Retinoblastoma Treatment Group The UIC Retinoblastoma Treatment Group is a multidisciplinary team of specialists who helps treat retinoblastoma, a blinding childhood cancer.
The cancer can be hereditary or sporadic, involving all the cells of the whole body or only the cells of eye. In the hereditary form, multiple tumors are found in both eyes, while in the non-hereditary form only one eye is affected and by only one tumor. Among children with retinoblastoma, 60% have cancer in one eye while 40% have tumors in both eyes.

The Retinoblastoma Treatment Group has incorporated new treatment therapies, including neoadjuvant chemotherapy based on MRI studies, an aggressive transpulillary thermotherapy (TTT) protocol, intensity modulated radiotherapy (IMRT) for macular recurrent post-chemotherapy macular disease, and subconjunctival carboplatin. Removal of the eye, or enucleation, is still recommended in situations where eyes contain large tumors and where there is no expectation for useful vision. The appearance of infants requiring enucleation is exceptional, thanks to Dr. Mark Duffy, who heads the Occuloplastic Surgery Service.

Oncogenes and the retinoblastoma pathway The retinoblastoma gene, RB , was the first tumour-suppressor gene to be identified in children with hereditary retinoblastoma . Its protein product, RB, was subsequently found to be functionally inactivated in several other human tumour types. During the late 1980s and early 1990s, RB was identified as a cellular target for viral oncoproteins, and significant steps were made towards the understanding of the role of RB in the regulation of cell proliferation and tumour formation. In 1988, Whyte et al . and DeCaprio et al . discovered that E1A and the large tumour (T) antigen, the transforming oncogenes of adenovirus and SV40 virus (which cause tumours in rodents), are found in a complex with RB. Soon after, Dyson et al . demonstrated that the E7 protein of human papillomavirus (HPV) type 16 et al . showed that E1A disrupted cellular complexes containing the transcription factor E2F, releasing free E2F and stimulating its transcriptional acitivity. This required the same domain of E1A that is responsible for RB binding, immediately indicating close ties between the functions of E2F and RB. Chellappan et al . and Bandara and La Thangue went on to identify E2F as the first cellular target for RB. Hamel

Our Retinoblastoma Team Retinoblastoma is the most common type of eye tumor seen in children. Radioterapia come unico trattamento in pazienti al I, II e III stadio; come supporto nei pazienti trattati con terapia chirugica negli stadi IV e V.

About the disease
Retinoblastoma is cancer of the thin layer of nerve tissue that lines the back of the eye. This layer is called the retina, which is the light-sensitive portion of the eye that forms images. Retinoblastoma may affect one eye (unilateral), both eyes (bilateral), and in very rare cases, the brain (trilateral). Most often it is found in children under 5 years old. Retinoblastoma does not usually spread to nearby tissues or other parts of the body. When treated early, 95 percent of the cases can be treated and vision may be spared.
Cancer types
There are two types of retinoblastoma: hereditary and non-hereditary. Hereditary retinoblastoma makes up 30 to 40 percent of retinoblastoma cases. It can be found in one or both eyes, and is usually found in younger children. When the disease is found in both eyes, it is always the hereditary form. Because of the genetic basis of this disease, siblings need to be examined by an eye specialist, and the parents should meet with a genetic counselor. A child with heriditary retinoblastoma also is at greater risk for developing other kinds of cancer elsewhere in the body later in life. These children should continue to have medical check-ups even after the disease has been treated. Non-hereditary retinoblastoma affects one eye only, and is generally found in older children. There is a slight chance that these children are at greater risk for developing cancer elsewhere in the body later in life. As a result, they should continue to have medical check-ups even after the disease has been treated.

When it occurs without a family history, we are unaware of any risk factors. You can get RB in one or both eyes. Generally children who have it in both eyes have the inherited form, while those with it in only one eye probably developed the disease with no known cause. Parents with the hereditary form of RB have a 50 percent chance of having a child with RB. For this reason, children with a family history of RB are routinely screened for this disease.
