeks after birth.
    • A follow-up examination should be carried out approximately 2 weeks after every examination that reveals no ROP.5.

    Retinopathy of Prematurity
    Retinopathy of Prematurity (ROP), also known as retrolental fibroplasia, is a potentially blinding condition affecting the retina of newborns. The factors that put infants at greatest risk of developing ROP are low birth weight (less than 3.5 pounds) and premature delivery (26-28 weeks). In babies born prematurely, the growth and development of normal blood vessels in the retina is halted and abnormal vessels may begin to develop.

    Strabismus is a problem caused by one or more improperly functioning eye muscles, resulting in a misalignment of the eyes. The brain controls the eye muscles, which keep the eyes properly aligned. In a very short time, the brain permanently suppresses vision from the turned eye, causing a weak or amblyopic eye.

    Retinopathy of Prematurity
    ROP is a disorder of the developing retinal vasculature that occurs with interruption of the forming retinal vessels. Constriction and obliteration of the advancing capillary bed are followed by neovascularization of the retina, which can extend into the vitreous. The most serious and feared complication of ROP is retinal detachment. ROP has previously been known as RLF- Retrolental Fibroplasia, a very advanced form or ROP with end stage fibrosis and scarring behind the lens.
    Development of ROP: It is postulated that there are two events that occur
  • Vasoconstriction and obliteration of the capillary network in response to a vascular insult ( possibly high supplemental O2 concentration) Vasoproliferation- possibly a response by the hypoxic retina to an angiogenic factor released by the insult — thought that hypoxia can cause an overexpression of VEGF that can induce abnormal retinal angiogenesis.
  • Etiology of ROP: multifactorial and still unclear
  • Oxygen administration originally was thought to contribute to the development of ROP. This is now being debated. The STOP ROP- Supplemental Therapeutic Oxygen to Prevent ROP study investigated whether supplemental therapeutic oxygen for premature infants reduces the proportion of infants that progress to threshold ROP. The study found that the more liberal use of oxygen actually decreased the risk of progression to threshold ROP in these infants from 48% to 41%. Threshold ROP is defined as disease progression to the point of necessitating peripheral retinal ablation therapy.

    Prematurity Retinopathy of prematurity or ROP is a retinal disorder found exclusively in premature infants. The incidence of ROP increases for younger and smaller babies. It is normally not seen in infants weighing more than 1500 grams or about three pounds five ounces. With ROP, the normal blood vessels of the retina, which is the nerve tissue in the back of the eye, do not complete their growth until the end of a normal full term pregnancy. In very small babies, the blood vessels may stop growing when they are born. When they resume their growth, it can be in an abnormal fashion which causes damage to the retina. A screening examination schedule has been determined to aid in detecting ROP and prevent its progression when possible. Eye examinations are usually performed in the hospital nursery with the use of eye drops to dilate the pupils so that the retina can be examined. If any ROP is found, follow-up evaluations are recommended over the following weeks at intervals determined by the stage of the disease. In most cases, the ROP will resolve spontaneously as the blood vessels complete their growth pattern. In some cases, the ROP reaches a certain stage for which treatment becomes necessary. This treatment is in the form of laser therapy to the retina. It has been shown that treatment at a certain stage of ROP can decrease, though not eliminate the chances of retinal damage and subsequent poor vision.

The sequelae of untreated or non-responsive ROP can be devastating and may often be associated with other disabilities, and so steps to limit the condition are of vital importance for the individual children concerned, for their families and for the healthcare system that provides for them. Definition and Classification Threshold ROP is 5 contiguous clock hours of stage 3 retinopathy in zones I or II (8 clock hours or more if not contiguous), in the presence of 'plus' disease (dilated retinal arterioles and congested veins, abbreviated as '+'). If untreated, it is associated with a poor anatomical outcome in approximately 50% of cases. Pre-threshold ROP is usually defined as any retinopathy in zone 1, or Stages 2+ or 3 in zone II. Stage 3 ROP has a greater likelihood of an anatomically unfavourable outcome if present in the zone 1 (59% poor outcome)than in zone II (44% poor outcome if stage 3+
